        Surgical correction of cleft lip lower-lip deformity: a report of three cases

        Cleft lip lower-lip deformity is a secondary deformity in patients who underwent primary cheiloplasty of the upper lip, characterized by an enlarged and anteriorly rotated lower lip. In these cases, soft-tissue imbalances remain even after skeletal correction with orthognathic surgery, and additional soft tissue treatment is required for lip harmony and esthetic facial balance in CLP (cleft lip palate) patients. This study describes three cases of trans-verse myomucosal excision of the lower lip for correction of cleft lip lower-lip deformity to restore facial esthetic balance. Each patient underwent orthognathic surgery, rhinoplasty, or upper lip revision cheiloplasty according to condition. Postoperatively, volume of the lower lip decreased and lip harmony was improved in all three patients. The surgeon should fully understand the anatomical structure around the lips and be able to evaluate over-all harmony of the soft tissue. When a lower lip deformity is present, careful surgical planning and execution are important for each patient.

        A comparative study between data obtained from conventional lateral cephalometry and reconstructed three-dimensional computed tomography images

        Objectives: The aim of this study was to verify the concordance of the measurement values when the same cephalometric analysis method was used for two-dimensional (2D) cephalometric radiography and three-dimensional computed tomography (3D CT), and to identify which 3D Frankfort horizontal (FH) plane was the most concordant with FH plane used for cephalometric radiography. Materials and Methods: Reference horizontal plane was FH plane. Palatal angle and occlusal plane angle was evaluated with FH plane. Gonial angle (GA), palatal angle, upper occlusal plane angle (UOPA), mandibular plane angle (MPA), U1 to occlusal plane angle, U1 to FH plane angle, SNA and SNB were obtained on 2D cephalmetries and reconstructed 3D CT. The values measured eight angles in 2D lateral cephalometry and reconstructed 3D CT were evaluated by intraclass correlation coefficiency (ICC). It also was evaluated to identify 3D FH plane with high degree of concordance to 2D one by studying which one in four FH planes shows the highest degree of concordance with 2D FH plane. Results: ICCs of MPA (0.752), UOPA (0.745), SNA (0.798) and SNB (0.869) were high. On the other hand, ICCs of gonial angle (0.583), palatal angle (0.287), U1 to occlusal plane (0.404), U1 to FH plane (0.617) were low respectively. Additionally GA and MPA acquired from 2D were bigger than those on 3D in all 20 patients included in this study. Concordance between one UOPA from 2D and four UOPAs from 3D CT were evaluated by ICC values. Results showed no significant difference among four FH planes defined on 3D CT. Conclusion: FH plane that can be set on 3D CT does not have difference in concordance from FH plane on lateral cephalometry. However, it is desirable to define FH plane on 3D CT with two orbitales and one porion considering the reproduction of orbitale itself.

      • Experimental investigation of mechanical properties of UV-Curable 3D printing materials

        <P><B>Abstract</B></P> <P>More recently, three dimensional printing (3D Printing), also known as an additive manufacturing (AM), has been highlighted since it shows a great promise to realize almost any three dimensional parts or structures with computer aided design (CAD). Several different processes are available for 3D printing, which includes fused deposition modeling, selective laser sintering, stereolithography, photopolymerization, and etc. In particular, considerable attention is paid to the 3D printing technique with photopolymerization due to their high resolutions. Unfortunately, the 3D printed products with photopolymerization however possess poor mechanical properties. Understanding of this should be necessary for the advantages of the 3D printing to be fully realized. Here, this study experimentally investigates the mechanical properties of the 3D printed photopolymer through thermomechanical analysis and tensile testing. In this study, it is found that the printed specimens are not fully cured after the 3D printing with photopolymerization. DiBenedetto equation is employed to better understand the relationship between the curing status and tensile properties. In addition to the poor mechanical properties, anisotropic and size dependent tensile properties of the 3D printed photopolymers are also observed.         톨게이트 광고의 사업성 판단을 위한 광고효과 예측 방법

        본 연구는 ‘고속도로 톨게이트 상단 광고(이하 T/G광고)’의 효과를 예측할 수 있는 방법을 제시할 목적으로 진행되었다. 이를 위해 우선 (1) 교통량, (2) 유효가시거리, (3) viewpoint 도달점, (4) 유효가시도, (5) 상대적 유효가시도, (6) 광고 노출시간 등 T/G광고의 효과에 영향을 미칠 것으로 예상되는 변인들을 제시하였다. 그리고 이 변인들을 바탕으로 T/G광고의 정량적 효과를 나타내는 ‘impact 지수’를 계산하는 방법을 제시하였다. 마지막으로 impact 지수에 T/G광고의 정성적 가치를 결합하여 T/G광고 효과의 index값을 산출하는 방법을 제시한 뒤, 이를 전국 348개 톨게이트 중 교통량 상위 30위에 해당하는 톨게이트 및 그 외 기타 요소들을 고려하여 추가한 3개의 톨게이트 등 총 33개의 톨게이트에 적용하여 각 톨게이트의 효과등급을 제시하였다. This study aimed to develop a method that can predict the impact of tollgate advertising. To this end, this study first identified the several key factors that are expected to affect the impact of tollgate advertising, such as (1) traffic, (2) valid distance of visibility, (3) viewpoint reach point, (4) valid degree of visibility, (5) relative degree of visibility, and (6) time of advertising exposure. Based on the factors, this study developed a method that can estimate the degree of impact indicating the quantitative impact of tollgate advertising. Further, the degree of impact was combined with the qualitative value of tollgate advertising to index the impact of tollgate advertising. Finally, the index was applied to the top 30 tollgates out of 348 tollgates across the country in terms of traffic and the other 3 tollgates added to grade the impact of the 33 tollgates.

        편측성 구순구개열환자의 일관 치료 증례

        This case report described sequential processes of normalizing cleft deformities through three surgical procedures with time intervals. This report discussed the application of surgical methods applied at each stage. At the age of 7 months, surgery for a unilateral cleft lip was performed using the Fisher method, along with the closure of the hard palatal cleft using the Vomer flap. One year later, at one year and seven months of age, the remaining soft palatal closure was performed using the von Langenbeck method without lateral releasing incision. Simultaneously, corrective muscle realigning surgery was conducted to restore their direction and functionality. At age 8, corrective surgery addressed nasal deformity and asymmetric lip length, resulting in esthetic improvement.
