        Spine Fractures of Patients with Ankylosing Spondylitis and Diffuse Idiopathic Skeletal Hyperostosis: Fracture Severity and Injury-Related Mortality at a Level I Trauma Center

        Study Design: Retrospective review of prospectively collected cohort.Purpose: To identify differences in treatment and mortality of spine fractures in patients with ankylosing conditions of the spine. Overview of Literature: Ankylosing spondylitis (AS) and diffuse idiopathic skeletal hyperostosis (DISH) are the two most common etiologies of ankylosing spinal disorder (ASD). However, studies on the treatment and outcomes of spine fractures in AS and DISH patients remain few.Methods: Patients presenting with a spine fracture were diagnosed with AS or DISH at a single tertiary care center between 2010 and 2019. We excluded those who lacked cross-sectional imaging or fractures occurring at spinal segments affected by ankylosis, as well as polytraumatized patients. Patient demographics, injury mechanism, fracture level, neurologic status, treatment, and 1-year mortality were recorded. Computed tomography imaging was reviewed by two independent readers and graded according to the indicated AO Spine Injury Classification System. Differences in fracture severity, treatment method, and mortality were examined using Student t -tests, chi-square tests, and two-proportion Z-tests with significance set to p <0.05.Results: We identified 167 patients with spine fracture diagnosed with AS or DISH. Patients with AS had more severe fractures and more commonly had surgery than patients with DISH (p <0.001). Despite these differences, 1-year mortality did not significantly differ between AS and DISH patients (p =0.14).Conclusions: Although patients with AS suffered more severe fractures compared to DISH and more frequently underwent surgery for these injuries, outcomes and 1-year mortality did not differ significantly between the two groups. For patients with ASDs and fractures, outcomes appear similar regardless of treatment modality. Consequently, there may be an opportunity for critical reappraisal of operative indications in ASD and a larger role for nonoperative management in these challenging patients.

      • Automated electrospray ionization FT-ICR mass spectrometry for petroleum analysis

        <P>Analysis of petroleum samples at the molecular level by Fourier transform ion cyclotron resonance mass spectrometry (FT-ICR MS) typically requires a prolonged accumulation of ions and/or summing up a large number of scans. Here, a chip-based micro-ESI system (Advion NanoMate, Ithaca, NY) has been successfully automated in combination with FT-ICR MS analysis of petroleum samples. A foil-sealed 96-well glass plate prevents solvent evaporation, with no visible loss of sample after 20 h of continuous operation. Mass spectra obtained from the same sample but taken from different wells after various time delays were very similar. Data from replicate samples in different wells could be combined to enhance mass spectral signal-to-noise ratio and dynamic range. Furthermore, the automated data acquisition eliminates sample carryover, and produces heteroatom class distribution, double-bond equivalents (DBE), and carbon number very similar to those from the conventional (manual) micro-ESI experiments.</P>

        Highly selective trace ammonium removal from dairy wastewater streams by aluminosilicate materials

        Water is a key solvent, fundamental to supporting life on earth. It is equally important in many industrialprocesses, particularly within agricultural and pharmaceutical industries, which are major drivers of theglobal economy. The results of water contamination by common activity in these industries is well knownand EU Water Quality Directives and Associated Regulations mandate that NH4+ concentrations ineffluent streams should not exceed 0.3 mg L 1, this has put immense pressure on organisations andindividuals operating in these industries. As the environmental andfinancial costs associated with waterpurification begin to mount, there is a great need for novel processes and materials (particularlyrenewable) to transform the industry. Current solutions have evolved from combating toxic sludge to theuse of membrane technology, but it is well known that the production of these membrane technologiescreates a large environmental footprint. Zeolites could provide an answer; their pore size and chemistryenable efficient removal of aqueous based cations via simple ion exchange processes. Herein, wedemonstrate efficient removal of NH4+ via both static and dynamic methodology for industrialapplication. Molecular modelling was used to determine the cation–framework interactions which willenable customisation and design of superior sorbents for NH4+ capture in wastewater.

        Three Rare Concurrent Complications of Tertiary Hyperparathyroidism: Maxillary Brown Tumor, Uremic Leontiasis Ossea, and Hungry Bone Syndrome

        A 48-year-old woman in her 40’s with end-stage renal disease and tertiary hyperparathyroidism (HPT) presented for a rapidly progressive maxillary tumor. Initial workup was notable for elevated intact parathyroid hormone (PTH) and diffuse thickening of skull and facial bones on computed tomography, and maxillary tumor biopsy with multinucleated giant cells. She underwent subtotal parathyroidectomy (with removal of a parathyroid adenoma and 2 hyperplastic glands) and partial resection of maxillary brown tumor. The patient’s post-operative course was complicated by hungry bone syndrome, with hypocalcemia refractory to aggressive calcium repletion. Teriparatide (recombinant PTH) was utilized with rapid resolution of hypocalcemia. To our knowledge, this is the first case of maxillary brown tumor in tertiary HPT to be reported in the USA. This case also supports teriparatide as a novel therapeutic for hungry bone syndrome refractory to aggressive calcium repletion.
