        전문대학 응급구조과의 국가직무능력표준(NCS) 기반 교육과정 개발 및 운영방안 연구

        Purpose: Although appointed as a national competency standards (NCS) based reserves department, the department of emergency medical technology, an NCS-based emergency department, is mainly focused on subject deduction for a NCS-based curriculum. Methods: Job models were formed and verified by combining the competency unit of NCS and the duty of Developing a curriculum (DACUM) based on the development procedure indicated in the guidelines for a NCS-based curriculum. The mapping method of the subject was performed by deducting necessary competency units (duty) and competency unit elements (task) by connecting with the composition items of NCS and DACUM. Results: Job models combined with job analysis for the NCS and DACUM were reduced to 13 competency units (duty) and 79 competency unit elements (task). A modified method such as the 1:N method was mainly applied as a subject-matching method with consideration of the competency level and size of the competency unit. Conclusion: It would be a desirable direction to develop a NCS-based curriculum in the center of the practice subject in consideration of the size of the competency unit and competency level of the competency unit element. The existing curriculum should be promoted as a field-oriented curriculum at the complementary level.

        119구급대원의 직업기초능력 요구도 및 우선순위 분석 기반 직무기초역량 제안 연구

        Purpose: This study aimed to determine the core competencies for 119 emergency medical technicians (EMTs) and to provide evidence for the development and utilization of 119 EMTs, education and training programs. Methods: Data were collected with a questionnaire that consisted of items on the general characteristics of the subjects (8 items) and importance and satisfaction levels for 10 competencies, including 34 items on subfactors. The Borich Needs Assessment Model was used in designing the questionnaire. A locus for focus model was used to derive the top priority competencies for the improvement of core competency in the profession. Data were analyzed with by SPSS ver.21 (IBM, Armonk, NY, USA). Results: The core competencies for 119 EMTs were derived from technical competency, information competency, communication competency, problem-solving competency, self-development competency and interpersonal competency. In addition, among the sub-factors of these competencies, 16 abilities (including leadership), ability to apply technical knowledge, technical comprehension, conflict management ability, ability to create documents, problem handling ability, ability to think, technical selection ability, and computer literacy were included in the priority category. Conclusion: This study derived and presented the core competencies for 119 EMTs to enable them to meet the modern day requirements of their roles, which may contribute to enhancing their professionalism.

        응급구조(학)과 학생들의 구급현장실습 대체 간접실습 학습 항목 제안

        Purpose: This study aimed to present the learning items of indirect practice as an alternative means of ambulance riding practice. Methods: Twelve on-site professionals participated in testing the effectiveness of learning items of utility value in indirect experience practice among the recommended learning objectives of ambulance ride practice using the modified Delphi technique. To validate the learning objectives, the Delphi technique was used to check content validity, and analytic hierarchy process (AHP) analysis was used to analyze the weighting value. Results: As the learning goal of indirect experience practice replaced the ambulance riding practice, four items were derived in consideration of practicality. These items were consistent with the Delphi panel's consistency index, convergence, and agreement. The first learning item to consider was 'Prevention of infection and risk of infection.' Conclusion: The results of this study are meaningful in that they propose the learning items that can achieve the minimum educational goal through indirect experience practice as an alternative means of ambulance training.

        외근직 소방공무원의 직무유형에 따른 요통발생의 차이

        The purpose of this study was to analyze the effect of fire fighters` duty type on occurrence and severity of low back pain. Data were collected with questionnaire to 450 fire fighters working at 119 safety center and rescue unit of 30 fire stations in Gyeonggi-do, Finally the study was analyzed for 360 persons among them. The risk of experiencing of lower back pain occurred in tre order of Rescue[odds ratio(OR) 3.36, 95% confidence interval(95% CD 1.18-9.52]. Vehicle operation(OR 2.63, 95% CI 1.26-5.50), First aid(OR 2.18, 95% CI 1.03-4.59), and Futting out a fire, The risk on severity of lower back pain occurred in the order of Vehicle operation(OR 4.00, 95% CI 1.57-9.80), First aid(OR 1.88, 95% CI 1.00-4.81), Futting out a fire, and Rescue, These result suggest that duty type of fire fighters may be a prognostic factor for occurrence and severity of low back pain, The potential implications of these findings are discussed about different approach for primary and secondary prevention by duty type of fire fighters.

        기본인명구조술 교육방법에 따른 술기 수행능력의 차이

        The purpose of this study was to provide a more effective training method for BLS training in order to improve the competence of atheletic leaders and students who will perform basic life support(BLS) skills. The subjects of this study consisted of 38 atheletic students and were divided into two groups; one group learned BLS skills with the existing CPR program, and the other group participated in a new program of which the training course was designed on the basis of 2005 AHA BLS Guideline. Both training of basic CPR skills for adults and that of automated electrical defibrillation(AED) procedure were included in each program, which was taught by senior emergency medical technicians who had the certification of BLS Provider with experience in more than 3 years of clinical practice. To compare outcomes from two programs, SPSS Ver. 12.0 was used for the analysis of scores both from the evaluation sheet and the recording sheet, the latter had been obtained from a CPR mannequin, and t-test was performed to compare corresponding scores from two groups, at a significance level of p<.05. Analysis of CPR performance by the results from the evaluation sheet showed that a higher average scores in total were marked at the new program group than were at the existing program group, and a significant difference between two groups was found in some items, such as appropriateness of call for help, check for respiration, identification of chest compression site, rate of chest compression, ratio of respiration versus chest compression, reevaluation, and cessation of CPR. From analysis of CPR performance by scores on the recording sheet, it was revealed that outcomes of the new program reached more closely to the standard or valid scores, which were recommended as appropriate by AHA 2005 BLS Guideline, than did those of the existing program, and there was a significant difference between the two groups, in respect of the average rate of chest compression, the duration required for 5 cycles of CPR to be completed, and the correctness of chest compression(p<.05). As a result, the newly designed program of this study was shown to be a more effective than the existing conventional program in training CPR skills. It seems that the new program has stimulated the motivation for learning BLS skills, has integrated the knowledge about CPR skills into the CPR practice, has caused a better outcomes, and is considered as a program which will enhance the atheletic students' BSL performance in future.

        응급구조(학)과 교육과정의 표준화에 대한 연구

        Purpose: This study describes current curricula for paramedic students in South Korea and proposes a standardization of the curriculum. Methods: Data were collected from 38 colleges and universities from March 1 to 31, 2016. Descriptive statistics were calculated using SPSS 23.0. Results: The proposed standard curriculum was below. Requisite liberal arts consisted of 2 subjects and 6 credits including biomedical ethics, communications and human relationships. Common major subjects were composed of 6 areas, 22 subjects, and 78 credits. The areas of basic medicine consisted of 6 subjects and 16 credits including medical terminology. Introduction to paramedicine consisted of 3 subjects and 7 credits. Emergency patient management consisted of 2 subjects and 9 credits. Particulars to paramedic care consisted of 8 subjects and 31 credits. The law area consisted of 1 subject and 3 credits. Other major areas consisted of 2 subjects and 12 credits including integrated simulation and physician assistance. Common field practice area consisted of 3 to 4 subjects and 9 to 12 credits. Conclusion: It is important to establish and adapt a standardized curriculum for paramedic students in order to ensure competence and to provide high quality emergency medical services.

        Comparing with spinal anesthesia, saddle block anesthesia has little hemodynarnic effect and side effect. So saddle block anesthesia is widely practiced technique for simple anorectal area surgery. We experienced severe hypotension and bradycardia during saddle block anesthesia. And the patient was immediately treated and recovered without any adverseeffect. We report our clinical experience, including brief review of the relevant literature.

        4차 산업혁명 시대 응급구조(학)과 융복합 교과목 개발을 위한 연구

        Purpose: This study aimed to examine the development and improvement direction of university education according to job type and investigate the types of knowledge required for employment after graduation for departments of emergency medical technology in preparation for the era of the fourth industrial revolution in a converged society. Methods: From June 3 to June 10, 2019, data were collected through structured questionnaires from 90 paramedics working in firefighting, hospitals, different industries, and emergency patient transportation. Data were analyzed using the software SPSS version 18.0. Results: For employment after graduation in departments of emergency medical technology, character and sincerity education should be strengthened. To prepare for the fourth industrial revolution, educational innovations such as improving the field practice system and devising a convergence curriculum are needed. Preparation for the fourth industrial revolution should be accomplished through the development of convergence subjects such as forensic science, information and communications technology, and safety engineering. Ninety percent of the study respondents indicated their need for convergence subjects. Conclusion: The demands of future society should be responded to flexibly by holding and conducting convergence subjects. Furthermore, preparations should be made in anticipation of a shortage of 120,000 college freshmen for the 2024-2025 school year by increasing the competitiveness of departments of emergency medical technology.
