신장이식 환자 남자 14례, 여자 6례에서 뇨중 neopterin을 연속적으로 측정하고 그 성적을 정상대조군 20례와 비교하여 다음과 같은 결과를 얻었다. 이식후 1주째의 뇨중 neopterin은 176.2±72.7 uM/mol of creatinine로서 이는 정상 대조군의 112.0±16.1 uM/mol of creatinine보다 유의하게 높았으나 이식후 11개월째의 뇨중 neopterin은 127.0±29.2 uM/mol of creatinine로서 정상 대조군과 유의한 차이가 없었고 이러한 성적은 3개월 및 6개월이상이 지난 뒤에도 계속되었다. 이식후 면역학적 합병증이 없었던 예에서는 뇨중 neopterin과 혈청 creatinine은 비슷한 변화를 나타내었다. 이식후 급성 거부반응이 있었던 1례에서 뇨중 neopterin은 혈청 creatinine의 상승보다 더 일찍 증가하였으며 치료에 잘 반응하는 경우에도 다른 임상증상 및 검사실 소견과 함께 호전상을 나타내었다. 이상의 결과로 미루어 볼 때 신장이식 환자에서 뇨중 neopterin검사가 신장이식후의 면역상태를 알알볼 수 있는 지표가 될 수 있으며 또한 급성 거부반응에서 면역학적 추적검사의 하나로서 유용할 것으로 생각된다. In order to evaluate the usefulness of urinary neopterin determination of immunological chanes after transplantation, urinary neopterin excretions were measured consecutively in 20 kidney-transplanted patients. The level of urinary neopterin excretion at one week after kidney transplantation was 176.2±72.7 uM/mol of creatinine which was significantly higher, compared with the level of 112.0±16.1 uM/mol of creatinine in 20 healthy controls. The elevated level of urinary neopterin in transplanted patients was decreased to 127.0±29.2 uM/mol of creatinine at one month after transplantation and the level remained stable thereafter. The changes of urinary neopterin levels were very similar to those of serum creatinine levels in immunologically uncomplicated transplanted cases. In one case with an episode of acute rejection around one month of post-transplantation, the rise of serum creatinine was preceded by the increase of urinary neopterin excretion, 2 days earlier. With the pulse theraphy of high dose methylprednisolone infusion, the levels of serum creatinine and urinary neopterin were normalized in same pattern, along with the improvement of clinical symptoms. It thus seems to be appeared that sequential measurement of urinary neopterin excretion would be one of useful means in immunological monitoring of allograft kidney recipients.

We experienced two families of polycystic kidney disease. Of the two families one case was associated with subarachnoid hemorrhage. We have reviewed the literatures of familial occurrence of polycystic kidney disease, and of the case associated with subarachnoid hemorrhage.
