Purpose: Rapid and accurate assessment of impaired consciousness is very important, especially for critically ill patients. Therefore, the Glasgow coma scale (GCS) has been widely adopted in the assessment of adult and pediatric comas. But the GCS should not be used for small children who show various normal responses according to their developmental stage. Therefore, the objectivity of pediatric coma measurements was verified by applying the modified GCS to patients in a pediatric intensive care unit (PICU) at a university hospital in Korea. Methods: The level of consciousness was evaluated for 200 cases who were admitted to a PICU from July 4, 2009 to September 18, 2009 and could show a verbal response, with our modified GCS for children. In addition, and we confirmed the frequency analysis and Kappa statistics with SPSS/WIN 17.0. Results: Kappa statistics which show inter-observer reliability were very good for all components (eye opening, verbal, and motor score) and was good for total GCS scores. Conclusion: It is concluded that our modified GCS is reliable. Therefore, reliable assessment for the level of consciousness is clinically practicable if enough training is supported.

<B>Purpose</B><P>To compare the diagnostic performance of multirow-detector computed tomography (MDCT) and magnetic resonance imaging (MRI) in the differentiation of intraductal papillary mucinous neoplasms (IPMNs) from other pancreatic cystic masses.</P><B>Materials and Methods</B><P>A total of 53 patients with pathologically proven pancreatic cystic lesions who had undergone MDCT and MRI were included in this study. Two radiologists analyzed the morphologic features of the lesions and graded the lesion conspicuity on each examination. The readers assigned their confidence level regarding the differentiation of IPMN from other lesions and predicting ductal communication of the lesion. The radiologists' diagnostic confidence was compared using receiver operating characteristic (ROC) analysis.</P><B>Results</B><P>The Az values for each observer for predicting ductal communication of the lesion and differentiating IPMN from other lesions were as follows: For MRI they were respectively 0.949 and 0.995 for reader 1, and 0.916 and 0.932 for reader 2. For MDCT they were respectively 0.790 and 0.875 for reader 1, and 0.774 and 0.850 for reader 2. In addition, for differentiating IPMNs from other lesions, MRI was significantly more accurate than MDCT (P < 0.05) for one observer, but for the other observer there was no significant difference between the two examinations (P = 0.059). For predicting ductal communication of the cystic lesions for both observers, MRI was significantly more accurate than MDCT (P < 0.05). The weighted κ values indicate good agreement (κ = 0.61) between observers for MDCT, and excellent agreement (κ = 0.82) for MRI.</P><B>Conclusion</B><P>Pancreatic MRI shows better diagnostic performance than MDCT for differentiating IPMNs from other cystic lesions of the pancreas. The whole plants of Selaginella tamariscina, Orostachycis japonicus, the cortex of Ulmus mandshuria, and the wood of Alnus japonica have been used as folk medicine for treating concer. The cytotoxic activity of these plants were tested using a colorimetric tetrazolium assay (MTT assay). S. tamariscina and A. japonica showed mild IC_50 value, comparing with O. japonicus and U. mandshurica. So, MeOH extracts of S. tamariscina and A. japonica were partitioned into CHCI_3, EtOAc and n-BuOH, successively. The CHCI_3, EtOAc and BuOH fractions of S. tamariscina and A. japonica showed low percent of survival against P388 and MKN45 cells respectively. To isolate active components, they were subjected to silica gel column chromatography. Compound I was obtained from EtOAc extracts of S. tamariscina and identified as ametoflavone by chemical and spectral analysis. Amentoflavone inhibited the survival of P388 cells dose dependently, while not clearly inhibited that that of MKN45 cells.

This study was done to use a guideline adaptation process to develop a Korean evidence-based diabetic foot care clinical practice guideline for diabetic foot prevention and management. Methods: The guideline adaptation process was conducted according to the guideline adaptation manual developed by the National Evidence-Based Healthcare Collaborating Agency. The process consists of three main phases, with 9 modules including a total of 23 steps. Results: The newly developed diabetic foot care clinical practice guideline consisted of an introduction, description of diabetic foot, summary of recommendations, recommendations, references, appendices, and glossary. There were 165 recommendations in 4 sections (risk assessment for diabetic foot ulcers, prevention of diabetic foot ulcers, wound assessment of diabetic foot ulcers, and management of the diabetic foot). In grading by recommendations, for A there were 30 (18.2%), B, 8 (4.8%), C, 30 (18.2%) D, 97 (58.8%). Conclusion: This guideline can be used as educational material for healthcare workers and diabetic patients. It can also be utilized as a practice guideline for healthcare workers in the hospital and community setting.

<P>Summary</P><P>Context </P><P>Dideoxy sequencing is the most commonly used method for detecting the BRAF<SUP>V600E</SUP> mutation in thyroid cancer and melanoma. However, this gold standard method often makes less definite results in detecting the BRAF<SUP><B>V600E</B></SUP> mutation when there are relatively low amounts of the mutant template in biopsy specimens, which are invariably contaminated with normal tissues. Pyrosequencing, which measures the incorporation of each of the four nucleotides at each template position and indicates the amounts of mutant template present, may be more useful in such situations.</P><P>Objective </P><P>To investigate the diagnostic efficiency of pyrosequencing for the mutant BRAF allele in ultrasound (US)-guided fine needle aspiration biopsies (FNABs) of thyroid incidentalomas.</P><P>Design, setting and subjects </P><P>A total of 101 thyroid incidentaloma cases were included prospectively. Cytological diagnoses of the FNAB samples were made according to the American Thyroid Association (ATA) guidelines, 2006. The presence of the BRAF<SUP><B>V600E</B></SUP> mutation was investigated by pyrosequencing and dideoxy sequencing.</P><P>Results </P><P>On the basis of cytological analysis, the thyroid incidentalomas were classified into benign (<I>n</I> = 43), malignant (<I>n</I> = 30), indeterminate or suspicious neoplasm (<I>n</I> = 24), and nondiagnostic (<I>n</I> = 4) categories. Pyrosequencing detected the BRAF<SUP><B>V600E</B></SUP> mutation in 30 cases: 22 malignant cases, 7 indeterminate cases, and 1 nondiagnostic case. Dideoxy sequencing also detected the BRAF<SUP><B>V600E</B></SUP> mutation in 28 of the same cases but failed to clearly distinguish the mutant allele from the wild-type allele in one indeterminate case and one nondiagnostic case. Histopathological analysis ascertained that all BRAF<SUP><B>V600E</B></SUP>-positive cases were papillary thyroid carcinomas.</P><P>Conclusions </P><P>Pyrosequencing may be suitable for detecting the BRAF<SUP><B>V600E</B></SUP> mutation in thyroid incidentaloma and may be superior to dideoxy sequencing when low amounts of the mutant template are present in the biopsy.</P>
Growth of GaAs Nanowires on Si Substrates Using a Molecular Beam Epitaxy

<P>Au-catalyzed GaAs nanowires were grown on Si substrates by vapor-liquid-solid growth method using a molecular beam epitaxy (MBE). The MBE growth could produce controlled crystalline orientation and uniform diameter along the wire axis of the GaAs nanowires by adjusting growth conditions including growth temperature and V/III flux ratio. Growths of GaAslang001rang as well as GaAslang111rang nanowires were observed by transmission electron microscopy and scanning electron microscopy. Epitaxially grown GaAslang111rang nanowires on a Si(111) substrate were verified through x-ray diffraction out-of-plane 2thetas/omega-scans. A strong room-temperature photoluminescence (PL) was observed from the epitaxially grown GaAslang111rang nanowires on a Si(100) substrate. 2'-Hydroxycinnamaldehyde, which inhibits farnesyl-protein transferase (FPTase), has been isolated from the stem bark of Cinnamomum cassia Blume. The biologically active agent in the extract has been purified by silica column chromatography and HPLC. The structure of the isolated compound was elucidated on the basis of 500㎒ NMR experiments.
