        일부 치과위생사의 대학졸업연한 차이에 따른 희망진로와 현 근무처와의 일치도 및 직업 만족도 조사

        Objectives : Five years have passed since the departments of dental hygiene with four-year term released their graduates. It is necessary to investigate concordance between desired career and current job and job satisfaction in dental hygienists according to graduation term. Accordingly, a survey on concordance between desired career and current job and job satisfaction in dental hygienists according to graduation term was conducted to obtain a basis necessary for providing right job consciousness to students of departments of dental hygiene. Methods : Of departments of dental hygiene nationwide, a total of four departments consisting of two departments with 3-year term and two departments with 4-year term were randomly selected. Of 683 graduates for 2006-10 of the selected departments, 163 graduates who agreed to participate in this study through telephone and e-mail were chosen as subjects. The data of 159 graduates were analyzed except for 4 graduates who did not respond to job satisfaction. The questionnaire consisted of a total of 17 questions: 5 general questions such as graduation term, and current job and salary, 2 questions for desired career at the time of entrance and graduation, and 10 questions for the satisfaction of current job and working environments. Concordance between the desired career at the time of entrance and graduation and current job was calculated using Kappa-value. A cross analysis was conducted to investigate job satisfaction according to general features and graduation term. A statistical significance was examined using $X^2$-test (p<0.05). Results : The Kappa-value for concordance between the desired career at the time of graduation and the current job was 0.288, showing slightly low concordance. In particular, the Kappa-value was 0.089 in the graduates of departments of dental hygiene with 3-year term, which showed significantly low concordance. The job satisfaction of the graduates was 49.1%, which was higher in the graduates with 4-year term than in the graduates with 3-year term, but was statistically insignificant (p>0.05). The detailed job satisfaction was all higher in the graduates with 4-year term than in the graduates with 3-year term except for distance to a working place, but a statistical significance was only found in working place recognition and regional status (p<0.05 ). Conclusions : This study showed differences in concordance between desired career and current job and job satisfaction between the graduates of departments of dental hygiene with 3-year term and the graduates of departments of dental hygiene with 4-year term. Therefore, the curriculum of department of dental hygiene is required to be more specifically applied in accordance with graduation term. In addition, a further study is required to develop specialized curriculums in accordance with graduation term.

        Awareness and Necessity of Registered Dentist Program for Children and Adolescents

        Background: The program aims to enable the participants to receive oral health care with sustainability and give them the choice and prior autonomy of dental practitioners while enhancing the benefits of sustainability. The purpose of this study is to investigate the necessity, awareness, and satisfaction of the implementation of the registered dentist program for children and adolescents, and to promote the introduction of RDPCA. Methods: The survey targeted adult residents in their 20s or older who live in Seoul, an area that continuously implements the RDPCA system, especially in the cities of Cheonan and Asan Results: 79.7% were the most unaware. However, RDPCA participants were significantly more aware than non-participants. When surveying the desire to participate in RDPCA, 96.6% of the respondents said they would 'participate' in the non-RDPCA participate group. The analysis of the satisfaction level of the RDPCA received by their children indicated that 48.6% expressed their satisfaction, and 47.3% said they were healthy. There was a high percentage of people who said they would recommend RDPCA to others as well as a high rate of those who said they would continue to use RDPCA. Conclusions: The reality is that the awareness and implementation of RDPCA are low. For the development of oral health care programs for all children and adolescents in Korea, and the improvement of the satisfaction level of the dental care system, there should be an active consideration of high-quality programs and.

        PBL기반의 치과의료커뮤니케이션 교육과정을 통한 일부 치위생학과 학생들의 치위생 의사소통 역량수준 평가에 대한 사례연구

        Background: The purpose of the present study was to evaluate changes in the communication capabilities of dental hygiene students after completing a problem based learning (PBL) dental communication curriculum. With this information, we intend to gather the basic data necessary to stress the need for and value of an education in communication regarding dental hygiene. Methods: PBL-based education was provided to a total of 49 third-year dental hygiene students who took the dental communication class taught by the Department of Dental Hygiene at S University during the 2<sup>nd</sup> semester of 2020. The relevant self-evaluation was modified, based on the aim of the present study, to refer to three basic key competencies related to the communication capabilities of dental hygienists. An assessment of dental communication competency was conducted by analyzing the changes in self-evaluations before and after completing the course, for each question, using a paired t-test. The statistical significance level was set at p < 0.05. Results: Analysis of core competencies before and after PBL-based dental communication education indicated that all competencies were significantly improved after education (p < 0.01). Conclusion: Dental hygiene students' dental communication skills increased significantly after completing PBL-based dental communication education. Therefore, a PBL-based dental communication curriculum is effective in improving dental communication skills for dental hygiene students.

        일부 치위생학과 학생의 팀 기반 학습의 임상치위생학 학습자 성찰일지 질적 분석

        Objectives: The purpose of this study was to qualitatively analyze the reflective journals to evaluate the effectiveness of team-based learning (TBL) method in clinical hygiene subjects. Methods: This study was conducted by analyzing the reflective journal according to the qualitative research methodology based on ground theory. Results: The students’ overall comprehension was 4.46±0.78, which was a very high level of understanding. Students’ Reflective Journals promoted the effectiveness of TBL activities and improved learners desire, learning motivation, comprehension, and responsibility. Conclusions: Through qualitative analysis of the contents of the reflection journal in team-based activities, a paradigm could be derived and the educational implications of writing a reflection journal could be confirmed.         치위생학과 교육과정에서의 치과의료커뮤니케이션 교과목 운영 사례연구

        이번 연구는 치위생학과 교육과정에서 이론과 실습으로 구성된 효과적인 치과의료커뮤니케이션의 교육과정을 개발하기 위한 목적으로 운영되었으며, 다음과 같은 결론을 얻었다. ``관계 형성하기``, ``대화 열기``, ``정보 수집하기``, ``상대방의 시각 이해하기``, ``정보 공유하기``, ``의견일치 보기``, ``마무리짓기`` 등 7가지 평가항목에서 교육 전보다 교육 후 모든 점수가 상승되었다. 이 중 ``정보 수집하기``의 자가평가수준은 수업 후 통계적으로 유의한 변화가 있었다(p=0.008). 치과의료커뮤니케이션 수업에 참여한 학생들은 교과목에 대한 중요성과 필요성에 대해 수업운영 전보다 후에 높게 인식하는 것으로 조사되었는데, ``치과의료커뮤니케이션 수업은 치과위생사의 전문성을 키우는 데 중요하다고 생각하나요``, ``치과의료커뮤니케이션 수업이 교육과정에서 필요하다고 생각하나요``라는 항목에서는 수업 전후 각각 통계적 유의한 차이를 보였다(p=0.019, p=0.016). 이러한 결과로 볼 때 치과위생사에게 필요한 핵심역량인 치과의료커뮤니케이션 역량을 향상시킬 수 있는 교육과정은 치위생(학)교육과정으로 반드시 포함되어야 한다. 치과의료커뮤니케이션 역량은 교육과 학습을 통해 계발 및 향상될 수 있다. 따라서 치과의료현장에서 필요한 치과의료커뮤니케이션 요소들을 보다 구체적으로 정리하고, 이론 교육만이 아닌 실습 교육을 포함한 체계적이고, 학년 간 연계 가능한 교육과정의 개발이 시급한 것으로 판단된다. 또한 연구 대상자 수를 늘리고, 자가수준의 평가가 아닌 학생들의 커뮤니케이션 역량을 객관적으로 측정할 수 있는 평가도구 개발과 표준화된 치과의료커뮤니케이션 교육과정이 필요할 것으로 본다. The purpose of this study was to develop a dental communication course in dental hygiene schools that included theory, practice, and skill for effective dental communication. Thirty-six senior dental hygiene students in a dental hygiene school took a dental communication course and responded to a questionnaire. The instrument used in the study was a modified form of Kim’s communication skill self-assessment sheet. The self-assessment questionnaire about communication competencies was administered before and after the class, and the difference between scores at each time point was analyzed using the Wilcoxon signed rank sum test. Among seven elements of dental communication competencies, the competency of “information gathering” was significantly improved (p=0.008). Students’ scores on perception of the importance (p=0.019) and necessity (0.016) of a communication course significantly increased after the course. Competencies in communication are essential requirement for dental hygienists. This study showed the possibility of communication skill training. An objective evaluation tool regarding students’ communication competencies should be developed. Further studies with larger samples size are needed to develop a standardized comprehensive communication course in dental hygiene schools.

        일개 대학 치위생학과의 치과의료커뮤니케이션 교육 과정 타당성에 관한 사례연구

        이에 본 연구에서는 양적 연구의 한계를 보완하여 정규치위생(학)과 교육과정에서 운영 중인 치과의료커뮤니케이션 교육내용에 대한 질적 평가를 통해 타당성을 검토하고자 하였다. 치과의료커뮤니케이션 교과목은 인문사회학적 소양을 기반으로 한 전문가적 태도로 진료실에서의 환자와의 커뮤니케이션 역량 향상을 목적으로 하여, 치과위생사로서의 전문가적인 수행 및 태도를 함양하기 위해 자기이해를 통해 전문가적 태도를 함양하여 환자들과 소통할 수 있는 방법에 대한 교육내용으로 구성하였다. 교육내용의 타당도는 전문가 3인에게 기재된 타당도 평가지를 제공하여 검토를 의뢰하였다. 주차별 교육내용 및 교육시기에 대한 타당도는 최소 4점, 최대 5점으로 전반적으로 타당하다는 평가와 인증평가 기준에도 합당하다는 평가를 받았다. 그러나 해당학년 수준에 맞는 학습내용으로 구성하되, 1개 학기에 그치는 교육과정이 아니라 단계적, 연계성을 갖고 이론과 실습 학습에 노출될 수 있는 부분이 보완되어야 한다고 분석하였다. 따라서 본 결과는 향후 치위생(학)과 커뮤니케이션 역량 달성을 위한 학습목표 및 교육내용을 개발하고, 향후 치과의료커뮤니케이션 교육과정 표준화를 위한 기초자료로 활용될 것으로 생각된다. The purpose of this study was to examine the validity of the content of an educational curriculum by understanding the opinions of experts in the development of weekly educational content for dental communication or communication-related subjects. It was composed of educational content on how to communicate with patients by cultivating a professional attitude through basic contents and a self-understanding of communication. Three experts were asked to evaluate the feasibility of educational contents and their appropriateness in order to evaluate the criteria for certification as a dental hygienist. The validity of the weekly educational contents and the period of education were awarded 4 points out of a maximum of 5 points; overall, the curriculum was evaluated to be valid and to be acceptable for use as criteria for certification evaluation. On the other hand, it was confirmed that not only should the contents be made appropriate for the grade level, but it should be complemented so that theoretical and practical learning can be achieved in a gradual and interrelated manner rather than as a one-time curriculum. Therefore, the result of this study can be considered for use as background data for curriculum development, and for standardization of the communication course in the dental hygiene department.

        회전법과 바스법의 잇솔질 수행요소별 적절성과 난이도인식 및 치면세균막지수의 연관성

        Objective: The objective of this study was to determine the correlation among adequacy of toothbrushing factors, awareness of difficulty in toothbrushing and the plaque score for improving the method of toothbrushing instructions. Method: One hundred twenty-seven subjects were divided into rolling and Bass` method group. Subjects were advised to brush their teeth everyday under a guided instruction instructed 3 times during 4 weeks of research. The plaque score was measured by Turesky`s index. The evaluation criteria the adequacy of toothbrushing factors were categorized as toothbrush grasps, stroke action, stroke frequency, and dividing dental arch. Awareness of the subjective degree of difficulty in toothbrushing was measured by a questionnaire with a 5-point Likert scale. (very easy to very difficult). Results: There was no significant relationship between the awareness of the subjective degree of difficulty in toothbrushing and the plaque score. Also, there was no significant relationship between the awareness of toothbrushing difficulty and the adequacy of toothbrushing factors. Repeated instructions were more important than awareness of difficulty or adequacy of toothbrushing factors on reducing the plaque score. The priority setting of toothbrushing factors with a given opportunity and circumstance of instruction is needed to enhance the effect of TBI. Conclusion: It is strongly believed that more detailed research on the role of toothbrushing education and on which educational factors are of importance regarding dental plaque care are taken into further consideration.

        2010~2011년 국민건강영양조사를 활용한 한국 노인의 치아보유상태에 따른 사회경제적 수준 분석

        The insurance payment plan for dental implants in Korea has been criticized because the payment priority has not been properly established, and the benefits are concentrated among middle-class citizens. In the current study, the relationship between the oral health condition and socioeconomic status of the elderly was analyzed using data from the Korea National Health and Nutrition Examination Survey (KNHANES). This study aims to determine the reason underlying the criticism of the insurance payment plan. The subjects were >65-year-old individuals in the 2010 and 2011 database of KNHANES. Data from 2,812 subjects were analyzed. The socioeconomic status was determined based on edentulousness, molar tooth loss, and presence of 28 teeth. According to the analysis, the average income was 1,560,000 won for edentulous elderly, 1,811,000 won for elderly who had lost molar teeth, and 1,896,000 won for elderly with 28 teeth (p<0.05). In addition, elderly with a low education level demonstrated a poor oral health condition (p<0.001). In conclusion, the insurance plan currently under examination is not properly designed for economically impoverished elderly because the plan only covers 50% of the costs and is limited to implantation of molar teeth only. This plan will not provide practical benefits to elderly with a poor socioeconomic status; therefore, the insurance payment plan needs to be improved so that the appropriate beneficiaries can be targeted.

        미국 폰즈스쿨의 임상치위생학 교육과정 분석을 통한 국내 임상치위생학 교육과정에 관한 고찰

        본 연구에서는 국제적인 치위생계의 임상실무와 교육의 표준인 치위생 관리 과정을 기반으로 표준화된 치위생 교육기관인 미국 폰즈스쿨의 임상치위생학 교육과정에 대해 심층분석 함으로써 국내 실정에 맞는 임상치위생학 교육과정의 표준화 방안에 대해 고찰해 보고자 하였다. 2015∼2016년 폰즈스쿨의 임상치위생학은 모두 `Dental Hygiene Clinical Practice (DHYG)`라는 과목명으로 운영되었고, 실습의 비중이 매우 컸으며, 실습강의는 교수 1명당 학생 5명이 한 팀으로 운영되었다. 폰즈스쿨의 학과 목표는 브릿지포트 대학교의 미션에 따라 설정되었고, 폰즈스쿨의 임상치위생학 교육목표 또한 폰즈스쿨의 학과 목표에 기반하여 설정되었다. 또한, 임상치위생학의 교육목표는 ADHA에서 제시한 치위생임상 실무표준에 따라 개발되었고, 이를 기반으로 임상치위생학 교육을 통해 달성해야 하는 임상 핵심역량과 세부역량을 제시하고 있었다. 임상치위생학 교육내용은 다양한 대상자에게 적절한 치위생 관리 과정을 제공하기 위한 이론 및 실습으로 구성되었고, 특히, 졸업 후 실제 임상 현장에서 수행하게 될 업무에 대한 실습내용이 다뤄졌다. 학생은 대상자/환자별로 수행한 술식에 문서를 작성하여 교수자와 함께 검토 및 보완작업을 통해 치위생 임상역량을 높이고자 하였다. 실습내용 중 치위생 관리 과정 실습은 필수적으로 어린이, 청소년, 성인, 노인, 특별환자를 대상으로 선정해야하며, 구강 상태에 따라 중등도 이상의 치주환자를 포함해야 했다. 이론평가는 지필고사 혹은 사례연구 발표 등으로 이루어졌고, 실습평가는 주로 임상역량의 달성 정도에 따라 평가되었다. 특히 교수자의 로테이션 평가를 실시하였는데, 이를 통해 모든 학생의 임상역량 달성 정도를 파악하고 부족한 역량에 대해 개선방안을 함께 모색하는 과정을 수행하였다. 본 연구는 ADHA에서 제시하는 치위생임상 실무표준에 따라 운영되고 있는 표준화된 임상치위생학 교육과정을 심층적으로 분석함으로써 향후 우리나라 임상치위생학의 개선방향을 모색하기 위한 기초 자료로 제시될 수 있을것이라 판단되었다. The aim of this study was to analyze in depth the standardized Clinical dental hygiene curriculum of the Fones School in the United States. We investigated the clinical dental hygiene curriculum in 2015∼2016 including title, credit, hours, contents, goals, competencies, and evaluation. We obtained the course syllabus and data related to each subject, for each grade, from the professors and students at the university. The goals and competencies, of the clinical dental hygiene program, which were based on the goals of the Fones School and the mission of the University of Bridgeport, were developed in accordance with the dental hygienist practice standards proposed by the American Dental Hygienists Association. The curriculum consisted of theory to teach proper dental hygiene care procedures and incorporated practical exercises that modeled an actual clinical setting. The students had to document the procedures performed for each client/patient and improve their clinical competency through discussion with the professors. Dental hygiene care should be provided for children, adolescents, adults, elderly, and patients, which includes patients with moderate or severe periodontal status. Students were evaluated by a paper test or case study presentation and their clinical evaluation was based on their clinical competency. In particular, professors evaluated students on a rotational basis, so they could evaluate the level of achievement of clinical competency of all students and find ways to improve any weaknesses. Therefore, the current study suggested that clinical dental hygiene program in Korea could be improved if based on the curriculum of Fones School in the United States.
