        2급 응급구조사의 성인외상환자에 대한 처치의 적절성 평가

        In Korea, EMT have two classified subgroup of first and second degree : EMT-P, and EMT. For the beginning, authorization of the first EMT was made in this year through the EMT educational program. There are 8 items which EMT can perform the treatment without the supervision by a physician. But there was no previous report of pertinent assesment of treatment performed by the EMT. There upon authors have conducted review of assessment done by the EMT in EC setting, and proposed the following treatment application can be used by EMT at the field of accident. The study materials include adult trauma patient older than 15 years, and sustained trauma within 48 hours before visiting Ewha University DongDaeMoon Hospital Emergency Deparment. The duration of study was from Apr. 1 1995 to Aug. 31 1995. the study was conducted by 5 certified EMT. The EMT's performance was evaluated by the attending Emergency Medicine physician. The result were as followings ; 1. Among the total 5532 ED visiting patients during the study period, 1267 were adult trauma patients who recieved treatment within 48 hours of injury. 2. Among 1222 out of 1267 patients have completed performed survey. 3. Male to female ratio was 2.6 : 1, age distribution was from 15 to 87 years old and mean age was 34.6 years old. 4. The MOI showed : MVA (33.1%), blunt trauma (29.9%), slip down (16.8%), penetrating injury(15.4%), and miscellaneous (5.7 %), burn (3.1%), falling down injury(2.9 %), bite injury(1.1%) in order. 5. There were 437 cases of survey completed by the EMT. For assessment of mental status of 3 different method : AVPU, 5 scale mental status and GCS, were correlated with EM physician with 99.1%, 98.6%, 97.7 %, respectively. 6. For 320 out of 437 cases of survey completed, 319 patients recieved treatment by the EMT within 30 minute and 1 patient between 30 to 60 minute of an ED arrival. 7. For those 437 cases of survey completed, 266 patient (60.9%) had adequate treatment and 54 patients (12.4%) had inadequate treatment. 67 patient (15.3%) did not received treatment but it was adequate inway, and 50 patients (11.9%) did not received treatment even though patient required treatment. In cases of adult trauma patients, EMT's mental status assessment was correlated with EM physician's assessment. But treatment done by EMT without the supervision of EM physician was adequate only in 76.2 %. Therefore, continuous education and assessment of treatment performed by EMT in needed to improve field treatment by EMT future.

        The Analysis on the Activation Procedure of Polymer Electrolyte Fuel Cells

        It is, in general, believed that during the activation process, the proton conductivity increases due to wetting effect and the electrochemical resistance reduction, resulting in an increase in the fuel cell performance with time. However, until now, very scant information is available on the understanding of activation processes. In this study, dominant variables that effect on the performance increase of membrane electrode assemblies (MEAs) during the activation process were investigated. Wetting, pore restructuring and active metal utilization were analyzed systematically. Unexpectedly, the changes for both ohmic and reaction resistance characterized by the electrochemical impedance spectroscopy (EIS) after initial wetting process were much smaller when considering the degree of cell performance increases. However, the EIS spectra represents that the pore opening of electrode turns into gas transportable structure more easily. The increase in the performance with activation cycles was also investigated in a view of active metals. Though the particle size was grown, the number of effective active sites might be exposed more. The impurity removal and catalytic activity enhancement measured by cyclic voltammetry (CV) could be a strong evident. The results and analysis revealed that, not merely wetting of membrane but also restructuring of electrodeand catalytic activity increase are important factors for the fast and efficient activation of the polymer electrolyte fuel cells.

        최근 3년간 서울 지역 단일기관에서 급성 바이러스 간염의 원인

        목적: 최근 급성 바이러스 간염 중 급성 A형 간염이 급격히 증가하고 있으며, 급성 E형 간염바이러스 간염도 드물지 않게 보고되고 있다. 이에 최근 3년간 단일기관에서의 급성 바이러스성 간염의 원인과 이 중 급성 A형과 E형 간염의 중복감염의 임상적 특징을 알아보고자 하였다. 방법: 2005년 1월부터 2007년 7월까지 중앙대학교병원에서 IgM anti-HAV, IgM anti-HBc, HCV RNA, IgM anti-HEV의 검사들 중 한 가지 이상에서 양성인 급성 바이러스성 간염으로 입원한 111명 환자들을 후향적으로 조사하였다. 결과: 전체 111예 중 A형 간염은 105예(94.6%), B형 간염 5예(4.5%), C형 간염 1예(0.9%)였다. 급성 A형 간염 105예 중 27예(24.3%)에서 IgM anti-HEV가 양성으로 급성 A형 및 E형 간염의 중복감염으로 확인되었고, 이중 9예(8.6%)에서 추적검사상 IgM anti-HEV가 음전되었다. 급성 A형 단독 간염(78예)과 급성 A형과 E형 간염의 중복감염(27예)의 임상양상을 비교 분석한 결과 급성 A형 단독 간염과 급성 A형 및 E형 간염의 중복감염의 평균연령(29세 vs 28세), 재원기간(12일 vs 10일), 남녀의 비(51:27 vs 14:13)는 두군 간에 차이가 없었다. 입원 당시 임상증상과 생화학적 간기능 검사 및 일반혈액소견은 두 군 간에 차이가 없었다. 두 군 모두 전격성 간부전, 사망 등의 중대한 합병증은 없었으며 8주내에 생화학적 간기능 검사가 정상화 되었다. 결론: 국내에서 최근 발생하는 급성바이러스 간염의 대부분의 원인은 A형 간염 바이러스이며, 흥미롭게도 급성 A형 간염과 E형 간염의 중복 감염 예가 관찰되었으며, 급성 A형 단독간염과 급성 A형과 E형 간염의 중복 감염에서 두 군 간의 임상 증상 및 경과에 큰 차이가 없었다. Background/Aims: Hepatitis A is increasing during the recent years in Korea, and sporadic cases of hepatitis E are not rare. We investigated the etiology of acute viral hepatitis, including 27 cases of coinfection with hepatitis A and E, during the last 3 years. Methods: Retrospective analysis of one hundred eleven patients with acute viral hepatitis at Chung-Ang University Hospital was done from January 2005 to July 2007. The patients were positive for any one of the following tests: IgM anti-HAV, IgM anti-HBc, HCV RNA and IgM anti-HEV. Results: The proportions of acute viral hepatitis A, B and C were 94.6% (105/111), 4.5% (5/111) and 0.9% (1/111), respectively. Among the patients with hepatitis A, 27 patients (24.3%) were positive for IgM anti-HEV. We analyzed 105 patients with either hepatitis A only or they had coinfection of hepatitis A and E. The mean age, the duration of the hospital stay, the mean levels of serum transaminase, bilirubin and albumin and the protrombin time were similar between the patients with hepatitis A only and those with coinfection of hepatitis A and E. All the patients were discharged without serious complication such as fulminant hepatic failure. The biochemical liver function tests were normalized in all patients within 8 weeks. Conclusions: The most common etiology of acute viral hepatitis in Koreans was the hepatitis A virus. Patients coinfected with hepatitis A and E were observed. The patients with coinfection of hepatitis A and E showed no significant difference in their clinical features and laboratory parameters, as compared to the patients with hepatitis A only.(Korean J Med 74:624-631, 2008)

        N/A Background : Many factors have been implicated in the etiology of varicose veins. Among them, the most widely advanced are weakness of the vascular wall due to defective smooth muscle and connective tissue metabolism, dysfunction of the endothelium, valvular incompetence, and hemodynamic stress. However, the etiology remains unclear. Objective : This study was performed to analyze the wall structure and composition changes of primary varicose veins of the legs. Methods : Varicose vein samples were collected from 13 patients having primary varicose veins who underwent ambulatory phlebectomy. Control veins(below the age of 50 years) were collected from 2 patients with no history or clinical evidence of varicose vein. Samples were stained with hematoxylin and eosin, Masson`s Trichrome, Van Gieson stains and immunohistochemical stains for collagen type Ⅳ, α-smooth muscle actin antigen were performed. Tissue samples were examined by light microscopy for comparative histological changes and different components of the varicose vein wall as compared to control veins. Results : In the varicose vein, there was intimal hypertrophy in almost all the sections examined. The thickness of intima varied at different places. There were hyperplasia of smooth muscle cell and collagen in the varicose vein. Collagen type Ⅳ exhibited an accumulation especially in the subendothelial region of varicose vein. Conclusion : Dilatation and varicosities are not due to deficiency in the vein wall components, but rather to hemodynamic response, Modulation in the extracellular proteins may be the cause for the development of varices.         <P><B>Abstract</B></P> <P>We analyzed the consumer goods containing NORM by ICP-MS and evaluated the external dose. To evaluate the external dose, we assumed the small room model as irradiation scenario and calculated the specific effective dose rate using MCNPX code. The external doses for twenty goods are less than 1 mSv considering the specific effective dose rates and usage quantities. However, some of them have relatively high dose and the activity concentration limits are necessary as a screening tool.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The consumer goods containing NORM are analyzed by ICP-MS. </LI> <LI> The external doses by consumer goods are evaluated assuming the small room model. </LI> <LI> The activity concentration limits are derived as a screening tool. </LI> </UL> </P>

      • Introduction of Prediction Method of Welding Deformation by Using Laminated Beam Modeling Theory and Its Application to Railway Rolling Stock

        The welding deformation and its prediction method at the HAZ (Heat-Affected Zone) are presented in this paper. The inherent strain method is well known as analytical method to predict welding deformation of large scale welded structure. Depend on the size of welding deformation in welding joints, the fatigue life, the stress concentration factor and the manufacturing quality of welded structure are decided. Many welded joints and its manufacturing control techniques are also required to railway rolling stock and its structural parts such as railway carbody and bogie frame. Proposed methods in this paper focus on the two different the inherent strain area at HAZ. This is main idea of proposed method and it makes more reliable result of welding deformation analysis at the HAZ.
