<P><B>Summary</B></P><P>Tissue inhibitors of metalloproteinases (TIMPs) play key roles in maintaining homeostasis of the extracellular matrix by controlling matrix metalloproteinases (MMPs). In addition to their role in regulating MMPs, TIMPs have also been shown to have pluripotential effects on cell growth, apoptosis, and differentiation. The aim of this study was to evaluate TIMP-2 level in serous ovarian tumor tissues and to understand further the role of TIMP-2 protein in ovarian tumorigenesis. The expression of TIMP-2 was assessed by immunohistochemistry in a total of 57 ovarian specimens, including 5 normal ovaries, 12 benign serous cystadenomas, 20 serous borderline tumors, and 20 serous carcinomas. In addition, we transfected a TIMP-2 plasmid into the gynecologic cancer cell lines SKOV-3, 2774, and HeLa and then assayed cell growth, apoptosis, and MMP-2 activation. We found that TIMP-2 immunostaining was significantly more frequent in serous carcinomas, mainly in tumor epithelium, compared with cells of the other tissues studied. Tissue inhibitor of metalloproteinase-2 overexpression in ovarian cancer cells did not mediate proapoptosis, inhibited cisplatin-induced apoptosis, and induced MMP-2 expression. These findings suggest that TIMP-2 may function to favor tumor growth in serous ovarian tumorigenesis. Additional research is now needed to elucidate further the role of TIMP-2 in the biologic behavior of ovarian serous tumors.</P>

CDK7 as a potential therapeutic target for ovarian cancer; preclinical study

Objective: Cyclin dependent kinases (CDKs) are involved in temporal regulation of the cell cycle and transcription and play central roles in cancer development and metastasis. CDK7 activates cell cycle CDKs and is a member of the general transcription factor TFIIH. In this study, we evaluate the therapeutic effects of CDK7 inhibition in ovarian cancer using in vitro and in vivo models. Methods: We analyzed the clinical significance of CDK7 expression using TCGA data. Using CDK7 siRNA or covalent CDK7 inhibitor (THZ1), we performed the cell proliferation, apoptosis and the effects of cell cycle analysis in ovarian cancer cell lines including A2780, HeyA8, A2780-CP20 and RMG1. Based on in vitro results, we also performed in vivo experiments including cell line xenograft (A2780) and patient-derived xenografts (PDXs). Results: Higher CDK7 expression is significantly associated with poor progression-free survival of ovarian cancer patients in the analysis of TCGA data. CDK7 suppression with its siRNA or THZ1 significantly decreased cell proliferation and increased apoptosis with cell line experiments. Moreover, combination treatment with CDK7 inhibition and cisplatin decreased cell proliferation in drug-resistant A2780-CP20 cells. In the flow cytometric analysis to check the influence in cell cycle of CDK7 inhibition, we found that THZ1 triggered G0/G1 cell cycle arrest in a dose-dependent manner. In in vivo therapeutic experiment with cell line xenograft, CDK7 suppression with its siRNA or THZ1 could significantly decrease the tumor weight in A2780 and HeyA8 models. Moreover, we also found similar effect of this treatment in a PDX model with clear cell carcinoma. Conclusion: These results showed that CDK7 suppression with its siRNA or THZ1 could significantly suppress ovarian cancer growth in in vitro and in vivo by triggering G0/G1 cell cycle arrest. Therefore, CDK7 might be a novel promising therapeutic target for ovarian cancer and will be explored in the near future for therapy of ovarian cancer.
Q - switched Nd ; TAG 레이저를 이용한 오타 모반의 치료 효과

Background : The nevus of Ota is a benign dermal melanocytic lesion that was previously proved difficult to treat. Recently, the Q-switched ruby laser and Q-switched Nd: YAG laser have been reported to by, successful in the treatment of benign pigmented lesions and tattos. Objective : Our study evaluates the treatment of 60 patients with nevus of Ota with Q-switched Nd: YAG lase (l064nm). Method : Sixty patients with nevus of Ota were treated by the Q-switched Nd: YAG laser with pulse width 10nsec, and energy fluences ranging from 5.8 to 12J/cm and the laser repetition rate was 5 or 10Hz. The response to treatment was assessed by four independent investigators with photographs. Results : 1. Total therapeutic effects of Q-switched Nd: YAG laser showed excellent result in 23.3%, good in 45%, fair in 18,3%, and poor in 13.3%. 2. Therapeutic effects by distribution proposed by Tanino showed excellent or good results in 84.6% at type Ia, 78.6% at, type Ib, 66.7% at type II, 60% at type III and 45.5% at type N. 3. Therapeutic effects by the number of treatments showed excellent or good results in 22.2% of patients after one treatment, 72.7% after two treatments, 93.3% after three treatments and 100% after four treatment. 4. Therapeutic effects by energy showed excellent or good results in 9.2% at energy range of 10 to 12J/arl and 66.6%, at energy range of 5.8-9.5J/cm. 5. Complications included transient hyperpigmentation (6.7% ), secondary bacterial infection (1.7 %), hypopigmentation (1.7%) and atrophic scar (1.7%). However these subsided spontaneously in a year. 6. The recurrence rate was 2.4%. Conclusion : The Q-switc:hed Nd: YAG laser is a very effective mode of treatment for nevus of Ota which has provent unresponsive to other therapeutic modalities. Objective: Knotless, absorbable barbed suture devices have been widely used in gynecologic surgery, especially, suturing myometrium in laparoscopic myomectomy. MonofixTM has a triangular stopper which acts as anchor at the end of device. The objective of this study was to compare the biomechanical strength and histologic features of MonofixTM with other barbed suture devices in intradermal skin wound closure and fascial closure in porcine model. Methods: Sixteen female domestic pigs were randomly assigned to one of 4 groups designated by postoperative day of assessment. Twelve skin incisions were made along the dorsum in the thoracolumbar region of each pig with an equal number (n=4) randomly assigned to closure with 3-0 MonofixTM and controls. Pigs were euthanatized on postoperative days 3, 7, 14, and 28, respectively. The skin incision specimens were harvested for biomechanical testing and histologic evaluation. Five female domestic pigs were included in fascial closure group. Each had two paramedian incisions, one fascial incision was closed with 2-0 MonofixTM, and the other was closed with control. At 6 weeks, the fascia excised and the suture line were tested with histologic evaluation. Results: In the ex-vivo biomechanical analysis of intradermal closure, MonofixTM shows similar tissue tensile strength compared with other commercially used barbed suture devices, regardless of day. All three test articles resulted in mild tissue reaction scores in histology, however, the average tissue reaction score was no difference between MonofixTM and controls. There were no significant complications in all three suture devices. In fascial closure group, there were no wound infections and incisional hernias. Also, the average tissue reaction score of fasical closure group was no difference between MonofixTM and control. Conclusion: This preclinical study suggests that the use of barbed suture devices with stopper (MonofixTM) can be a safe and efficacious alternative compared with other barbed suture devices. Objective: Ovarian clear cell adenocarcinoma (Ov-CCA)has a significant ethnic difference between Western populations and Japanese. Understanding of specific genetic alterations which associated with ethnic differences could lead to understanding of pathogenesis in Ov-CCA. The purpose of this study is to investigate the single nucleotide polymorphisms (SNPs) and the associated genes which correlated with the Japanese Ov-CCA using expression quantitative trait loci (eQTL) analysis with the publicly available datasets. Methods: The differences of genotype between three races (Western, Chinese, and Japanese) were compared using eQTLs analysis with the HapMap3 and the 1000 genome project datasets. 4 SNPs and 7 associated genes were identified. To evaluating the clinical significance of 7 identified genes in clear cell histology, the survival analysis according to the gene expression were perfomred with kidney renal clear cell carcinoma (KIRC) and high grade serous ovarian carcinoma (HGSOC) datasets in TCGA database. Results: Of the 935 identified cis-eQTLs, 4 SNPs (rs4873815, rs12976454, rs11136002, and rs13259097) had the different frequencies of alternative allele among Western, Chinese and Japanese with statistically significant (all p < 0.001) and these 4 SNPs were associated with 7 transcripts (APBA3, C8orf58, KIAA1967, NAPRT1, RHOBTB2, TNFRSF10B and ZNF707). To investigate the clinical significance, survival analysis was done using TCGA dataset and the expressions of the 3 genes (ZNF707, TNFRSF10B and RHOBTB2) were correlated with overall survival in KIRC (p < 0.001, p < 0.001 and p = 0.007) which were not shown in HGSOC. Conclusion: In this study, we identified that the expression of ZNF707, TNFRSF10B and RHOBTB2 were correlated with overall survival in clear cell histology although the KIRC dataset were used. eQTL analysis using the publicly available datasets may facilitate the discovery of novel genetic alteration in Ov-CCA and provide the basis for the understanding of Ov-CCA pathogenesis.

Term of gossypiboma (textiloma, cottonoid, gauzoma, and muslinoma) is used to describe a retained surgical pad or gauze in the body after an operation. Retained postoperative foreign bodies, of which sponges are the most common, is a preventable condition and almost always iatrogenic. Gossypiboma may present acutely or subacutely and sometimes it may be symptom free. We present a case of abdominal gossypiboma 20 years after an appendectomy. A 44-year-old woman presented with irritability, abdominal discomfort. A forgotten surgical gauze in the left side of the pelvis was seen on abdomen-pelvis x-ray. She had a past history of acute appendicitis operated on 20 years earlier and had vague abdominal discomfort and anorexia after the operation. Computed tomography scan revealed a foreign body measured 6.2 cm with heterogeneous internal structure in the left iliac fossa that had been surrounded by the small intestine and the colon. Laparoscopy was done with a 12-mm suprapubic port and three 5-mm ports in the left, right lower quadrants and infra-umbilical areas. The small bowel loops adhered to the gauze, which together made up a mass. Using sharp dissection, bowel loops were carefully separated. The gauze had decomposed into a semi-liquid material consisting of hundreds of small particles and could not be removed in one piece; a basket was used.
