        Efficacy of Cranial Electrotherapy Stimulation Combined with Biofeedback Therapy in Patients with Functional Constipation

        Background/Aims A large number of studies have shown that function constipation (FC) has an extremely high incidence of mental and psychological disorders. Cranial electrotherapy stimulation (CES) was applied to the treatment of psychological disorders such as anxiety and depression. We explored the effects of CES combined with biofeedback therapy (BFT) on the psychological state, clinical symptoms, and anorectal function in patients with FC. Methods A total of 74 patients with FC were randomly divided into 2 groups. The control group received BFT. CES combined with BFT was carried out in the experiment group. All patients were assessed using the self-rating anxiety scale (SAS), self-rating depression scale (SDS), and Wexner constipation score at baseline and the end of each course. Anorectal manometry and balloon expulsion tests were performed before and after treatment. Results After treatment, the participants in the experiment group had significantly lower score SAS, SDS, and Wexner constipation scores than the control group (all P < 0.05). The number of successful expulsion in the experiment group was larger than the control group (P = 0.016). Conclusions CES combined with BFT was effective in improving the psychological status of anxiety, depression, and bowel symptoms in patients with FC. (J Neurogastroenterol Motil 2016;22:497-508)

      • The Blood Neutrophil-to-lymphocyte Ratio Predicts Survival in Patients with Advanced Hepatocellular Carcinoma Receiving Sorafenib

        Background and Aim: Increasing evidence correlates the presence of systemic inflammation with poor survival in patients with hepatocellular carcinoma (HCC). The aim of this study was to investigate the prognostic significance of the blood neutrophil-to-lymphocyte ratio (NLR) in patients with advanced HCC who received sorafenib monotherapy. Methods: A total of sixty-five patients with advanced HCC, not eligible for locoregional therapy, treated with sorafenib were enrolled. Potential prognostic factors such as age, gender, tumoral characteristics, performance status and NLR were analyzed. Results: Median OS and TTP for the entire cohort were 10.0 months (95%CI, 7.6-12.3 months) and 4.5 months (95% CI, 4.0-4.9 months). The mean NLR at baseline was 2.89. The median OS of patients with a high NLR (>4) was 6.5 months (95%CI, 5.2-7.7 months) compared with 12.5 months (95%CI, 9.9-15.0) for patients with a normal NLR (${\leq}4$) (P=0.01). Age ${\leq}65$, NLR>4, extrahepatic metastases and vascular invasion were all predictors of poorer overall survival. Multivariate analysis showed that NLR > 4, vascular invasion and extrahepatic metastases were independent predictors of poorer overall survival. The median TTP of patients with a high NLR was 2.5 months (95%CI, 1.4-3.6 months) compared with 4.5 months (95%CI, 3.9-5.1 months) for patients with a normal NLR (P=0.012). Conclusions: High baseline NLR was associated with worse OS and TTP for patients with advanced HCC treated with sorafenib.

        Identification and Heterologous Expression of a Δ4-Fatty Acid Desaturase Gene from Isochrysis sphaerica

        The marine microalga Isochrysis sphaerica is rich in the very-long-chain polyunsaturated fatty acids, including eicosapentaenoic acid (EPA, C20:5ω-3) and docosahexaenoic acid (DHA, C22:6ω-3) that are important to human health. Here, we report a functional characterization of a Δ4-fatty acid desaturase gene (FAD4) from I. sphaerica. IsFAD4 contains a 1,284 bp open reading frame encoding a 427 amino acid polypeptide. The deduced amino sequence comprises three conserved histidine motifs and a cytochrome b5 domain at its N-terminus. Phylogenetic analysis indicated that IsFad4 formed a unique Isochrysis clade distinct from the counterparts of other eukaryotes. Heterologous expression of IsFAD4 in Pichia pastoris showed that IsFad4 was able to desaturate docosapentaenoic acid (DPA) to form DHA, and the rate of converting DPA to DHA was 79.8%. These results throw light on the potential industrial production of specific polyunsaturated fatty acids through IsFAD4 transgenic yeast or oil crops.

      • Molecular cloning, characterization, and heterologous expression analysis of heat shock protein genes (hsp70 and hsp90) of the invasive alien weed, Ageratina adenophora (Asteraceae)

        An invasive alien weed, Ageratina adenophora, has become widespread in the southwest of China due to its high environmental adaptation. In the present study, in order to understand the molecular aspects of resistance to heat and cold stress, we investigated the characterization and expression of heat shock protein genes (designated as Aahsp70 and Aahsp90) in A. adenophora. The cDNA of A. adenophora Aahsp70 and Aahsp90 was cloned by the combination of homology cloning and rapid amplification of cDNA ends (RACE). The Basic Local Alignment Search Tool analyses revealed that Aahsp70 and Aahsp90 shared high levels of identity with corresponding proteins from other species. Hybridization was employed to examine the expression patterns of Aahsp70 and Aahsp90. The relative expression levels of the Aahsp70 and Aahsp90 transcripts were both up-regulated and reached maximal levels at 2-4 h after heat shock. Then, Aahsp70 dropped progressively to the original level at ~4 h after heat shock. The transcriptional changes of Aahsp70 and Aahsp90 both were more obvious under cold stress. Recombinant Aahsp70 and Aahsp90 improved the viability, in comparison with the control cultures, of Escherichia coli under stress conditions. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis analysis of the cell lysates suggested that the protective effect in vivo was related to increased thermo-stability of the soluble cytosolic proteins. This study emphasizes that the relationship between, and regulation of, heat shock proteins might incorporate variable strengths to increase the adaptation of A. adenophora.

      • Adenovirus-mediated Expression of Both Antisense Ornithine Decarboxylase and S-adenosylmethionine Decarboxylase Induces G<sub>1</sub> Arrest in HT-29 Cells

        To evaluated the effect of recombinant adenovirus Ad-ODC-AdoMetDCas which can simultaneously express both antisense ornithine decarboxylase (ODC) and S-adenosylmethionine decarboxylase (AdoMetDC) on cell cycle distribution in colorectal cancer cell and investigated underlying regulatory responses, human colorectal cancer cells HT-29 were cultured in RPMI 1640 medium and infected with Ad-ODC-AdoMetDCas. Cell cycle progression was detected by flow cytometry analysis. The expression levels of cell cycle regulated proteins were measured by Western blot analysis. The mRNA level of cyclin D1 was measured by RT-PCR. And a luciferase reporter plasmid of cyclin D1 promoter was constructed to observe the effect of Ad-ODC-AdoMetDCas on cyclin D1 promoter activity. The results showed that recombinant adenovirus Ad-ODC-AdoMetDCas significantly induced $G_1$ arrest, decreased levels of cyclin D1 protein and mRNA and suppressed the promoter activity. Ad-ODC-AdoMetDCas also inhibited nuclear translocation of $\beta$-catenin. In conclusion, downregulation of ODC and AdoMetDC mediated by Ad-ODC-AdoMetDCas transfection induces $G_1$ arrest in HT-29 cells and the arrest was associated with suppression of cyclin D1 expression and inhibition of $\beta$-catenin nuclear translocation. As a new anticancer reagent, the recombinant adenovirus Ad-ODC-AdoMetDCas holds promising hope for the therapy of colorectal cancers.

      • Mll3 Genetic Variants Affect Risk of Gastric Cancer in the Chinese Han Population

        It is reported that the expression level of MLL3 in gastric cancer tissue highly correlates with tumor progression. However, whether MLL3 genetic variants are associated with the risk of gastric cancer remains unclear. In this study, we conducted a genotyping analysis for MLL3 in 314 cases of gastric cancer and 322 controls from the Chinese Han population. 4 SNPs (rs6943984, rs4725443, rs3800836, rs6464211) were selected for the present analysis. We found 2 SNPs (rs6943984, rs4725443) of MLL3 gene were significantly associated with the risk of gastric cancer : the rs6943984 with the minor allele A and rs4725443 with the minor allele C revealed strong associations with increased gastric cancer risk [P < 0.001, OR=1.97, 95% CI=1.48~2.64 and P <0.001, OR=2.23, 95% CI=1.54~3.24]. Haplotype analysis of the four SNPs showed that haplotype A-T-A-C, G-T-G-C, and G-C-A-C increased the risk of gastric cancer (P <0.001, P=0.18, and P<0.001, respectively), while haplotype G-T-A-C significantly reduced the risk of gastric cancer (P <0.001). We concluded that MLL3 variants are significantly associated with gastric cancer risk. Our results for the first time provided new insight into susceptibility factors of MLL3 gene variants in carcinogenesis of gastric cancer of the Chinese Han population.

        Experimental study on vibration characteristics of double-helical gearbox with isolators

        A power closed double-helical gear test rig with isolators was created to investigate the dynamic response characteristics of the double-helical gearbox. Experimentally, the vibration accelerations of the bearing seat, machine foot, and base were tested under various working conditions. Experiments validated the validity of the generalized finite element theorybased dynamic model of the double-helical gear system. The study found that the unloaded vibration of the gearbox is greatly caused by the shaft frequency excitation due to assembly/manufacturing errors. As the load increases, the assembly/manufacturing errors are compensated by the system deformation. In the acceleration spectrum of the bearing seat, it is found that the tooth surface modification can reduce the amplitude of the mesh frequency, but the bearing outer ring frequency components are increased. That is, the axial movement of double-helical gears is intensified. Furthermore, the closer the excitation frequency is to the natural frequency of the base, the worse the vibration isolation effect is.

        Choledochoduodenal fistula in Mainland China

        Purpose: Choledochoduodenal fistula (CDF) is an extremely rare condition even in the most populous nations. However, diagnostic tools are inadequate for the young surgeon to be made aware of such a rare condition before surgery. Hence, basic understanding of the epidemiology, etiology, and management for this unusual but discoverable condition are necessary and essential. Methods: The exclusive case reports of CDF, which were published from 1983 to 2014 concerning mainland Chinese people, were performed to review the epidemiology, etiology, and management. Results: A total of 728 cases were incorporated into this review among 48 papers. More than half of the CDF cases were female (416) with an average age of 57.3 years. CDF was usually caused by cholelithiasis (573 of 728). Epigastric pain (589 of 728) and cholangitis (395 of 728) were the most common symptoms of CDF. CDF was usually detected and confirmed by endoscopic retrograde cholangiopancreatography (ERCP) (475 of 728) in Mainland China. The fistulas larger than 1 cm (82 of 654) were recommended for surgical biliary reconstruction. Fistulas between 0.5 cm and 1.0 cm (467 of 654) which were followed frequently by cholangitis attacks also required surgery; the rest were recommended to have stone removal and/or the application of an effective biliary drainage. Fistulas less than 0.5 cm (105 of 654) were usually received conservative therapy. Conclusion: CDF should be considered in differential diagnosis of recurrent epigastric pain and cholangitis. A possible ERCP should be arranged to investigate carefully. Depending on the size of fistula and clinical presentation, different programs for CDF are indicated, ranging from drug therapy to choledochojejunostomy.

        Effect of Global Warming on Chloride Ion Erosion Risks for Offshore RC Bridges in China

        Chloride ion penetration, which leads to corrosion of steel reinforcement, is the leading cause of offshore reinforced concrete bridge deterioration. Global warming would accelerate the process of chloride ion penetration and then structural degradation. In China, durability design code for concrete structures has been issued in 2008, and some thresholds of design parameters have been specified to mitigate corrosion in concrete bridge. However, influence of global warming on structure durability was not considered in the issued code. This would overestimate the durability of concrete structures. In this paper, a reliability based method was adopted to evaluate the influence of climate change on the durability of offshore RC bridges considering the acceleration of chloride ion penetration caused by temperature rise. Two climate change scenarios according to IPCC report were considered to define the global warming. A baseline scenario, in which the mean average temperature equals to that of the year 2000, were defined. It is found that the amount of chloride ion on the surface of reinforcing steel bar in the global warming scenarios are 6%-15% higher than that of structure in the baseline scenario. Meanwhile, the probability of corrosion initiation and corrosion damage increased also. In 2100, structures in environmental categories III-C and III-F, designed according to Chinese codes, can no longer adapt the impact of climate change on structure. Comparison of reliability of improved and pre-improved structure was conducted to verify the effectiveness of the proposed improvement measures.

      • UAV-Based Remote Sensing and Risk Assessment of Rockfalls in Guangzhou, China

        Unmanned Aerial Vehicle (UAV) technique has been extensively applied in geohazards assessment. In this study, we illustrate the advantages of the UAV technique in the remote sensing and risk assessment of rockfalls, through a case study of the rockfalls located in Guangzhou, China; in which, the UAV images collected are adopted for identifying both potential rockfalls and geohazards bearing bodies. On the basis of a visual inspection of the high-resolution aerial photographs, the potential rockfalls are readily identified. Then, the post-failure runout behavior of the identified rockfalls is analyzed with the numerical software of particle flow code (PFC). Due to the inherent uncertainty in the determination of the micro-parameters in the PFC models, the micro-parameters required in the PFC modelling are calibrated as random variables. With the aid of the Monte Carlo simulation (MCS), the distributions of the runout distance and the velocity of the rockfalls could be obtained. From there, the zones that might be impacted by the rockfalls and the destructive forces on the potential geohazards bearing bodies (i.e., buildings) in these zones can be evaluated. Further, the geohazards bearing bodies in the zones that are impacted by the rockfalls are recognized with an automated image recognition model, which is based upon a deep learning-based image recognition technique of RetinaNet. On the basis of the post-failure runout behavior analysis of the rockfalls and the geohazards bearing bodies recognized, a rapid risk assessment of the rockfalls in the studied region can be conducted.
