하지부동이 있는 감염성 고관절염의 후유증에서 인공 고관절 전치환술의 추시 결과

목적: 심한 변형을 동반한 감염성 고관절염의 후유증 환자에서 길이 교정을 동시에 시행한 인공 고관절 전치환술의 최소 3년 이상 추시 결과를 분석하과 한다. 대상 및 방법: 1987년 12월부터 2003년 5월까지 수술한 102예 중 최소 3년 이상 추시가 가능한 74예를 대상으로 술 후 합병증 및 임상적, 방사선학적 결과를 분석하였다. 평균 연령은 42.8세였으며, 평균 추시 기간은 4년 11개월(3년 0개월-15년 4개월)이었다. 원인은 결핵성 감염이 50예, 화농성 감염이 24예였으며 강직인 경우는 23예였다. 하지 진성 단축은 평균 4.1 ㎝ 0.0-10.0 ㎝)였다. 결과: 임상적으로 Harris hip score는 수술 전 평균 65.0 (25.0-92.5)에서 수술 후 평균 90.2 (80.2-98.2)로 개선되었으며, 하지 진성 단축은 평균 0.4 ㎝ (0.0-3.0 ㎝)로 교정되었다. 방사선학적으로 재치환술을 시행한 3예를 제외하고 완전 해리 범주에는 해당되지 않았다. 술 후 합병증으로 결핵의 재발이 2예, 신경마비는 대퇴 신경 마비가 3예, 좌골 신경 마비가 3예 있었으나 현재 완전 치유되었고, 술 전 미확인된 대퇴 동맥 폐색이 있어 부행 동맥으로 혈액 순환이 되던 1예에서 수술 후 혈액 순환 장애가 악화되어 하퇴 절단술을 시행하였다. 결론: 감염으로 인한 후유증으로 심한 하지 단축 및 변형을 보이는 환자에서 길이 교정을 동시에 시행한 경우 수술 수기가 어려울 뿐 아니라 술 후 합병증도 비교적 높게 예상되나, 장기적으로 보면 술 후 환자의 만족 및 보행의 개선을 이룰 수 있었다. Purpose: To analyze the results of a minimum of three-year follow-up in patients with a severely deformed sequelae pyogenic hip, who had undergone total hip arthroplasty with a simultaneous limb length correction. Materials and Methods: The postoperative complication, clinical and the radiographic results were analyzed in 74 out of 102 patients, who were followed for a minimum of three years between December 1987 and May 2003. The average age was 42.8 years. The average follow-up duration was 4 years and 11 months. There were 50 tuberculosis infections, 24 pyogenic infections, and 23 fused hip cases. The average leg length shortening was 4.1 ㎝ (range, 1.0-10.0 ㎝). Results: Preoperative Harris hip score improved to 90.2 postoperatively from 65.0 (range, 25-92.5) preoperatively. Leg length shortening was corrected by 0.4 ㎝ (0.0-3.0 ㎝). Radiographically, excluding the 3 revisions, no case met the criteria of definite loosening. The postoperative complications included the recurrence of tuberculosis (2 cases), and variable nerve palsies, including femoral nerve (3 cases), and a sciatic nerve (3 cases). These patients completely recovered from nerve palsy. B-K amputation, after the postoperative exacerbation of circulation to the affected limb, was needed in one case with a preoperatively undetected femoral artery occlusion. Conclusion: Although, restoring the normal limb length in the severely deformed pyogenic hip sequelae patients requires highly demanding surgical techniques, with a relatively higher incidence of postoperative complications, it is possible to achieve a higher level of patient satisfaction and definite gait improvement.
