        Ciliated foregut cyst of the gallbladder

        Ciliated foregut cyst of gallbladder is a very rare benign cystic lesion. A 39-year-old woman was referred to our hospital after abdominal ultrasonography revealed a cystic lesion of gallbladder. On abdominal ultrasonography and computed tomography, a unilocular cystic lesion was found at right upper quadrant with attachment to the gallbladder neck. The gallbladder with cystic lesion was resected through laparoscopic cholecystectomy. The cystic lesion revealed a unilocular cyst with ciliated cuboidal or columnar epithelium and abundant goblet cells. Pathologic examination is essential to distinguish from other cystic lesions of the gallbladder and avoid unnecessary additional treatment. In the current case report, we presented the clinico-pathologic findings of the ciliated foregut cyst of the gallbladder and review of literature.

      • Docetaxel-cisplatin-fluorouracil Induction Chemotherapy Followed by Concurrent Chemoradiotherapy Versus Concurrent Chemoradiotherapy for Locally Advanced Head and Neck Cancer :A Meta-analysis

        서론: 국소 진행성 두경부암 환자에서 선행 항암요법 후 동시 항암화학방사선요법은 원격 전이를 줄이고, 국소 병변을 줄여 방사선 치료의 효과를 높이거나, 기관의 기능을 보존할 목적으로 시도된다. 선행 항암요법의 약제로는 docetaxel, cisplatin, fluorouracil (DPF) 삼제요법이 가장 효과적인 것으로 알려져 있다. 선행 항암요법 후 동시 항암화학방사선요법과 표준치료인 동시화학방사선요법을 비교한 3상 연구들이 모두 선행 항암요법이 더 낫다는 결과를 보여 주지 못하였지만, 이 연구들은 충분한 환자를 모집하지 못하고 조기 종료된 불완전한 연구라는 한계가 있었다. 이에 저자들은 DPF 선행 항암요법 후 동시 화학방사선요법과 표준치료인 동시 화학방사선요법을 비교하는 메타분석을 시행하였다. 대상 및 방법: 체계적 문헌고찰을 통해 국소진행성 두경부암 환자를 대상으로 시행된 DPF 선행 항암요법 후 동시화학방사선요법과 현재 표준치료인 동시화학방사선요법을 비교한 5개의 3상 연구 결과를 분석하였다. 대상환자는 862 명이었고, 분석 결과 DPF 선행 항암요법 후 동시화학방사선요법은 표준 치료와 비교하였을 때 반응률, 2년 및 3년 생존율, 2년 및 3년 무진행 생존율, 점막염 및 빈혈 발생 빈도에서 통계적으로 유의한 차이가 없었다. 하지만, 완전관해율과 3~4도의 백혈구감소증 및 혈소판 감소증의 빈도는 선행 항암요법 시행군에서 더 높았다. 결론: 국소진행성 두경부암의 치료에서 DPF 선행 항암요법 후 동시 항암화학방 사선요법을 시행하는 것은 표준치료인 항암화학방사선요법에 비해 생존율 개선을 보이지 못하였다. 선행항암치료를 추가하는 것이 특정 환자군에서 효과가 있을지에 대해서는 추가적인 연구가 필요하다.

        In establishing the fatigue design criterion and estimating the reliability of the spot welded lap joint thin sheet structures such as the automobile and train, it is necessary to estimate the fatigue strength of spot welded lap joints systematically and reasonably. So far, many investigators have numerically and experimentally studied on the method of systematic fatigue estimation for various spot welded lap joints. But, most of results did not consider actual fracture mechanical mode according to the joint angle at spot welded lap joint.<br/> In this study, by considering nugget edge of the spot weld part of the IE-type spot welded lap joint to the ligament crack, fatigue strength of various IB-type spot welded lap joints was estin1ated with the stress intensity factor (S. I. F.) K which is fracture mechanical parameter and fracture mechartical mode analysis according to various joint angle. We could find that mode Ⅲ(out of plane shear mode) is main parameter at IB type spot welding when joint angle is increased and fatigue strength evaluation of the II3-type spot welded lap joints by ??K- N relation is more effective than ??P- Nr relation<br/>

        This study describes hybrid power train design concept for small cart. Single cylinder spark ignition (SI) engine was applied as combustion engine and brushless DC motor (1.5㎾) as supplementary electric power. In order to improve robustness of vehicle body design, modal mode analysis was carried out by using Nastran. The vehicle dynamic performance was pre-calculated based on estimated rolling and air resistance. In order to control the power mode, the electric clutch and optimized reduction gear were applied. To control motor power, the motor control unit (MCU) was applied and control strategy was studied for different driving pattern. To improve fuel economy, the braking energy recovery system is included in power control logic based upon brake pedal signal. Considering vehicle curb weight and power balance between combustion engine and motor, the 48V conventional Pb-acid battery was applied.

        한국 금융산업의 학력별 노동시장구조 변화

        2000년 이후 금융산업의 학력별 노동시장구조를 살펴보면 전체적인 고용자수는 거의 변동은 없는데 고학력노동(대졸이상, 전문대졸 이상)은 증가하고 저학력노동(고졸이하)은 감소하고 있고, 학력별 임금격차는 지속적으로 확대되고 있다. 학력을 근거로 고학력노동과 저학력노동으로 구분하여 생산요소를 물적자본(K), 저학력 노동(L)과 고학력 노동(E)로 하는 다요소 콥-다글라스 생산함수를 가정하고, 외생변수로 산출량(Q), 세계화(G)와 기술진보(t)를 포함한 학력별 노동수요함수를 유도하였다. 이 함수를 바탕으로 로그선형회귀추정모형을 구성하고 학력별 노동수요에 영향을 미치는 요인분석을 시행하였다. 추정결과 생산요소의 상대가격(R/WL,R/WE,WL/WE)과 세계화(G)는 금융산업의 학력별 노동수요에 거의 영향을 미치지 못하였다. 반면 산출량과 기술진보 등 금융산업의 외생적 요인 변화가 금융산업의 학력별 노동수요에 통계적으로 유의미하게 영향을 미치는 것으로 나타났다. 산출량변화는 금융산업의 학력별 노동수요에서 저학력노동 수요에 비해 고학력노동 수요에 미치는 영향이 더 크지만 그 영향력은 미미하였다. 반면 기술진보는 고학력노동에서 고용증가효과를, 저학력노동에서 고용구축효과를 유발하여 금융산업의 노동시장이 고학력노동 중심으로 구조변화가 진행 중인 것으로 판단된다. 이런 연구결과를 바탕으로 금융산업의 고용에 대해 다음과 같은 시사점을 제공한다. 첫째, 상대임금 변화는 학력별 노동수요에 거의 영향을 미치지 않기 때문에 이와 관련된 정책은 상당히 제한적일 수 있다. 둘째, 기술변화와 세계화가 진전되고 있는 상황에서 고학력노동의 고용증가에 부응하는 인재양성정책이, 반면 저학력노동의 고용감소에 따른 고용안정정책이 필요하다. Looking at the labor market structure by education level in the financial industry since 2000, the overall number of employees has remained almost unchanged, but the number of high-educated labor (college graduates or higher) has increased, and the wage gap by education level has continued to extended. Based on education level, high-educated and low-educated labor were classified, assuming a multi-factor Cobb-Daglas production function that uses production factors as physical capital (K), low-educated labor (L), and high-educated labor (E), and induced labor demand functions for each education level including output (Q), globalization (G), and technological progress (t). Based on this function, a linear regression estimation model was constructed, and factor analysis affecting labor demand by education was implementedd. As a result of the estimation, the relative price of production factors(R/WL,R/WE,WL/WE) and globalization () had little effect on the labor demand by education level in the financial industry. On the other hand, the changes in exogenous factors, were found to have a significant effect on the labor demand by education level in the financial industry. The change in output had a little more impact on the demand for high-educated labor than that for low-educated labor in the financial industry, but its influence was insignificant. Technological progress causes employment increase in high-educated labor and employment decrease in low-educated labor, so the labor market in the financial industry is undergoing structural changes centered on high-educated labor. Based on these estimation results and interpretation, the following implications are provided for employment in the financial industry. First, since changes in relative wages have little effect on educational labor demands in the labor market, policies related to this may be quite limited. Second, in the face of technological change and globalization, there is a need for a policy to cultivate talent in response to the increase in employment of high-educated labor, while a policy to stabilize employment due to the decrease in employment of low-educated labor is needed.

      • 도로 하중조건을 고려한 상용차 판스프링의 피로설계 기준연구

        Suspension system of vehicle have enough endurance during its life time to protect passenger. Spring is one of major part of vehicle. Thus, a fatigue strength evaluation for leaf spring based on road load response was carried out. At first, strain of leaf spring is measured on the city condition and proving ground condition. And next, the damage analysis of road load response data was carried out. And fatigue test of leaf spring were also carried out. Based on ε-N life relation, fatigue life of leaf spring was evaluated at belgian mode, city mode and drawing test specification. After that, it is compared the design life of leaf spring and evaluated fatigue life by belgian mode, city mode and drawing test specification. From the above, the maximum load-fatigue life relation of leaf spring was defined by test. and new test target of belgian mode and city mode was proposed to accept design specification of leaf spring. It is expect that proposed test target can verify leaf spring fatigue endurance at specific road condition.
