A decision support system (DSS) was developed to support doctor's decision-making in diagnosing hearing loss. The final diagnosis encompassed 41 diseases with the problem of hearing loss. The system was developed by integrating model-oriented DSS technique and artificial intelligence technology. The system can be used as both diagnosis tool and teaching tool for medical students. Furthermore, the AI technology obtained from this study may also be used in developing DSS for hospital management.

Objectives: The purposes of this study were to develop the HL7-based Classification System for the Long Term Care Insurance for the elderly in order to reduce classification errors and to reduce variations in classification among rating doctors. Specifically, the classification system was developed for the elderly who were eligible for the Long Term Care Insurance based on HL7 standards and eligibility criteria developed by the National Health Insurance Cooperative (NHIC). Methods: 38 cases that were actually reviewed by rating committee in May 2009 were selected from the classification data submitted to the long term care insurance unit at the D branch of NHIC on HL7. The system was designed by selecting 46 items from health insurance and NHIC criteria and IPI(Insurance Patient Information) message based on HL7 2.5 in order to transmit messages to other institutes. The system was analyzed and designed by DFD(Data Flow Diagram) and HIPO(Hierarchical Input, Process, Output). The system was evaluated through frequency analysis of system environment and rating data after calculating the 2nd grade by operating each module of HL7 message. Results: The results of this research were as follows: 38 cases submitted by doctors were 22 right cases and 16 wrong cases for the physical condition area; 27 right cases and 11 wrong cases in the area of mental state; and 26 right cases and 12 wrong cases in the area of independence of daily life. The number of cases whose grade was changed was 4: 1 case from 4th grade to 3rd grade, 2 cases from 3rd grade to 2nd grade and 1 case from 2nd grade to 1st grade. The cause of functional disorder for changing grade was 3 dementia and 1 bleeding. Conclusions: The model of the HL7-based Classification System for the Long Term Care Insurance for the elderly was designed to reduce classification errors and to reduce variations in classification among rating doctors. After the use of the model system, its usability was assessed.
CDSS 실용화를 위한 ArdenML XSLT 개발 -VB.NET을 활용한-

Objectives: The ArdenML is an HL7 standard language for an XML-based representing medical knowledge as logic statements. Problems related to execution of ArdenML are the curly braces problem and the compiler problem, which may be resolved by the introduction of XML-based techniques. This study was aimed at develop an ArdenML based XSLT conforming to HL7 standard. Methods: There were several structural differences in three approaches: Arden Syntax is category; ArdenML is tag; and Visual Basic is comment, function, events, class, and procedure. XSLT was developed by analyzing three approaches. Based on XSLT, ArdenML was translated into Visual Basic and the interface module was developed to allow communicating with legacy systems. Results: Most of the CDSS are independently developed and run, to require reentry of rules. But this system makes it easier to interface with the existing systems simply by translating or compiling rules. Two hundred sixty five of the 285 rules were showed the success after compiling, and 6 errors for the operater query, 14 warnings related to data type were resolved by data re-modeling. Conclusions: ArdenML has been converted into executable files using standard transforms written in the eXtensible Stylesheet Language Transformation (XSLT) in visual basic language. Development of Arden Syntax compiler requires not only a lot of time and efforts, but also reorganization of existing systems according to the MLM. It also has curly brace problem and problem of requiring additional compiler.
Comparison of Alternative Knowledge Acquisition Methods for Allergic Rhinitis

This paper compared four knowledge acquisition methods (namely, neural network, case-based reasoning, discriminant analysis, and covariance structure modeling) for allergic rhinitis. The data were collected from 444 patients with suspected allergic rhinitis who visited the Otorlaryngology Deduring 1991-1993. Among four knowledge acquisition methods, the discriminant model had the best overall diagnostic capability (78%) and the neural network had slightly lower rate (76%). This may be explained by the fact that neural network is essentially non-linear discriminant model. The discriminant model was also most accurate in predicting allergic rhinitis (88%). On the other hand, the CSM had the lowest overall accuracy rate (44%) perhaps due to smaller input data set. However, it was most accurate in predicting non-allergic rhinitis (82%).

Since the national insurance system has implemented in 1977. the aspect of supply and demand in terms of treatment has been solved to a certain degree. but the health of the people has not been satisfied. Furthermore. considering the efficiency of cost-effective analyais. the development in the health promotion of medical field has gradually come to be called for. As a part of the research, we have established the the major adult disease of our country. such as hypertension, diabetes, gastric cancer and cervical cancer, elicited the items of knowledge. attitude and practice( KAP) aimed at the prevention and early diagnosis of the above disease, prepared self-questionires. and made the persons concerned evaluate the KAP for their health. We think that the utilization of the KAP questionires is effective for the promotion of health by the self-efficacy consciousness. and that the KAP itself is greatly significant as an excellent materials for public health education. The sustained research and study which is to be followed is to go over the validity, reliability, efficacy. which would, make a great contribution to public health in the community.
