        동상의 임상적 분석

        Purpose: Frostbite can affect still soldiers. Initial clinical manifestations are similar for superficial and deep frostbite, so early treatment is identical. It is under-estimated by physicians. We try to identify the challenges of managing these complex tissue injuries. Methods: A retrospective analysis of 84 patients hospitalized at AFCH from 2009 to 2015 was conducted. We investigated differences of epidemiological characteristics, identification of soft tissue injury, treatment and complications between superficial (SF: 43; 51.2%) and deep (DF: 41; 48.8%) frostbite. Results: The major (94.0%) developed frostbite in dry circumstances (89.3%). Wet circumstances (66.7%) were more susceptible to DF rather than dry (46.7%). The 38 (45.2%) arrived to specialist within 7days. Most prone sites were feet, followed by hands. Toes had more deep injuries. DF presented more increased levels of ALT, CPK, CKMB, CRP. The bone scan of W+S+ was 48.3%, 87.1% and W+S- was 20.7%, 12.9%, respectively. The treatment resulted in improved or normalized perfusion scan with matching clinical improvement. It was a good tool to assess treatment response. Eighteen normal and 8 stenotic type of PCR resulted in normal with matching clinical improvement. One continuous obstructive waveform led to minor amputation. Twelve underwent both PCR and MRA. Among 6 normal PCR, 5 showed normal and one stenosis in MRA. All 5 stenosis and one obstruction showed the same findings in MRA. It was a good tool to evaluate vascular compromise. They were treated with rapid rewarming (11.6%, 22.0%), hydrotherapy (16.3%, 29.3%), respectively. Six (14.6%) underwent STSG, 2 (4.9%) had digital amputation in DF. Berasil, Ibuprofen, Trental were commonly administered. PGE1 was administered selectively for 6.8, 10.8 days, respectively. Raynaud’s syndrome (16.3%), CRPS (4.7%), LOM (14.6%) and toe deformity (4.9%) were specific sequelae. Conclusion: We should recommend intensive foot care education, early rewarming and evacuation to specialized units. The bone scanning and PCR should allow for a more aggressive and active approach to the management of tissue viability. [ J Trauma Inj 2015; 28: 158-169 ]

      • Effect of Cadmium on Renal Organic Anion Transport In vitro

        가토 신피질 절편에서 p-aminohippurate (PAH) 이동에 대한 카드뮴 이온(Cd)의 영향을 조사하여 다음과 같은 결과를 얻었다. 조직절편과 용액내의 PAH 농도비(S/M PAH)는 용액내에 Cd이 0.1 mM이상 존재할 때에 현저히 감소되었다. PAH influx에 대한 동력학적 분석결과 Cd에 의하여 influx의 최대이동율(즉 $V_{max}$)은 심하게 감소되지만 운반체와 기질간의 친화력(즉 $K_{m}$)에는 변화가 없었으며, 수동적 influx 역시 변화되지 않았다. 신피질조직의 산소 소모율은 1 mM Cd에 의하여 35% 가량 억제되었으며, 신피질 microsome 분획의 Na-K-ATPase 활성도는 Cd 농도가 $10^{-7}M$ 이상일 때 의의있게 억제되었다. 이상의 결과로 미루어 볼 때 신장조직이 카드뮴이온에 직접 노출될 경우 유기음이온의 능동적 이동능이 심하게 저해된다고 사료된다.

        내건 내서성 감자 ‘아리랑 2호’

        In 2018, a new potato variety, ‘Arirang-2’, was released by the Highland Agriculture Research Institute in Pyeongchang, Korea. To create new varieties with better tuber formation at high temperature under long day, a cross was performed between high-temperaturetolerant ‘Eigenheimer’ and drought tolerant ‘Daegwan 1-116’ in 2011. The shoot (57.9 cm) of ‘Arirang-2’ was taller than ‘Superior’ and‘Sante’, and its tuber skin and flesh were red and light-yellow colored, respectively. Average number of tubers per plant of ‘Arirang-2’ was9.1 while ‘Superior’ and ‘Sante’ had 5.4 and 14.1 tubers, respectively. Average weight of tuber of ‘Arirang-2’ was 98.1 g, which was slightlyheavier than that of ‘Superior’ (90.0 g), and much heavier than that of ‘Sante’ (38.9 g). Total weight of tuber per plant of ‘Arirang-2’ was892 g, which was remarkably higher than that of ‘Superior’ (486 g) and ‘Sante’ (549 g). Total yield (TY) of ‘Arirang-2’ measured in Gangwon-Do,Korea was around 5,022 kg/10a while that of ‘Superior’ was 3,036 kg/10a. In Kazahstan, total yield of ‘Arirang-2’ was 4,657 kg/10a andthat of ‘Sante’ was 4,155 kg/10a. Marketable tuber yield (MTY) of ‘Arirang-2’ was 3,119 kg/10a in Gangwon-Do, Korea while that of ‘Superior’was only 2,147 kg/10a. In Kazahstan, MTY of ‘Arirang-2’ was 3,162 kg/10a, which was 60% higher than that of ‘Sante’ (1,974 kg/10a). ‘Arirang-2’ is resistant to early blight (Alternaria solani) and late blight (Phytophthora infestans) compared to ‘Superior’ and ‘Sante’, andit shows tolerance to drought or high temperature. ‘Arirang-2’ was approved as a new potato cultivar by The Korea Seed and Varieties Serviceon March 3, 2021 (Registration No. 8444).

      • 한 점의 피부온도를 이용한 평균피부온도의 산출

        Mean skin temperature (Tsk) is a common parameter in the study of human's temperature regulation. The purpose of this study was to propose a conventional formula to estimate Tsk from one point local skin temperature (Ts). Infrared thermography was carried out in 10 naked males at rest while decreasing the room temperature from 40℃ to 4℃ at 4℃ intervals. Tsk and Ts were measured from 110 whole-body infrared thermograms using an image processing system. Ts was taken at 17 different sites as the temperature of one pixel, corresponding to 1.24㎠ plane area. Tsk was measured as the mean temperature of 10,841 pixels that was equivalent to about 88% of total body surface area, and it changed from 34.2℃ to 24.4℃ in average. Each of the Ts was linearly proportional to Tsk (r = 0.92∼0.99, p<0.001), suggesting that Tsk can be estimated from one point Ts. The standard error of estimates in 17 linear regression equations were ±0.4∼1.3℃. By the way, the physiologically reliable error range in Tsk estimation was assessed to be valid at below 0.5℃. In the estimation of Tsk using one point Ts, therefore, we proposed 4 different linear formulas of which the standard error of estimate was less than 0.5℃. The accuracy of the proposed formulas was 75∼85% in the error range of ±0.5℃ and 95∼99% in the error range of ±1.0℃, respectively. However, because of lower accuracy than 95% in the physiologically reliable error range, the proposed formulas should be suitable to use in the field and its corresponding studies.

      • 잠수복 착용 피검자에서 침수시 잠수장갑 및 버선 착용에 따른 체온조절기능의 변화

        In 5 wet-suited subjects, changes in body temperature, heat exchanges and thermal insulations were studied at rest and during exercise in water of 13℃. The subjects wore neoprene gloves and boots in one series and exposed their hands and feet to water in the other series of experiments. In the subjects resting in water, the gloves and boots did not affect the level of rectal temperature but increased the mean skin temperature slightly, reducing the core-to-skin temperature gradient. However, the total thermal insulation of the subject was reduced significantly and consequently the rate of skin heat loss was increased by wearing gloves and boots. The metabolic response of the subject appeared to be significantly augmented by wearing gloves and boots. In exercising subjects the level of body temperature and the thermo-regulatory variables were not apparently changed by wearing gloves and boots. These results indicated that in wet-suited subjects resting in cold water, exposure of hands and feet increases peripheral insulation and consequently effectively reduces heat loss to water. Such an effect of peripheral cooling is lost during exercise by hypermia to skeletal muscles in the limbs.

        수종의 치수 처치제가 백서의 결체조직에 미치는 자극에 관한 비교 연구

        Tissue irritation potential of several pulpotomy medicaments(2, 5, 10% glutaraldehyde and 1 :5 diluted formocresol) during various time intervals(1, 7, 14, 28 days) in sixteen Sprague-Dawley rats were evaluated histologically under light microscope. No significant differences in the degree of irritation could be found between 2, 5, and 10% glutaraldehyde groups, expect 1 day-10% glutaraldehyde group which showed higher inflammatory response than other 1 day-glutaraldehyde groups. Specimens of 1 : 5 diluted formocresol groups were shown to be highly irritating to connective tissues in general and the inflammatory responses persisted through out the whole test periods. From the results of the present study, the clinical application of 2% glutaraldehyde being recommended as an adequate concentration might be reconsidered and the possible use of higher concentration could be thought.

        지혈적 치수 절단술의 효과에 관한 조직학적 연구

        The efficacy of several polpotomy methods were svaluated histologically on animal model using 6 beagles. At 1, 4, 6 weeks after pulpotomy, animals were sacreficed by perfusion method. Histomorphometric analysis was performed using computerized image analyzing system. Statistical comparisons were done using SPSS program. The following results were obtained: 1. Tissue responses after ferric sulfate treatment mainly consisted of fibrous ourface layer with the underneath pulpal tissue layer containing well-preserved odontoblasts. 2. Bleeding, fibrosis and necrosis are the main reactions obsereved in electrosurgical pulpotomy and the normal pulpal tissues were limited to the apical portion. 3. In the aspect of preserving the normal pulpal tissue, ferric sulfate pulpotomy was evaluated to be superior to formocresol or electrosurgical pulpotomy.
