        경추통 환자에 대한 신장운동과 도수치료가 즉각적인 통증과 ROM에 미치는 효과

        Purpose: Cervical pain is caused mainly by a static position, lasting work, bad habits and stress, and is accompanied by pain in the upper trapezius. Traditionally, heat and traction, exercise, mobilization, manipulation have been used to treat cervical pain. This study examined the effect of stretching and manipulation (high-velocity low-amplitude: HVLA) on the pain and ROM in women with chronic cervical and upper shoulder pain. Methods: Fifty-two women diagnosed with chronic cervical and upper shoulder pain were enrolled in this study. Among them, 26 patients (experiment group) were managed by passive stretching and manipulation, and another 26 patients (control group) were treated with physical therapy intervention (hat pack: HP, transcutaneous electrical nerve stimulation: TENS, ultra sound: US). Each group made use of a Visual Analogue Scale (VAS) and the highly reliable (flextion: 0.92, extention: 0.99) cervical range of motion (CROM) to compare the possible changes in pain and ROM in the two groups after treatment. Results: In the experimental group, the pain decreased and the ROM has increased in all directions. In the control group, the pain decreased but the ROM was not changed in all directions. Conclusion: According to the results, passive stretching and manipulation is effective for increasing the ROM and decreasing the level of pain. Moreover, physical therapy intervention (HP, TENS, US) is effective for immediately decreasing the pain but has little effect on the ROM.

        2주간의 도수치료와 요부 안정화 운동이 만성 요통환자들의 요부관절 가동범위와 통증에 미치는 영향

        The purpose of this study was to investigate on the lumbar Range of Motion(ROM) and pain with Chronic Low Back Pain(CLBP) used to manipulation and lumbar stabilization exercise for 2 weeks. We introduced Maitland`s manipulation and lumbar stabilization exercise in this study in order to recover the CLBP with pain and lumbar ROM. Forty-eight patients diagnosed with CLBP were included for the study. Among them, twenty-four patients(experimental group) were managed by Maitland`s manipulation and lumbar stabilization exercise, another twenty-four patients(control group) were treated by physical science methods(H/P, TENS, U/S). Each group made use of Visual Analogue Scale(VAS) to pain and tape to lumbar ROM to compare the possible changes of pain and lumbar ROM in two groups after treatment. In experimental group, lumbar ROM has increased in flexion extension, left bending, right bending and pain reduced for 2 weeks. In control group, lumbar ROM has not changed in flexion, extension, left bending, right bending and pain has reduced for 2 weeks. According to the results, we concluded that Maitland`s manipulation and lumbar stabilization exercise for 2 weeks is effective for increasing lumbar ROM and reducing pain by making joints normal position in lumbar vertebrae and lumbar stabilization in spinal muscles.

        회내발에 대한 관절가동술과 능동운동이 근활성도와 동적 균형능력에 미치는 영향

        A foot supports body weight moreover working. Because unbalance body weight and abnomal walking, Foot transformation can decrease dynamic balance with unbalance body weight and abnomal walking. Transformation foot is supination and pronation foot. This study carry out in order to normal anatomical balance and function increase in transformation foot. Experimental (pronation foot) group 17, control(normal foot) group 17 made an experiment in 4weeks. Experimental group made intervention sub-talar joint mobilization and ankle joint used Active exercise (meidal & lateral muscule stretching and isometric exercise) and control group not intervention. Each group made use of EMG and highly reliable(0.88~0.91) star excursion balance test(SEBT) and highly reliable(0.81~0.96) navicular drop test(NDT) to compare the possible changes of muscle activities and dynamic balance ability in two groups after treatment. In experimental group, Peroneus longus muscle activities has increase but in control group, not changed(p<.05). In experimental group, dynamic balance ability has increased in forward, rightward, leftward, and backward, but in control group, not changed(p<0.5). According to the results, we concluded that sub-talar joint mobilization and ankle joint used Active exercise is effective for increasing dynamic balance ability by making muscle activities more increase in peroneus longus.

        동결 견 환자에 대한 보존적 물리치료와 관절 가동술의 병행이 주관적 통증지수와 관절가동범위에 미치는 효과

        동결 견은 통증과 관절가동범위의 감소를 임상적 특징으로 한다. 동결견의 치료목적은 통증감소와 감소된 관절가동범위를 증가함으로 기능적인 동작을 수행하게 하는데 있다. 따라서 본 연구에서는 동결 견으로 진단받은 26명의 환자를 대상으로 보존적 물리치료(온열, 전기, 초음파)와 관절 가동술을 적용한 후 주관적 통증지수와 관절가동범위에 미치는 효과를 알아보고자 실시하였다. 실험군 13명에게는 보존적 물리치료와 관절 가동술을 병행해서 4주간 주3회 적용하였고 대조군 13명에게는 보존적 물리치료를 4주간 주3회 실시한 후 주관적 통증지수와 관절가동범위를 측정하였다. 실험군에서는 주관적 통증지수의 감소와 외전, 외회전, 내회전의 관절가동범위가 증가하였다. 대조군에서는 주관적 통증지수는 감소하였지만 관절가동범위는 차이가 없었다. 군 간의 비교에서는 주관적 통증지수에서는 차이가 없었고 외회전, 내회전의 관절가동범위가 증가하였다. 따라서 보존적 물리치료와 관절 가동술의 병행이 통증감소와 외전, 외회전, 내회전의 관절가동범위의 증가, 특히 외회전과 내회전의 관절가동범위의 증가에 효과적임을 보여 준다. Frozen shoulder is clinically characterized by pain and reduced ROM. The therapeutic goal of frozen shoulder can be reducing pain and increasing ROM in shoulder joint, resulting in improvement of joint movement. Therefore, this study was conducted to know the effect of mobilization in parallel with conservative physical therapy(H/P, TENS, U/S) in patients with frozen shoulder on subjective pain scale and ROM. 26 patients diagnosed with frozen shoulder were included for study. Among them, 13 patients(experiment group) were managed by mobilization as well as conservative physical therapy, another 13 patients(control group) were only treated by conservative physical therapy alone. All the patients were treated three times a week for 4 weeks, and after each treatment subjective pain scale and ROM were measured. In experimental group, pain has decreased and ROM has increased in abduction, external rotation and internal rotation. In control group, pain has also decreased but ROM has not changed in abduction, external rotation and internal rotation. There were no significant difference in subjective pain scale between two groups but some difference in ROM (external rotation, internal rotation).According to the results, we concluded that both conservative physical therapy and joint mobilization technique are more effective for increasing ROM especially external rotation, internal rotation than conservative physical therapy alone.

        메이트란드 도수치료가 만성 요통환자의 즉각적인 동적 균형능력에 미치는 영향

        만성 요통 환자들에 대한 다양한 치료적 중재 방법이 있다. 그러나 대부분의 치료들이 통증에 대한 치료들이다. 만성 요통 환자들은 요추관절들의 구조적 고착들을 가지고 있다, 이러한 구조적 고착들은 관절가동범위을 저하시키고 나아가 동적 균형능력의 저하를 발생시킨다. 동적 균형능력의 저하는 손상을 가중시키고 위험상황에서의 대처능력을 감소시킨다. 따라서 본 연구에서는 만성요통환자들에 대한 즉각적인 동적 균형능력의 회복을 위하여 메이트란드의 도수치료의 방법들을 이용하였다. 만성 요통 환자 64명을 대상으로 메이트란드의 도수치료를 적용한 실험군 32명, 이학적 도구(온열팩, 저주파치료기, 초음파)를 사용한 대조군 32명을 대상으로 하였다. 각 군의 치료 전, 후의 동적 균형능력을 비교하기위하여 신뢰도가 높은 스타 익스커션 밸런스 테스트(0.81-0.96)를 이용하였다. 실험군에서 치료 후 동적 균형능력이 전방, 오른쪽, 왼쪽, 후방모두에서 증가하였고 대조군에서는 변화가 없었다. 따라서 메이트란드의 도수치료가 만성 요통환자들의 요추 관절을 유연하게 해서 즉각적인 동적 균형능력을 증가시키는데 영향을 주었다고 생각된다. There has been a diversity of therapeutic interventions for patients with chronic low back pain. However most of them have been focused on only pain. But the patients usually have a kind of structurally abnormal have fixed in lumbar vertebrae, which can decrease range of joint movement and, what is worse, reduce immediate dynamic balancing ability(IDBA). The reduced IDBA can increase the risk of injury and decrease coping ability in dangerous situation. So we introduced Maitland's manual therapy in this study in order to recover the IDBA in patients with chronic low back pain. Sixty-four patients diagnosed with chronic low back pain were included for the study. Among them, thirty-two patients(experimental group) were managed by Maitland's manual therapy, another thirty-tow patients(control group) were treated by physical science methods(H/P, TENS, U/S). Each group made use of highly reliable(0.81~0.96) star excursion balance test(SEBT) to compare the possible changes of IDBAs in two groups after treatment. In experimental group, IDBA has increased in forward, rightward, leftward, and backward, but in control group, not changed. According to the results, we concluded that Maitland's manual therapy is effective for increasing IDBA by making joints more flexible in lumbar vertebrae.

        수중 걷기 운동이 우측 편마비 환자의 발 운동학과 보행 속도에 미치는 영향

        본 연구는 편마비 환자에게 수중 걷기 훈련이 미치는 영향에 대해 알아보고자 10주간 수중 걷기 훈련과 지상 걷기 훈련 후 족저압, 거골하관절의 움직임, 보향각, 보행 속도를 측정하였다. 대상자는 20명으로 수중걷기 훈련 그룹(n=10)이 엄지발가락 영역, 뒤꿈치영역, 발허리부분의 족저압이 유의하게 증가하였고, 거골하관절의 움직임과 보향각이 안정화되었으며, 보행 속도 또한 증가함을 보였다. 보행 속도의 증가와 거골하 관절의 움직임 안정화와 보향각의 감소는 수중 걷기가 편마비 환자의 보행 속도 뿐만아니라 보행의 안정화에도 영향을 미친다고 생각되어진다. 또한 엄지발가락 영역과 뒤꿈치 영역의 족저압 증가는 보행시 뒤꿈치 닿기와 발가락 밀기 동작의 회복으로 해석되어진다. 이와 같은 결과로 볼때, 현재 사용되고 있는 치료사에 의한 전문적인 물리치료를 받지 못하는 환자들의 경우 스스로 수중 걷기 훈련만으로도 지상걷기에 비하여 많은 효과를 볼 수 있을 것으로 기대된다. The purpose of this study was to investigate the effect of aquatic gait training on plantar foot pressure, foot kinesiology and gait speed in right hemiplegic patients. The subject were 20 stroke patients who elapsed from 12 month to 24 month after stroke(aquatic gait training group(n=10), land gait training group(n=10)). This study measured plantar foot pressure, toe out angle, subtalar joint angle, gait speed from data of gate on 2m long measuring apparatus for RS-scan system(RS scan Ltd. German). This experiment performed in twice, before and after the aquatic gait training and land gait training. Collected data were statistically analyzed by SPSS Ver. 12.0 using descriptive statistics, paired t-test. Aquatic gait training group had more variety pressure area on their foot such as T1(Toe 1), HM(Heel medial), and HL(Heel lateral). But motion of subtalar joint flexibility and toe out angle decreased considerably and gate speed also increased. According to the result, aquatic gait training is considered as more effective way in foot stability and normal gait pattern than land gait training.
