한의학적 치료로 호전된 하지부 폐색성동맥경화증 환자 치험 1례

Objectives: The purpose of this study was to investigate the clinical application of blood-letting therapy and oriental medicine with arteriosclerosis obliterans. Methods: The patient in this case had been already treated by anticoagulation, percutaneous transluminal angioplasty and thrombolysis. But these methods didn`t work on patient and condition had gone bad. Finally the patient`s left limb was about to be amputated. Patient did not want to cut off her lower limb, so she visited us to find a way for treating and preserving her lower limb. A drastic treatment was necessary in this situation. We treated the patient with Korean traditional treatment. Specially excessive blood-letting therapy and oriental medicine helped the patient to decrease the pain, numbness and frigidity. Results: After treatment, the chief complaint and accompanying symptoms were subsided and improved. And personal and social performance scale was increased. Conclusions: According to this study, Korean traditional treatment such as blood-letting theraphy, and oriental medication is effective for the cure of arteriosclerosis obliterans. During this period, foot necrosis was delayed and amputation is not needed any more.

The increase of fault current due to larger power demand has caused the capacity of power machines in grid to increase. To protect the power system effectively from the larger fault current, several countermeasures have been proposed. Among them, the superconducting fault current limiter (SFCL) has been expected as one of the most effective solutions. Therefore, to introduce SFCL into power distribution system, the analysis on protection in power distribution system with SFCL is essential. In this paper, the problems of the protective coordination in power distribution system with SFCL were described.
척추 수술 후 발견된 요장근 농양을 동반한 화농성 고관절염: 증례 보고

고령 환자에서 둔부 및 하지 동통을 호소하는 경우 고관절과 척추의 병변을 감별하기가 쉽지 않다. 본원 응급실로 방문한 78세 여자 환자는 타병원에서 척추관 협착증 수술을 받은 후에도 지속되는 우측 둔부 및 하지 통증에 대하여 보존적 치료하던 중, 수술 후 약 두 달째 시행한 방사선 검사상 대퇴골 두의 심한 손상 및 근위 대퇴골을 침범한 화농성 고관절염과 둔부 농양 및 장요근 내 농양이 확인되어, 추가적으로 시행된 요추 MRI상 척추 수술 부위의 농양이 발견되었다. 이러한 진단 및 치료의 지연을 피하기 위하여 척추 수술 후 지속적인 통증에 대한 원인으로서 고관절 병변의 의심이 필요하며 고관절 감염 발생 시 척추 및 장요근 감염의 동반 여부를 감별하기 위한 적극적인 진단적 평가가 필요하다. In the elderly patients who complain of pain in the buttock and leg, it is not easy to distinguish whether the pain comes from the lesion of the hip or from the spine. A 78-year-old female who was treated conservatively for persistent pain in the right buttock and leg after an operation for spinal stenosis in the local clinic visited our clinic. Septic hip arthritis with severe femoral head destruction and multiple abscesses in the buttock and iliopsoas muscle were diagnosed 2 months postoperatively, and spinal abscess in the site of the previous operation was detected by a subsequent MRI study. To avoid such a delay of the diagnosis and treatment, it is important to suspect hip joint lesion earlier for the source of persistent pain after a spine operation. Further more, diagnostic evaluation is necessary to rule out co-infection of the spine or iliopsoas muscle when a hip joint infection exists.
급성 경항통 환자의 흉쇄유돌근에 대한 근 에너지 기법(MET) 시술 후의 경근전도 변화 연구

ObjectivesTo evaluate the clinical utility of MET performed on sternocleidomastoid mus-cle of acute nuchal pain patients, we measured a change of meridian electromyography.MethodsWe compared electrical activity before and after MET performed on sternoclei-domastoid muscle of acute nuchal pain patients in same group (n=20) in isometric exercise state during five seconds. We analyzed amplitudes and areas of electrical activity and asymmetry index (AI).Results1. After MET performed on sternocleidomastoid muscle of acute nuchal pain pa-tients, maximum voluntary isomeric contraction (MVIC) was significantly increased more than before MET performed (p<0.005). 2. Comparing with before MET performed on ster-nocleidomastoid muscle of acute nuchal pain patients, muscle fatigue after MET performed on sternocleidomastoid muscle of acute nuchal pain patients decreased but there was no significant difference. 3. Comparing with before MET performed on sternocleidomastoid muscle of acute nuchal pain patients, asymmetry index (AI) after MET performed on sterno-cleidomastoid muscle of acute nuchal pain patients decreased but there was no significant difference.ConclusionsAccording to above results, after performing MET on sternocleidomastoid muscle of acute nuchal pain patients, maximum voluntary isomeric contraction (MVIC) in-creased significantly, so it is certain that performing MET on sternocleidomastoid muscle has a clinical effect. 여가 운동선수에서 발생한 전경골근 탈출의 Polypropylene Mesh를 이용한 수술적 치료

We report a case of tibialis anterior muscle hernia with large fascial defect of the leg in a recreational athlete, resulting from intense physical activity (soccer). The large fascial defect was closed with polypropylene mesh. Twelve months postoperatively, there was no recurrence of the hernia and any discomfort. We recommend the use of artificial mesh patches for the repair of larger defect which is unsuitable for direct closure.

The purpose of this paper is to analyze electrical safety of a low voltage direct current distribution system with 6-pulse three-phase thyristor rectifier. For analysing electrical safety. we performed modeling of the low voltage direct current distribution system with a 6-pulse three-phase thyristor rectifier using PSCAD/EMTDC and surveyed impedance of sources. transformers and distribution lines to run a simulation. In this paper, we verified the modeling of the low voltage direct current distribution system with a 6-pulse three-phase thyristor rectifier which is controlled PSCAD's PI controller, PLL, and a Interpolated Firing Pulse.

폐쇄형 경골 근위부 절골술 후 경골 후방 경사각이 경향이 있음은 잘 알려져 있으나 심한 전반슬이 발생하여 교정 수술이 필요하였던 증례에 대한 보고는 없었다. 내측 구획 퇴행성 슬관절염 및 내반슬을 가진 54세 환자로 타병원에서 폐쇄형 경골 근위부 절골술 후 잘못된 수술의 결과로 심한 전반슬이 발생하였으며 지속적으로 슬관절통과 심한 변형을 호소하였다. 이에 대하여 두 개의 쐐기형 이중피질 자가골 이식을 이용한 개방형 교정 절골술 및 이중 금속판 고정을 시행하여 만족할 만한 결과를 얻었다. 폐쇄형 경골 근위부 절골술시 내반의 교정뿐만 아니라 경골 후방 경사각의 변화에도 세심한 주의를 기울여야 할 것으로 사료된다. Although a closing wedge high tibial osteotomy can decrease the posterior tibial slope, there are no reports of severe genu recurvatum after a closing wedge osteotomy that required corrective surgery. A 54-year-old male with medial compartment osteoarthritis and genu varum was treated with a closing wedge high tibial osteotomy in another hospital, which led to severe recurvatum because of surgical failure. He complained of knee pain and a severe deformity, but of which were corrected by an open wedge corrective osteotomy, a two-wedge bicortical autograft reconstruction, and double plate fixation. Surgeons should pay close attention to both varus deformity correction and changes in the posterior tibial slope during a closing wedge high tibial osteotomy.
