        유료노인복지주택 거주 노인에 대한 간호중재 분석

        Purpose: This study was done to examine nurses' work and roles by analyzing the nursing practice of nurses caring for elders at home under the paid welfare system. Method: The participants were 352 elders receiving nursing care at home under the paid welfare system between Jan. 2006 and Oct. 2008. Frequencies and percentages with MS Office Excel were used to analyze the data. In addition, correlation between the number of illnesses for each patient and nursing interventions according to the age of the elder were analyzed using the SPSS program. Results: The mean age was 78.6(±5.95) years, 67.7% had high school graduation or higher education, the average number of chronic illnesses was 2.36(±1.35), but over 92.6% had at least one chronic illnesses. Based on the 4 categories of the OMAHA intervention scheme, surveillance was the category with the highest frequency at 52.9%. The category of treatments and procedures was the lowest at 9.3%. Conclusion: The results indicate that it is more important to manage chronic illnesses rather than acute illnesses for elderly people at home. So, home care nurses ought to have professional education and ability to assess health problems and environment problems correctly in order to make good use of community resources.

      • 레이저 포화흡수체 Nd:LSB에 관한 연구

        본 논문에서는 다른 Q-스위칭용 포화흡수체보다 넓은 천이 영역을 가지고 있는 포화흡수체 Nd:LSB가 Q-스위칭 소자로서 가능한지 보였으며, 최적화된 출력 거울의 반사율, 최대 출력, 출력 에너지, 펄스폭, 출력 효율을 수치적으로 분석하였다. 그 결과 Nd:LSB를 이용한 1319nm 파장의 수동형 Q-스위칭 Nd:YAG 레이저의 특성을 예측할 수 있었고, 공진기 거울의 반사율 98%, 공진기 길이 30㎝로 하였을 경우 출력 31.3MW, 펄스폭 6.22ps를 얻을 수 있다. In this study, it's shown that the saturable absorber Nd:LSB which has broad absorption bandwidth than other saturable absorber as Q-switching system has a capability as a saturable absorber in Q-switching system. In this system, we theoretically analyzed the optimized output mirror, peak power, output power, pulse width and output efficiency. As the result, the passively Q-switched 1319nm Nd:YAG laser using Nd:LSB as a saturable absorber gives the optimized output mirror reflectivity 98% and peak power of 31.3MW and pulse width of 6.22ps at 30㎝ resonator length theoretically.

        정상 성인의 좌심실 벽두께와 내강 크기에 대한 심초음파영상과 전산화단층영상의 비교

        The purpose of this study was evaluated the relationship between echocardiography and cardiac computed tomography in normal adults by visual assessment, and evaluated the effective doses of cardiac computed tomography. The subjects were 100 normal patients who visited two general hospitals in Chungnam, from January 1 to May 30, 2018. We obtained images by using these modalities and evaluated the wall thickness and internal dimension of the Left Ventricle with visual assessment. To evaluate the appropriateness of the visual evaluation, two evaluators who were trained for one week were measured and the agreement between the evaluators were verified by statistical analysis. The effective doses of computed tomography were evaluated using the dose length product. As a result, there was a high correlation between the two modalities. The agreement between the two visual evaluator were also highly agreed. The mean X-ray dose of the subjects was 11.1±3.1 mSv. Although the purpose of imaging is somewhat different in the clinical setting, echocardiography could achieve the same results as radiation-invasive computed tomography. It is thought that utilization will become even larger.

        ND:YVO4 마이크로칩 레이저의 단일 종모드 동작

        - We developed LD pumped Nd:YVO4 microchip laser with the cavity length of 1㎜. The microchip laser output was 85.5㎽ at the wavelength of 1063.9㎚ with the input power of 241㎽ at the wavelength of 809㎚. The slope efficiency was 40.7% and the threshold input power was 31.1㎽. We have also defined input power limit for the single longitudinal mode operation theoretically. It was 2.5 times larger than that of threshold input intensity. According to the results of simulation, the Nd:YVO4 microchip laser can be operated with the maximum output of 15㎽ for the single longitudinal mode up to the input power of 77.75㎽.

        말초 동맥 분광 도플러 파형 해석 및 명명법에 대한 고찰

        In 1959, Satomura used spectral Doppler ultrasound to express the velocity of red blood cells according to time change, and Kato defined a zero-base line that could tell the direction of blood flow, making it possible to know the direction of blood flow. This became the basis for the widely used classifications of Triphasic, Biphasic, and Monophasic. However, the above classification has limitations that confuse users with the meaning and timing of use in a clinical environment. As a result, the American Society for Vascular Medicine (SVM) and the Society for Vascular Ultrasound (SVU) A consensus document on Doppler waveform analysis was declared by the joint committee. This study tried to review this consensus and to suggest nomenclature and modifiers that can be used in the domestic vascular ultrasound clinical field. The joint committee formed by SVM and SVU recommended that the use of the triphasic waveform and the biphasic waveform be used as a multiphasic waveform rather than being used due to the ambiguity of interpretation. In addition, it was agreed to name the hybrid-type waveform, which is a monophasic and high-resistance waveform, which has always been a problem of interpretation in a clinical environment, as an intermediate resistive waveform. In addition, in order to increase the communication efficiency between the interpreter and the sonographer, waveform analysis was classified into a main descriptor and a modifier, and it was recommended to use a single nomenclature by unifying various synonyms. It is expected that this literature review will provide accurate arterial spectral Doppler waveform interpretation and an agreed-upon nomenclature to radiologists performing vascular ultrasound examination in clinical practice, and will be utilized as basic data that can contribute to the improvement of public health. 1959년도 Satomura가 분광 도플러 초음파를 이용하여 시간변화에 따른 적혈구의 속도를 표현하였고 Kato가 혈류의 방향을 알 수 있는 제로베이스 선(Zerobase line)을 정의하면서 혈류의 방향을 알 수 있게 되었다. 이는 현재 폭넓게 사용하고 있는 삼상파 (Triphasic), 이상파(Biphasic), 단상파(Monophasic) 분류의 기초가 되었다. 하지만 위와 같은 분류는 임상 환경에서 사용 의미와 시점에 사용자들에게 혼란을 주는 한계가 있었고 이와 같은 결과로 미국의 혈관의학회(Society for Vascular Medicine, SVM)와 혈관초음파학회(Society for Vascular Ultrasound, SVU)가 구성한 공동 위원회에서 도플러 파형 해석 합의문(Consensus document)을 선언하였다. 본 연구에서는 이 합의문을 고찰하고 국내 혈관 초음파 임상 현장에서 사용될 수 있는 명명법과 수식어를 제언하고자 하였다. SVM과 SVU가 구성한 공동 위원회에서는 동맥 삼상파(Triphasic waveform)와 이상파(Biphasic waveform)의 해석의 모호함을 이유로 사용을 지양하고 다상파(Multiphasic waveform)로 사용하길 권고 하였다. 또한 임상 환경에서 항상 해석의 문제가 되었던 단상파이면서 고저항성 파형인 하이브리드 형태 파형을 중저항성 파형(Intermediate resistive)으로 명명하기로 합의하였다. 또한 판독의사와 초음파사간에 의사소통의 효율성을 높이기 위해 파형 해석을 주 설명어(Main descriptor)와 수식어(Modifier)로 분류하였고 다양하게 사용하던 동의어들을 통일하여 하나의 명명법으로 사용하도록 권고하였다. 본 문헌 고찰을 통해 임상에서 혈관 초음파 검사 직무를 수행하는 방사선사들에게 정확한 동맥 분광 도플러 파형 해석과 합의된 명명법을 제공하여 국민보건향상에 이바지할 수 있는 기초자료로 활용되기를 기대한다.
