        HBsAg 양성인 환자에서 Pre - S ( 2 )Ag 의 임상적 의의

        N/A The Pre-S(2) gene in the HBV DNA genome encodes a peptide in the HBV envelope which is called Pre-S(2) Ag, and this antigenie determinant stimulates the formation of anti-Pre-S(2). It has been reported that Pre -S(2)Ag appears in the early phase of acute viral hepatitis with HBV DNA, HBeAg, and there is a correlation between the titer of HBV DNA and that of Pre-S(2)Ag. We collected a total of 73 cases with a diagnosis of acute viral hepatitis, chronic hepatitis, liver cirrhosis, or asymptomatic carrier from April to September 1988 in order to check Pre-S(2)Ag, anti-Pre-S(2), HBsAg, anti-HBs, HBeAg, and anti-HBe, HBV DNA. The following results were obtained: 1) Pre-S(2)Ag was seen in 60 cases(82.1%) of 73 HBsAg positive patients. 2) Anti-Pre-S(2)Ag was not detected in all cases. 3) Pre-S(2)Ag was seen in 92.3% of HBeAg patients patients and in 60% of anti-HBe positive patoents. There was a statistical significance between the two groups (p < 0.05). 4) The mean value of Pre-5(2)Ag was 1.047±1.007 O. D. in HBeAg(+) patients and 0.369+0.463O.D. in anti -HBe(+) patients. There is a statistical significance between the two groups(p<0.01). 5) Pre-S(2)Ag was seen in 92% of HBV DNA(+) patients and in 75% of HBV DNA(-) patients and this was a statistical significance (p<0.05). 6) There was no relationship between the titer of HBV DNA and Pre-S(2)Ag. 7) The mean value of Pre-S(2) Ag w as 1.201±0.93 O.D. in HBV DNA(+) patients and 0582±0.785O.D. in HBV DNA(-) patients. There was no statistical significance. In concluion, Pre-S(2)Ag seemed to reflect the replication of the virus but not the degree of replication. Pre-S(2)Ag was suggested to have no more significance than HBV DNA or DNA polymerase, Pre-S(2)Ag disappeared later than HBV DNA, but earlier than HBsAg. We need a long-term follow-up study to deter- mine prognostic value with anti-Pre-S(2).
