        Perception, attitude, and demand for Korean medicine and Western medicine collaborative treatment of medical occupational groups in Korea: A scoping review

        Background: In Korea, there exists a dual medical license system whereby both traditional Korean medicine and Western medicine doctors practice independently. In 2009, via medical law revision, cross employment was allowed to activate Korean medicine and Western medicine collaborative treatment (KWCT). Despite its several advantages, there are several barriers to a well-organized KWCT. To activate KWCT, we investigated perception, attitude, demand of medical occupational groups and research gap via scoping review. Methods: Scoping review is an emerging methodology for knowledge synthesis adopting the Arksey and O’Malley framework. The research question was “What is known about perception, attitude, demand, and research gap on KWCT?” We searched articles published from 2009 to 2019 in 7 Korean and 4 English databases. We conducted descriptive and qualitative thematic analysis and presented the research gap. Results: From 1305 articles, 6 quantitative and 3 qualitative studies were included. In quantitative studies, perception and attitude of medical occupational groups did not change markedly. For better perception and attitude, experiencing, mutual understanding, and respect should precede. Academic exchange, education, supporting KWCT research and legal/institutional/administrative support is also important. In qualitative studies, education course, guidelines, trust, safety, academic integration, and scientification were also required. We found several research gaps about KWCT especially detailed on-site demand and activation strategy about KWCT. Conclusion: We investigated perception, attitude, and demand on KWCT. Based on research gap in our scoping review, quantitative studies using validated questionnaire and in-depth interview are needed to identify on-site demand to improve KWCT implementation.         Thepurposeofthisstudywastoestablishnationalstandardmethodofpolycyclicaromatichydrocarbons(PAHs)insediment.ThesampleswereextractedusingSoxhletextractionoracceleratedsolventextraction(ASE)andinjectedwithbenzo[e]pyrene-d<sub>12</sub>asasurrogateinternalstandard.Napthalene-d<sub>8</sub>,acenaphthene-d<sub>10</sub>,phenanthrene-d<sub>10</sub>,chrysene-d<sub>12</sub>,Benzo(a)pyrene-d<sub>12</sub>,Indeno(1,2,3-c,d)pyrene-d<sub>12</sub>wereusedasinternalcleanupstandards.P-terphenyl-d<sub>14</sub>wasinjectedasarecoveryinternalstandardandanalyzedusinggaschromatograph-massspectrometer(GC-MS).Aninterlaboratorycomparisonstudywasconducted,inwhichthethreeselectedparticipatinglaboratoriesquantitativelyanalyzedseveralPAHsincertifiedsedimentsamples:IAEA-383,IAEA-417,IAEA-408,NIST-1944.Internalqualitycontrolwasconductedindividually.TheresultsreportedforPAHsbythethreelaboratorieswereacceptable,witharangeofrelativestandarddeviations(RSD)of4.0-24.0%inIAEA-383(16.0ng/g-290.0ng/g).ResultsobtainedfortheIAEA-417(150.0ng/g-7,700.0ng/g)andNIST-1944samples(390.0ng/g-9,700.0ng/g)werealsofoundacceptablewitharangeofRSDof5.4-27.9%and2.4-25.3%,respectively.AlthoughtheresultsofIAEA-408(3.3ng/g-84.0ng/g)wereslightlyhigherthanthoseofotherRSDs,theywereacceptablewitharangedof2.5-38.7%attwoofthelaboratories.Thelimitofquantificationofthismethodwasfoundtobe0.003mg/kg,andrecoveryratesrangedfrom50.0%to120.0%.TheRSDwaslessthan25%,andtheaccuracyrangedfrom60.0%to135.0%insediment.

        심부전의 한약 임상연구에 활용된 한약재에 대한 기구축 DB(K-HERB NETWORK)를 활용한 네트워크 분석

        Objectives: Heart failure is a chronic disease with increasing prevalence rates despite advancements in medical technology. Korean medicine utilizes herbal prescriptions to treat heart failure, but little is known about the specific herbal medicines comprising the network of herbal prescriptions for heart failure. This study proposes a novel methodology that can efficiently develop prescriptions and facilitate experimental research on heart failure by utilizing existing databases. Methods: Herbal medicine prescriptions for heart failure were identified through a PubMed search and compiled into a Google Sheet database. NetMiner 4 was used for network analysis, and the individual networks were classified according to the herbal medicine classification system to identify trends. K-HERB NETWORK was utilized to derive related prescriptions. Results: Network analysis of heart failure prescriptions and herbal medicines using NetMiner 4 produced 16 individual networks. Uhwangcheongsim-won (牛黃淸心元), Gamiondam-tang (加味溫膽湯), Bangpungtongseong-san (防風通聖散), and Bunsimgi-eum (分心氣飮) were identified as prescriptions with high similarity in the entire network. A total of 16 individual networks utilized K-HERB NETWORK to present prescriptions that were most similar to existing prescriptions. The results provide 1) an indication of existing prescriptions with potential for use to treat heart failure and 2) a basis for developing new prescriptions for heart failure treatment. Conclusion: The proposed methodology presents an efficient approach to developing new heart failure prescriptions and facilitating experimental research. This study highlights the potential of network pharmacology methodology and its possible applications in other diseases. Further studies on network pharmacology methodology are recommended.
