N/A Objectives: We anticipated that elderly diabetic patients showed different clinical features to that of adult diabetic patients. So, we investigated clinical features and beta cell insulin secretory characteristics of elderly diabetic patients compared to that of adult diabetic patients. Methods: Study population were 160 NIDDM patients who were admitted at National Medical Center from March, 1989 to February, 1991. We investigated clinical features of each group and performed 100g oral glucose loading test and measured blood glucose, insulin, C-peptide for 2 hours at 30 miniute intervals after glucose loading. Results: In each group, there was no significant difference in sex ratio, BMI (body mass index), WHR (waist-hip ratio) and lipid profile. The rates of diabetic macrovascular complications were significantly higher in elderly patients than those in adult patients. In comparison of signs and symptoms on admission, main symptom were polyuria, polydipsia, generalized weakness in adult group but chest pain and change of consciousness were main causes in elderly group. After 100 g oral glucose loading, the response of insulin and C-peptide were delayed in elderly group compared to adult group. Conclusion: We observed that elderly diabetic patients showed different symptoms on admission compared to adult patients. And after 100g oral glucose loading, the response of insulin and C-peptide were delayed in elderly group, despite no significant difference in BMI and WHR.

The frequency and severity of intestinal tuberculosis have been significantly decreased due to the development of anti-tuberculosis chemotherapy, the preventive medicine, the vaccination and early detection of pulmonary tuberculosis. Among the gastrointestinal tract, the duode- num is one of site rarely involved by tuberculosis. It is very difficult to rnake a correct diagno- sis of duodenal tuberculosis, and even more difficult for its early diagnosis due to its non-spe- cific syrnptoms. Recently, the authors have experienced a case of duodenal tuberculosis pre- senting duodenal ulcer bleeding and obstructive jaundice due to tuberculous lymphadenitis. The patient was operated on for bleeding from a duodenal ulcer and the diagnosis was estab- lished at laparotomy by the presence of tuberculous (caseating) granulomas with epitheloid cells and multinucleated giant cells (horse shoe shape) of the duodenum and pancreaticoduo- lenal lymph nodes. And then, the patient improved following the administration of anti-tuber- culous chemotherapy. So, we report this case with a review of literature. (Korean J Gastroen- terol 1994; 26: 573 578)

The side effects of non-steroidal anti-inflammatory drugs (NSAIDs) involve mainly gastroduodenal mucosal lesions, however, recent studies suggest that NSAIDs ingestion can cause also the small intestinal diseases, which are characterized sequentially by inflammation, ulceration, perforation, and diaphramatic disease. This study examined the change of intestinal permeability after short-term oral administration of indomethacin and co-administration of synthetic prostaglandin E₁ analogue, Misoprostol, in healthy men. The subjects drank test solution containing ^(51)Cr-EDTA(100 μCi), and this was followed by 10 hour urine collection. The amount of ^(51)Cr-EDTA in the urine reflects non-mediated intercellular intestinal permeability. Indomethacin significantly increased intestinal permeability to ^(51)Cr-EDTA, and co-administration of prostaglandin E₁ analogue significantly decreased this detrimental effect of indomethacin. These findings suggest that prostaglandins are involved in maintaining intestinal integrity in man.

Background/Aims: Stomach cancer is the most common malignancy and one of the leading cause of death in Korea. However, the prognosis of early gastric carcinoma(EGC) has been reported execellent and this is why we emphasize the early diagnosis and treatment of gastric cancer. Methods: We analyzed 109 patients of early gastric carcinoma which had been confirmed by surgery and pathological examination from January 1986 to December 1992 in the National Medical Center. Resluts: The incidence of early gastric carcinoma was 18.5% of gastric resection for stomach cancer. Male to female ratio was 2.0:1 with male predominance. The commonly involved age of patients was 50 to 59 years and mean age was 55.2. The epigastric pain was predominant symptom and followed by epigastric discomfort and indigestion. The macroscopic classification revealed that the most common types were Ilc(31.9%) and its combined types(41.4%). In sixty cases(51.7%), the lesions extended to the submucosa, while fifty five cases(48.3%) were confined to the mucosa. The incidence of metastases to regional lymph nodes in mucosal carcinoma was in 8.9% and that of submucosal lesions was 28.3%(p$lt;0.05). The most commonly involved sites were antrum(60.3%) and lesser curvature(35.3%) of the stomach. Forty six cases(39.7%) were between 2.1 cm and 4.0cm in diameter and 15 cases less than 1cm. The most common pathologic type was tubular adenocarcinoma(82.8%), followed by signet ring ce11 carcinoma and mucinous adenocarcinoma. Overall 5 year survival rate for the entire group was 9l.6%. Conclusion: The clinical and pathologic features of early gastric carinoma in the National medical Center werc not much different from those of previous reports, except that the incidence was 18.5% in our series and higher than those of previous reports.
