철도안전관리에 위험도개념 적용을 위한 기반 요건에 대한 연구

In railway system, the first step of safety managements is the identification of current safety level. Among many safety indexes, risk concept is the prevailing and most effective one in many developed countries for the safety management. But many requirements such as, accident classification rule, risk levels or broadly acceptable regions, safety cost benefit analysis, VPF, equivalent fatality and failure database are needed for the application of risk concept in railway. Basic requirements for the application of risk concept on railway safety improvements are derived in this study.

Pressure tubes are major component of nuclear reactor, but only selected samples are periodically<br/> examined due to numerous numbers of tubes. Current in-service inspection result show there is high<br/> probability of flaw existence at un-inspected pressure tube. Probabilistic analysis is applied in this study for<br/> the integrity assessment of un-inspected pressure tube. But all the current integrity evaluations procedures are<br/> based on conventional deterministic approaches. So many integrity evaluation parameters are not directly<br/> apply to probabilistic analysis. As a result of this study failure assessment diagram are proposed based on test<br/> data.

In many countries safety management on track worker have been earned out after serious accidents and drastic safety improvement have been achieved as a result of safety management. In order to improve track worker safety in Korea, recent 5-year accident data were analysed and other countries" safety management systems were reviewed. As an result of this study 5-basic requirements for the safety improvement on track worker are derived: education, certification, clear responsibility definition, work planning and upgrade of train warning system.

Many railway safety measures reduce railway fatalities into half for last 10 years. But more improvement in railway safety is required to meet the social need after railway fire accident in Daegue. Reasonable railway safety index is needed to find out the weak point and efficiencies of various safety measures. But we still have only 2 kinds of safety index which are set for 10 years ago. In order to derive guideline for setting up safety index, we analyze accident cause, previous studies for railway safety improvement in Korea, and other countries safety plan. As a result some proposal for safety index was driven in this study.

Many developed countries use risk based safety management system. Currently many research efforts are made for the application of risk concept in safety management within the boundary of the railway safety act 2005. But results of these efforts can be applied around 2008 due to many background data and analysis techniques. For the prompt and efficient construction of mid and long term safety plan(2006~2010) various reports on safety issues and expert opinions are analysed in this study without risk analysis. Identification of railway safety issues are summarized and effective measure are proposed in this study.

Railway safety case is an important measure for demonstrates operating company"s capability, commitment and competence in accordance with Railway Regulation 2000 in UK. Safety case describes the scope of operation, definition and boundary of activity, risk analysis, system safety activity, annul safety plan. Relationships between these components and company"s policy, organization, human resource, control measure, investment plan are also included. A case study for Network Rail (infrastructure manager)"s safety case is described in this paper for the construction of safety rule and safety planning.
치수변화를 고려한 CANDU 압력관의 축방향 결함에 대한 파손확률 예측

The pressure tube is a major component of the CANDU reactor, which supports nuclear fuel bundle and heavy water coolant. Pressure tubes are installed horizontally inside the reactors and only selected samples are periodically examined during in-service inspection(ISI) due to numerous numbers of tubes. ISI results shows half of the pressure tubes have flaw, and axial flaw is dominant rather than circumferential flaw in pressure tube. In this respect, a probabilistic safety assessment method is more appropriate for the assessment of overall pressure tube safety. The failure behavior of CANDU pressure tubes, however, is governed by delayed hydride cracking which is not considered in the application of PFM on pipings and vessels. Since the delayed hydride cracking has more widely distributed governing parameters, it is impossible to apply a general PFM methodology directly. In this paper, a PFM methodology for the safety assessment of CANDU pressure tubes is introduced by applying Monte Carlo simulation in determining failure probability. Initial hydrogen concentration, flaw shape and depth, crack growth rate and fracture toughness were considered as probabilistic variables. Parametric study has been done under the base of pressure tube dimensions, which are derived from ISI data. Applying both unstable fracture condition and plastic collapse condition set failure criteria. The estimated failure probability showed about three-order difference with changing dimensions of pressure tube.

Railway accident analysis results show that accidents cased by human factors are not decreasing, whereas H/W related accidents are steadily decreasing. For the efficient management of human factors, many expertise on design, conditions, safety culture and staffing are required. But current safety management activities on safety critical works are focused on training, due to the limited resource and information. In order to establish railway human factors management requirements, human factors management status on all train operating companies are analysed in this study.

Recently many accidents have been occurred on track workers, these accidents have strong relationship with increase of train speed, electrification and multiple track portion. As a first step for the safety management, domestic and abroad track worker accidents data are analysed for the risk estimation of track worker. Analysis results shows that contact between track worker and train is the dormant reason. In order to reduce dormant reason fault trees are constructed in this study.
