Study Design: Retrospective study. Purpose: The purpose of this study was to investigate the influence of sagittal alignment of the strut graft on graft subsidence and clinical outcomes after anterior cervical corpectomy and fusion (ACCF). Overview of Literature: ACCF is a common technique for the treatment of various cervical pathologies. Although graft subsidence sometimes occurs after ACCF, it is one cause for poor clinical results. Malalignment of the strut graft is probably one of the factors associated with graft subsidence. However, to the best of our knowledge, no prior reports have demonstrated correlations between the alignment of the strut graft and clinical outcomes. Methods: We evaluated 56 patients (33 men and 23 women; mean age, 59 years; range, 33–84 years; 45 with cervical spondylotic myelopathy and 11 with ossification of the posterior longitudinal ligament) who underwent one- or two-level ACCF with an autogenous fibular strut graft and anterior plating. The Japanese Orthopaedic Association (JOA) score recovery ratio for cervical spondylotic myelopathy was used to evaluate clinical outcomes. The JOA score and lateral radiograms were evaluated 1 week and 1 year postoperatively. Patients were divided into two groups (a straight group [group I] and an oblique group [group Z]) based on radiographic assessment of the sagittal alignment of the strut graft. Results: Group I showed a significantly greater JOA score recovery ratio (p <0.05) and a significantly lower graft subsidence than group Z (p <0.01). Conclusions: Our findings suggest that a straight alignment of the strut graft provides better clinical outcomes and lower incidence of graft subsidence after ACCF. In contrast, an oblique strut graft can lead to significantly increased strut graft subsidence and poor clinical results.

바닷물에서 염의 농축과 센물로부터 순수한 물의 제조는 전기 투석법이나 역삼투법을 사용하는 것이 대부분 이지만 이 방법은 전기적 에너지가 많이 필요 하다. 대신에, 효율이 좋은 방법을 기대 하는 연구로써 압력차를 드라이빙-포오스로 이용한 역삼투법은 막을 가운데로 하여 양쪽의 용액속의 대량의 용매를 압력에 의한 추출 방법 이지만 반대로 소량의 염을 회수 하는 방법도 보다 효과적인 것으로 알려저 있다. 이 염의 압력차를 드라이빙-포오스로 하여 능동적으로 염을 회소하는 방법을 압투석법 이라 하지만 현재 실용화 단계 까지는 이러지 못하고 있다. 이 압투석법의 선구자로서 Sollner는 막내의 음, 양이온의 2 종류의 교환기를 갖는 경우에 단일 이온 교환기의 막과 비교 해서 특이한 막 현상을 나타내는 것을 보고 하였고^1,2), Kedom 과 Katchalsky 는 비평형 열역학을 기초로 하여 선형 현상 방정식을 해석 하여 염의 이동과 같이 環전류의 존재를 예상 하였고^3), 이 이론을 배경으로 실험실에서 양성이온 교환막 또는 모자이크 하전막 등의 출현과 개발의 연구가 시작 되었다^4-7). 앞에서 이야기한 것 처럼 압투석법에 의한 염의 수용액의 농축은 자원이 부족하고 바닷물이 풍족한 환경 국가에서 얻을 수 있는 대단히 매력적인 연구이다. 본 실험도 이 壓투석법을 이용한 기초 연구로서 보고 한다. 구체적으로 염의 막 투과 현상을 알기 위해서 우선 각각의 하전 상태(charge state)가 서로 다른 3종류의 하전막 즉, 양성 이온 교환막^7), 모자이크 하전막^8), 그리고 양이온 교환막^9)을 선택 하여 불투과성 물질 및 투과성 물질에 의하여 생성 되는 삼투압 기울기에서 염의 이동을 측정 하였다. 이 결과는 비평형 열역학 및 전기 화학의 분석법으로 고찰 한다^10-12).

Purpose Nursing is traditionally considered a female profession, and male nursing students face gender stereotype barriers. Most studies of male nursing students in Japan focus on their learning experiences in maternal nursing, and little is known about the experiences of male nursing students in their usual learning environment. Therefore, the purpose of this study is to explore the barriers and coping strategies of Japanese male nursing students in nursing education, including classroom and practical training. Methods A qualitative descriptive design was used to study 20 male nursing students from eight Japanese universities. Purposive sampling using the snowball method was used to recruit participants. Semistructured interviews were used to gather data about the participants’ thoughts and experiences in nursing education. All interview data were analyzed using thematic analysis. Results Four themes emerged from the data: (1) nursing choices of own volition; (2) loneliness due to the female-dominated environment and marginalization; (3) dealing with barriers and seeking support; and (4) positive experiences in nursing education. Male nursing students face barriers such as loneliness and feelings of alienation. Family support, mutual support among male nursing students, and the presence of role models were factors that addressed these barriers and positively influenced career choice. Conclusions This research suggests that nursing educators need to understand the barriers faced by male nursing students and provide a gender-neutral learning environment for all students.
Experience with Bilateral Risk-Reducing Mastectomy for an Unaffected BRCA Mutation Carrier

Women with BRCA1/2 mutations have a high risk of breast cancer and may opt for risk-reducing mastectomy (RRM). We report a 38-year-old Japanese woman who was diagnosed as a BRCA2 mutation carrier. She underwent prophylactic bilateral skin-sparing mastectomy (SSM) with excision of the nipple and preservation of the areola skin. It is unclear whether a bilateral RRM leads to better survival compared with intensive surveillance. The oncological risk associated with the presence of remnant breast glandular tissue after SSM or nipple-sparing mastectomy has been obscure. We report the first case of RRM for a Japanese BRCA mutation carrier and provide a literature review on risk management for BRCA mutation carriers with a focus on the concepts and procedures of RRM.
AKARI-FIS POINT SOURCE CATALOGUE: CURRENT STATUS AND FUTURE PLAN

The current status of the AKARI-FIS Point Source Catalogue is reported. The first version of the Bright Source Catalogue has been in public since March 2010 and used extensively in the various fields in astronomy. The second version of the Bright Source Catalogue and the first version of the Faint Source Catalogue are currently under development. The revised Bright Source Catalogue is expected to have improved completeness, reliability, and accuracy compared to the current version. The Faint Source Catalogue will have a scan-density dependent detection limit and will enable much deeper exploration of the sky especially in the high-ecliptic latitude regions. Both catalogues will be available in a year time scale.
