        Selective laser melted titanium implants: a new technique for the reconstruction of extensive zygomatic complex defects

        The restoration of extensive zygomatic complex defects is a surgical challenge owing to the difficulty of accurately restoring the normal anatomy, symmetry, proper facial projection and facial width. In the present study, an extensive post-traumatic zygomatic bone defect was reconstructed using a custom-made implant that was made with a selective laser melting (SLM) technique. The computer-designed implant had the proper geometry and fit perfectly into the defect without requiring any intraoperative adjustments. A one-year follow-up revealed a stable outcome with no complications.

        구강 편평 상피 암종에서 TGF-β1과 MMP-2의 발현과 경부 임파절 전이 간의 상관 관계에 관한 연구

        Several matrix metalloproteinases (MMPs) have been shown to play an important role in the invasion and metastasis of oral squamous cell carcinoma(OSCC). The members of the TGF-βsignaling pathway are being considered as predictive biomarkers for progressive tumorigenesis and molecular targets for the prevention and the treatment of cancer and metastasis. The aim of the present study was to find the clinical significance of the expression of TGF-β1 and MMP-2 related to the regional lymph node metastasis in OSCC. This study included 76 cases of primary OSCC, of which 42 cases showed regional lymph node metastases. Immunohistochemistry was used for the localization of protein. The relation between the expression of each protein and clinical variables was statistically evaluated. In results, the expression of TGF-β1 both main mass with lymph node metastasis and without lymph node metastasis was found not to be statistically significant (p>0.05). The expression of MMP-2 was found to be statistically significant related to regional lymph node metastasis (p<0.05). When compared the expression in the metastatic lymph node, TGF-β1 was significantly highly expressed than MMP-2 (p<0.05). In conclusion, the expression of MMP-2 was significantly elevated in patients with lymph node metastasis as compared to the patients without lymph node metastasis, which could be useful in predicting the risk of lymph node metastasis in OSCC.

        Salivary exosomal microRNAs as biomarkers for head and neck cancer detection—a literature review

        Background: MicroRNAs (miRs) are small, non-coding mRNA molecules which regulate cellular processes in tumorigenesis. miRs were discovered in extracellular environment and biological fluids, carrying marks of head and neck squamous cellcarcinoma (HNSCC). They were also identified in abundance in salivary exosomes, in which they are protected by exosomelipid barrier against enzymatic injuries and therefore, the accuracy of exosomal miR-based cancer detection increase. Thissystematic review aimed to reveal and inventorize the most reliable exosomal miRNAs in saliva samples which can be usedas novel biomarkers for early detection of HNSCC. Materials and methods: A systematic literature search, according to PRISMA guideline, was performed on Pubmed andGoogle Academic libraries, based on specific keywords. Original articles published between 2010 and 2021 were selected. The quality of each paper was assessed using the Quality Evaluation Scoring Tool. Results: At the end of selection process, five studies met the inclusion criteria. These studies analyzed twelve salivaryexosomal miRs, presenting different methods of exosome and miR identification for HNSCC detection. A comprehensiveexplanation of the miR pathways of action was drawn and illustrated in this review. Conclusion: Exosomal miRs are promising biomarkers for oral cavity and oropharyngeal cancer detection. miR-10b-5p, miR486-5p, miR-24-3p and miR-200a stand as the most useful ones in saliva sample examination

        The ethnic difference of the prevalence of SfaN polymorphism in the nonsyndromic cleft palate

        비증후군성 구순구개열은 가장 빈도가 높은 선천성 기형 중의 하나로 특히 한국이나 일본과 같은 극동 지방에서 높은 발생율을 보이고 동유럽에서는 드물게 보고되고 있다. 이러한 인종에 따른 차이는 이 질환에 유전적인 배경이 있음을 의미한다. 본 연구의 목적은 한국인의 비증후군성 구개열과 연관이 있다고 알려진 SfaN1 단일 염기 다형성증의 발현빈도가 한국인 구개열 가족과 루마니아 구순구개열 가족 사이에 통계적으로 유의할만한 차이가 있는지를 알아보기 위하여 시행하였다. 한국인 26가족과 루마니아 18가족을 대상으로 하였다. 전체 인원수는 한국인의 경우 78명이었고 루마니아의 경우 41명이었다. 유전자 서열분석에 사용된 샘플은 각 참여자의 혈액이나 타액을 채취하여 분석하였다. SfaN1 단일 염기 다형 성증은 TCF-β3 유전자의 5번 인트론에서 관찰된다 (A18141G). 결과를 보면 한국인과 루마니아인의 비증후군성 구개열 가족 사이에는 통계적으로 유의할만한 차이가 인지되었다. 전체 샘플 중에서 AA allele는 한국인에서는 18명 (23.1%)이었으나 루마니아는 27명 (65.9%)이었다. AS allele는 한국인에서는 27명 (34.6%)이었으나 루마니아에서는 13명 (31.7%)이었다. GG allele는 한국인에서는 33명 (42.3%)이었으나 루마니아에서는 1명 (2.4%)이었다. 두 집단 사이의 차이는 통계적으로 유의하였다 (p<0.001). 결론적으로 한국인과 루마니아인의 비증후군성 구개열 가족 사이에 SfaN1 단일 염기 다형성증의 발현빈도는 한국인에서 통계적으로 유의할만하게 높게 나타났으며, 이는 한국에서 루마니아보다 비증후군성 구개열의 빈도가 높게 나타나는 현상을 부분적으로 설명하여 주는 것으로 사료된다. Nonsyndromic cleft lip and/or palate (NSCLP) is one of the most common congenital deformities and its prevalence in Far East Asia, such as within Korean and Japanese populations, is relatively high. However, in the eastern part of Europe, clefts are relatively rare situations. These ethnic differences infer a genetic background of the disease. The objective of this study was to compare the frequency of single nucleotide polymorphism (SNP) in TGF-β3 between Korean and Romanian cleft families. Korean cleft families samples were collected from twenty-six families (n=78) and Romanian cleft families samples were collected from eighteen families (n=41). For sequencing, the blood or saliva of the subjects was sampled. A single nucleotide plolymorphism was observed in the intron 5 of TGF-β3 (A18141G). The frequency of each allele was significantly different between the Korean and Romanian samples. The AA allele was present in 18 out of 78 Korean samples (23.1%) and in 27 out of 41 Romanian samples (65.9%). The AG was present in 27 (34.6%) out of 78 Koreans and in 13 (31.7%) out of 41 Romanians. The GG was found in 33 (42.3%) Koreans and in 1 (2.4%) Romanian. The difference between the groups was significant (p<0.001). In conclusion, the frequency of observed SNP was significantly different between the two countries. SNP in TGF-β3 in the Korean population seemed to have a higher possibility of occurrence for nonsyndromic cleft palate than the Romanian population.

        Comparison of the Physical Properties and in vivo Bioactivities of Flatwise-Spun Silk Mats and Cocoon-Derived Silk Mats for Guided Bone Regeneration

        The main flaw of silkworm cocoon originated membranes for guided bone regeneration (GBR) techniques is that the maximum size of the membrane is determined by the size of the cocoon. Flatwise-spun silk does not have any limitation to its production size. The objective of this study was to compare flatwise-spun silk mats with cocoon-derived silk mats for a GBR technique. Tensile strength, scanning electron microscopy, Fourier transform infrared (FT-IR) spectroscopy and sericin contents analysis were done for in vitro test. Bone regeneration ability was tested in the critical sized defect of the animal model. In this study, flatwise-spun silk mats showed lower tensile strength and similar tensile strain to a cocoon-derived commercialized silk mat (TDI). Compared to TDIs, the flatwise-spun silk mats showed a similar second derivative spectrum, but they showed an increased abundance of the random coil and helix structures in the FTIR spectra because of a higher content of sericin. In animal model experiments, the bone volume (BV) after the application of a flatwisespun silk mat was similar to the volume observed after the application of a TDI. Both groups showed a significantly higher BV compared to an unfilled control group (P<0.05). Considering that there was no size limitation in producing flatwise-spun silk mats, their clinical indications could be much wider than cocoon-derived silk mats.

      • Cisplatin and 4-hexylresorcinol synergise to decrease metastasis and increase survival rate in an oral mucosal melanoma xenograft model: a preliminary study.

        <P>The present study was undertaken to examine the effects of cisplatin plus 4-hexylresorcinol (4-HR) combination therapy on oral mucosal melanoma (OMM) using cultured primary OMM cells in a tumour xenograft model. Cultured primary OMM cells were used for the MTT assay and DNA microarray. OMM cells were implanted into the submandibular glands of nude mice. The mice were then treated with cisplatin only or cisplatin plus 4-HR. Tumour size changes, survival rate and tumour metastasis were compared between the two groups by observation, micro-positron emission tomography (PET) and histological examination. In the MTT assay, the cisplatin plus 4-HR group showed significantly higher inhibition of OMM cell growth compared to the other groups (p<0.05). DNA microarray results showed significant inhibition of matrix metalloproteinase (MMP)-2 gene expression upon 4-HR application. The necropsy and micro-PET results showed that the mice from the cisplatin-only group had more distant metastases than the mice from the cisplatin plus 4-HR combination group (p=0.002). MMP-2 expression was lower in the primary tumours in the cisplatin plus 4-HR combination group than in the cisplatin-only group (p<0.001). Overall survival was longer in mice from the cisplatin plus 4-HR combination group than in the cisplatin-only group (p=0.049). In conclusion, the combined effect of cisplatin and 4-HR resulted in fewer metastases and longer survival than cisplatin-only treatment in the OMM xenograft model.</P>

      • Custom Implant for Reconstruction of Mandibular Continuity Defect

        <P><B>Purpose</B></P> <P>The purpose of this study was to compare the daily food intake rate and the rate of screw loosening between 2 groups of rabbits with mandibular continuity defects: custom implant (CI) group and 5-hole mini-plate group.</P> <P><B>Materials and Methods</B></P> <P>Two types of cylindrical implants were printed, and their physical strength was compared. In this study using rabbits, 1 group (n = 5) received a CI for the reconstruction of a mandibular continuity defect (CI group) and the other group (n = 5) received a 5-hole mini-plate without a bone graft (reconstruction plate [RP] group). After reconstruction, the daily food intake rate and the rate of screw loosening were examined postoperatively. Histologic examination in the CI group was performed 3 months after the operation.</P> <P><B>Results</B></P> <P>The design that mimicked the mandible showed greater physical strength. The amount of time required to achieve 50% recovery was shorter in the CI group than in the RP group (<I>P</I> = .011). The total number of loosened screws in the CI group was lower than that in the RP group at 3 months postoperatively (<I>P</I> = .008). New bone formation in the porous CI was evident in the CI group.</P> <P><B>Conclusions</B></P> <P>Rabbits with mandibular continuity defects treated with CIs for reconstruction showed faster recovery of the daily food intake rate and fewer loosened screws than those treated with a 5-hole mini-plate without bone graft.</P>
