The patella is the largest sesamoid bone in the body and lies within the quadriceps tendon. The patella does very important role as structure in the extensor mechanism of the knee. Fractures of the patella are predominantly intra-articular fracture and the methods of treatment are well known as closed reduction, open reduction and patellectomy. The subjects of our study are twenty eight cases of patellar fracture, these are followed at least more than one year. We analyze the causes of trauma, fracture pattern and the results of each treatment method. The results obtained, were as follows; 1. The majority of these fractures occurred in male patients between 20 and 50 years of age. 2. The majority of patellar fractures result from direct injury; the most common etiologies are slip down or fall down (15 cases) and direct blow in vehicular trauma (13 cases). 3. The most common configuration of fractures was the comminuted (17 cases), but the fracture pattern by indirect force is typical transeverse fracture. 4. Seven cases were treated by conservative meihod and twenty one cases by open reduction. The cases of open reduction are mostly fixed with tension band wiring ar modified tension band wiring. 5. Modified tension band wiring fixation has given acceptable results even in severely comminuted or displaced patellar fractures.
