본 연구는 급성기 뇌졸중 환자에서 연하장애 재활치료를 받은 환자와 받지 않은 환자의 회복정도를 비교하기 위함이다. 뇌졸중으로 진단받고 연하장애를 가지는 환자(n=98)에서 치료를 받은 군과 받지 않은 군으로 나누어 실험군과 대조군사이 연하장애 재활치료중재전-후 비교 연구 설계에 의해 실험군(n=52)에는 8주간의 연하장애 재활치료를 시행하였고 대조군(n=46)에는 아무런 처치를 하지 않았다. 두 집단 모두를 대상으로 중재 전-후 Video Fluoroscopy Swallowing Study(VFSS), Dysphagia Outcome and Severity Scale(DOSS), Dysphagia Severity Scale(DSS), Functional Outcome Swallowing Scale(FOSS)로 평가를 하였으며 평가값의 변화량은 paired t-test 및 ANCOVA를 통해 분석 하였다. 실험군의 중재전-중재후의 점수 변화비교에서 DOSS를 제외한 나머지 평가도구에서 연하기능이 회복되었다(p<0.05). 대조군의 전-후의 점수 변화비교에서는 모든 평가도구에서 점수값의 변화가 통계학적으로 유의하여 연하기능의 유의한 변화가 있었다(p<0.05). 실험군과 대조군 각각의 2차 평가를 비교한 결과 DOSS를 제외한 나머지 평가도구에서 변화가 통계학적으로 유의한 수준을 보였다(p<0.05). 본 연구 결과 대상자들은 평균적으로 급성기 8주 동안 연하기능의 회복을 보였고, 대조군에서는 세 평가도구 모두에서 유의하게 평가항목간의 점수가 상승하였다. 대상군들의 전-후 검사값의 차를 비교하였을 때 대조군에서 더 높은 점수의 상승을 보여 시간의 경과에 따른 자연적 신경학적 회복을 시사했다. The purpose of this study was to compare the effectiveness of rehabilitation therapy with natural recovery in patients who had dysphagia due to stroke. Swallowing functions were examined on a total of 98 patients with stroke-caused dysphagia based on a videofluoroscopic swallowing study (VFSS) for 8 weeks. The each group was evaluated with DOSS, DSS and FOSS to assess swallowing ability in the first week and re-evaluated in the 8th week during the session for investigating the recovery status. The author compared the above initial data and follow-up data using a Mann-Whitney U test, Kruskal-Wallis test and Wilcoxon"s Signed Rank test, ANCOVA to confirm statistically controlling for the effects of other continuous variables, performed by IBM SPSS Statistics 20.0 for windows. There was statistically significant recovery in the experimental group except measure of DOSS (p>0.05). In the control group, it showed improvement in all the scales(p<0.05). After terminating each session, the comparison of the two group of patients showed statistical significant differences in DSS and FOSS(p<0.05). These results suggest that possibility of natural neurologic recovery has positive effects on early stage of dysphagia caused by stroke.
상지 재활 로봇 응용을 위한 리니어 액추에이터 기반의 저항모드 시스템 설계

뇌졸중은 뇌혈관의 혈액순환 장애로 인하여 운동 조절(moto control)과 이에 따른 근력 감소 증상을 유발한다. 뇌졸중으로 인한 편마비 환자의 재활 치료는 대부분 치료사로부터 이루어지며, 주로 일상생활에 필요한 기초 근력 강화 훈련으로 구성된다. 이는 오랜 시간동안 같은 동작을 반복하기 때문에, 치료사의 피로도 증가와 환자의 동기부여가 감소될 수 있다. 따라서 본 논문은 로봇을 이용한 재활 운동을 통해 편마비 환자의 근력 강화가 가능하도록 저항모드(resist-mode) 기능을 탑재할 수 있는 시스템에 대해 제안한다. 이는 환자의 힘을 측정한 후, 저항 임계치(resistance-threshold)를 넘기는 힘을 인가하면 전단과 후단 방향으로 병진운동이 가능한 회로 기반의 제어 시스템을 구현한다. 시스템은 리니어 액추에이터와 로드셀을 기반으로 설계되었으며 이를 통해 환자의 근력 운동에 기여한다. 추후, 설계된 시스템에 가상증강현실 게임 환경을 도입하게 된다면 환자가 재활 운동을 하는데 동기부여를 증진시켜 응용가치가 클 것으로 기대된다. The stroke generally induces the moto control and resulting muscle loss symptom through the blood flow disorders in the cerebrovascular. The rehabilitation medical treatment for hemiplegic patients due to stroke is usually performed by a therapist and It consists of strength training mainly necessary for daily life. However, the therapist’s fatigue can be increased and patient’s motivation decreased since the same rehabilitation movement is repeated for a long time. In this paper, the proposed system for resist-mode performance with strengthen of muscle strength in rehabilitation exercise robot for hemiplegic is presented. The resist-mode implements a circuit-based control system to perform translational motion in the forward and back direction after measuring the patient’s force and applying a force that exceeds the resistance-threshold. The proposed reesist-mode system is designed based on a linear actuator and load cell, which contributes to the application of patient muscle strength exercises. If the designed system applied to introducing a virtual augmented reality game environment, it will bewidely used to help increases the motivation for the patient to perform rehabilitation exercise.
