      • 실험 생쥐에서 BCG 백신과 DHEA 복합 투여의 천식 발생 예방효과

        Background: Both Bacille Calmette-Guerin (BCG) vaccine and dehydroepiandrosterone (DHEA) have a suppressive effect on asthma. Objective: To evaluate the effect of BCG and DHEA combination against the development of asthma. Method: Female BALB/c mice were sensitized and provoked with ovalbumin (OVA). One week before sensitization, BCG (1×105 CFUs) was administered intraperitoneally and the animals were fed with a diet incorporated with 0.01% (w/w) of DHEA for 4 weeks. Methacholine brochoprovocation tests and bronchoalveolar lavages (BAL) were carried out. Result: As compared with control, the combined BCG and DHEA treatment significantly suppressed both maximal airway resistance expressed as enhanced pause (P<0.01) and airway hypersensitivity (35.0 vs. 10.2 mg/mL, P<0.05) to methacholine, and BAL fluid eosinophilia (1.4±0.3 vs.10.7±1.4%, P<0.001); however, the combination was not superior to each treatment. The level of IFN-γ in splenocyte supernatant was significantly higher in the combined group than that in control group (295.1 vs. 218.8 pg/mL, P<0.05). Conclusion: A combined BCG and DHEA treatment suppresses the development of airway hyperresponsiveness and eosinophilic inflammation along with increased Th1 response in OVA-induced asthma mice.         Background: Dehydroepiandrosterone (DHEA) increases the production of interferon (IFN)-γ, and IFN-γ suppresses the Th2 response. Objective: To investigate whether DHEA suppresses the in vitro allergen-induced response of airway muscles. Method: Female BALB/c mice were fed mouse chow containing 0, 0.001, 0.01 or 0.1% DHEA during the study period. The animals were sensitized with ovalbumin (OVA), and then the Schultz-Dale reaction to 50μg/ml OVA, serum OVA-specific IgE concentration and cytokine production by cultured splenocytes were measured. Result: OVA-sensitized mice (n=20) developed a Schultz-Dale reaction, had significantly high concentrations of IgE and IL-5 and a significantly low IFN-γ/IL-5 ratio compared to the controls (n=20). The treatment with 0.1% DHEA significantly decreased the IL-5 level (123 vs. 54 pg/ml, P<0.001) and tended to increase the IFN-γ/IL-5 ratio (2.2±0.3 vs. 3.7±0.7, P=0.09), but did not suppress the Schultz-Dale reaction and IgE levels. The contractile responses of muscles to OVA were significantly related to OVA-specific IgE levels (r=0.446, P<0.001) and the IFN-γ/IL-5 ratio (r=?0.427, P<0.001). Conclusion: DHEA did not suppress the Schultz-Dale reaction or the OVA-specific IgE production despite a significant suppression in the Th2 response in mice.       • 전립선비대증환자에서 경요도전립선절제술 및 경요도전립선절개술의 임상경험

        Purpose: To evaluate the effectiveness and complications of the combination of transurethral resection of the prostate (TURP) and incision of the prostate (TUTP) for benign prostate hyperplasia (BPH). Materials and Methods: Between Jan. 2001 and Jun. 2003, the pathological studies of 155 men with BPH followed by TURP and TUIP were available for a retrospective review. Patients were evaluated in terms of the change in symptom severity, maximal flow rate and sexual function in addition to operating time, length of hospital stay, duration of catheter positioning and the rate of bladder neck contracture using Student`s paired t-test. Results: The preoperative peak flow rate was changed from 8.66±3.96ml/sec, to 14.30±4.98ml/sec, 14.15±4.50ml/sec and 15.76/sec, at postoperative 1st, 3rd and 6th months, respectively. International prostate symptom score (IPSS) was decreased from 22.59±5.77, to 13.14±5.26, 13.50±5.59 and 11.29±5.59, at postoperative 1st, 3rd and 6th months, respectively. Quality of life (QOL) score was decreased from 4.14±1.17, to 2.56±1.20, 2.47±0.99 and 1.85±0.56, at postoperative 1st, 3rd and 6th month, respectively. Improvement in IPSS, maximal flow rate and quality of life score were statistically improved (p<0.05), but erectile dysfunction rates were not statistically different between preoperative and postoperative states (p>0.05). Bladder neck contracture was not found. Conclusions: The combination of transurethral resection and bladder neck incision in patients with BPH appears to provide satisfactory clinical outcome.

        BALB/c 생쥐에서 천식 발생에 대한 dehydroepiandrosterone의 억제효과

        Background : Dehydroepiandrosterone(DHEA). and adrenal androgen, may regulate Th1/Th2 cytokines. Objective : To determine whether DHEA have a preventive effect against the development of asthma, a Th2-associated disease. Materials and methods : Female BALB/c mice were fed with a standard diet incorporated with 0.1% (w/w) of DHEA for 1 week before sensitization with ovalbumin(OVA. 20㎍ i.p. twice with 2-week intervals). Enhanced pause (Penh) was monitored after 1% OVA inhalations for 3 successive days, and then the methacholine bronchoprovocation tests were performed using a whole-body plethysmography system. Analyses of inflammatory cells, cytokines, and OVA-specific IgE were carried out. Results : Both the excessive airway narrowing (maximum Penh) and the airway hypersensitivity (PC200, ㎎/mL) to methacholine were significantly suppressed with the DHEA pretreatment (F<.05 and F<.01, respectively). However, the numbers of the inflammatory cells and the levels of interleukin(IL)-4, IL-5, and IFN-γ in the bronchoalveolar lavage fluid, the production of IL-4, IL-5, and IFN-γ by Con A-stimulated splenocytes, and the serum IgE level in DHEA group were not significantly different from those in asthma group. Conclusion : DHEA pretreatment could suppress the development of asthmatic airway hyperresponsiveness without suppression of Th2 cytokines and airway inflammation. (J.
