Conventional face lift consisted essentially of removing facial wrinkles and reducing flaccidity but also the loss of defined facial contours by senile distribution of facial fatty tissue. Large amount of fat was accumulated on submental, cervical, parotid and submandibular area during aging process. In order to achieve a aesthetically pleasing results in facial rejuvenation, redundancy in the cervicomental area is of significant concern to many surgeon. It is important for the patient not only to reduce skin flaccidity, but also to reduce the bulkiness in the submental and submandibular angle. And most especially, the contour of the cervicomental region and jawline are corrected. Among the various methods to solve this problem, we use the open liposuction technique to reduce the adipose tissue with cannula orifice away from skin flap and lipectomy with face-lift scissors. It allows for better contouring along the angle of the mandible and helps eliminate or markedly reduce the puckered look that can occur immediately after face lift when a plication technique is employed. Using this open technique, supraplatysma fatty tissue can be identified and removed after elevating the skin flap through of extensive subcutaneous dissection. The advantages of our method are summarized as follows 1) Effective and easy removal of senile distribution of neck and facial fat by direct visualization after skin flap elevation 2) No submental scar 3) Allowing for better contouring of cheek, cervicomental area 4) No tendency to facial nerve and flap injury 5) Uniformly good results was obtained
