        Purpose: Understanding of early postoperative course is very important for planning of operation and postoperative management. The aim of this study was to identify the incidence of immediate postoperative complications associated with colorectal cancer surgery. Methods: This retrospective study examined clinicopathological data on 128 patients who underwent a resection for colorectal cancer between July 2006 and June 2007. Patients who underwent a palliative stoma, bypass, or emergent operation were excluded. Early postoperative complications were defined as complications occurring within 30 days of surgery. The male-to-female ratio was 67 : 61, and the mean age was 66.1 (range, 28~81) years. Results: At least one postoperative complication occurred in 22 patients(17.2%). The most common complication was wound complications (9.4%), followed by ileus (4.7%), voiding disturbance (3.9%), anastomotic leakage (3.1%), intraluminal bleeding (1.6%), and intraabdominal abscess (0.7%). The complication rate was 16.7 in patients with right colon cancer, 20% in those with left colon cancer, and 19.7% in those with sigmoid and rectal cancer. The wound complications occurred on mean postoperative day 5 and were resolved after a mean of 10 days. Ileus occurred on mean postoperative day 6 and required a mean of 10 days for resolution. Of the 4 anastomotic leakage patients, proximal stoma construction was required in 4 patients. Higher frequency of postoperative complications occurred in male patients (P=0.007), patients older than 70 years (P=0.017), and patients with co-morbid medical conditions (P=0.012). Conclusions: The overall early postoperative complication rate following colorectal cancer surgery was 17.2%. The postoperative complication rate was higher in male, elderly, and patients with co-morbid medical conditions. We suggest to identify early postopeative complications and to better understand the postoperative course in patients undergoing colorectal cancer resections.

      • 근치적 대장암 수술 후 CEA 치의 의의

        Purpose: Serum level of carcinoembryonic antigen(CEA) is clinically important prognostic factor in follow-up evaluation of the colorectal cancer patients. Also CEA is widely used for staging, recurrence, and assessing the response to therapy in colorectal cancer. Subjects and Methods: From January 2003 to December 2004, preoperative and postoperative levels of serum CEA levels checked in 142 patients who underwent curative resection for colorectal cancer at Pusan National University Hospital, Department of Surgery. Results: The positive value is higher than 5.0 ng/ml of serum CEA levels. The results are as follows 1) The positive rate of preoperative CEA level was 33.8%, so preoperative CEA level measurement was not useful as screening test for colorectal cancer. 2) There was no significant association between elevated CEA level and the location of tumor. 3) There was significant correlation between Duke’s stage of colorectal cancers. 4) There was significant correlation between increased serum CEA level and lymph node metastases. 5) The recurrence rate was 29.3% and 70.7% in patients with preoperative level of CEA < 6 ng/ml and ≥ 6 ng/ml, respectively. 6) The levels of CEA increased in 35 cases(85.4%) in 41 recurred patients. Conclusions : It is suggested that measurement of preoperative and serial postoperative CEA is useful in predicting the prognosis and detecting recurrences in colorectal cancer.

        NADPH oxidase inhibitor diphenyleneiodonium induces p53 expression and cell cycle arrest in several cancer cell lines

        Diphenyleneiodonium (DPI)는 NADPH oxidase 같은 flavoenzymes의 저해제로써 널리 사용되고 있다. 본 연구에서는 인간 대장암 세포주 HCT-116 (wild-type p53)와 HT-29 (p53 mutant) 및 인간 유방암 세포주인 MCF-7(wild-type p53)의 세포성장 과정에서의 DPI의 효과를 살펴보았다. DPI는 농도 및 시간 의존적으로 암세포주의성장을 막았으며 G2/M phase에서 cell cycle arrest를 일으켰다. Cell cycle arrest의 가장 높은 값은 DPI 처리후 12 시간에서 관찰할 수 있었다. 한편 DPI는 아폽토시스 그리고 cell cycle arres 에 관여하는 유전자 발현에 관여하는 p53의 표현을 크게 증가시켰으며, 이는 DPI처리 후 6시간 후 부터 관찰할 수 있었다. 그러나 NADPH oxidase의 조합을 억제하는 catechol 계인 apocynin은 p53의 발현을 유도하지 못하였다. 이것은 DPI에 의해 유도되는 p53의 발현증가는 NADPH oxidase활성의 저해와 관련되어 있지 않다는 것을 의미한다. 결론적으로 DPI는 HCT-116, HCT-15 및 MCF-7 암세포주에서 ROS에 비 의존적으로 wild-type p53 발현의 증가를 유도하며, 이 증가된 p53은 DPI에 의해 유도되는 성장 억제 및 C2/M phase에서의 cell cycle arrset과정의 조절기전에 관여한다는 것을 시사한다. The Diphenyleneiodonium (DPI) is widely used as an inhibitor of flavoenzymes, particularly NADPH oxidase. In this study, we investigated the effect of DPI on the cell growth progression of human colon cancer cells HCT-116 (wild-type p53), HT-29 (p53 mutant) and human breast cancer cells MCF-7 (wild-type p53). DPI treatment in cancer cells evoked a dose- and time-dependent growth inhibition, and also induced the cell cycle arrest in C2/M phase. The peak of cell population arrested in C2/M phase was observed at12 hr after treatment of DPI. In addition, DPI significantly induced the expression of p53, which induces proapoptotic genes in response to DNA damage or irreparable cell cycle arrest, at 6 hr in DPI-stimulated cells. However, a catechol apocynin, which inhibits the assembly of NADPH oxidase, did not induce p53 expression. This suggest that p53 expression induced by DPI is not associated with the inhibition of NADPH oxidase. In conclusion, we suggest that DPI induces the expression of wild-type p53 by ROS-in-dependent mechanism in several cancer cells, and upregulated p53 may be involved in regulatory mechanisms for growth inhibition and cell cycle arrest at C2/M phase in DPI-stimulated cells.

        제로에너지 건축물 인증 의무화에 따른 공동주택 단위 냉난방부하 기준에 관한 연구

        In this study, theoretical reference building model for apartment house was established, and heating and cooling loads per unit were calculated in accordance with the current standard for energy conservation design in buildings and zero-energy building certification guidelines. Subsequently, an analysis was conducted, categorizing the results by region for 17 different cities, by floor area for 3 unit types, and by unit layout for 9 unit types. The analysis also considered heating and cooling load components based on both the current standard for energy conservation design in buildings and zero-energy building certification guidelines. The significant conclusions derived from this analysis can be summarized as follows. According to the current standard for energy conservation design in buildings, it is possible to achieve a reduction of 55.1% to 62.0% in cooling load compared to KS C 9306. For heating, a load reduction of 31.7% to 37.0% is achievable compared to the heat utilization facilities standard of the Korea District Heating Corporation. Application of zero-energy building certification guidelines, in comparison to the current standard for energy conservation design in buildings, results in a cooling load reduction of 8.6% to 12.3% and a heating load reduction of 14.0% to 15.2%. These findings indicate that the insulation performance of current standard for energy conservation design in buildings is approaching the level required for zero-energy buildings. In the future, in order to reduce the energy demand of apartment house in compliance with mandatory zero-energy building certification, it can be concluded that a reduction in heating and cooling load through the mitigation of factors other than insulation will be necessary.

        소유역의 홍수도달시간과 서면길이의 분포특성을 고려한 홍수유출해석

        침투효과과 큰 산지 소유역은 사면류가 유역내의 주된 흐름이 되며, 강우일유출의 수문응답은 사면길이의 분포특성으로 나타낼 수 있다. 유역의 지형특성은 홍수도달시간으로 표현되며, 홍수도달시간에 확률밀도함수를 적용시키면, 강우, 유출의 응답함수인 순간단위도를 구성할 수 있다. 서면길이와 관측된 홍수도달시간의 확률분포를 비교해 본 결과 형상계수가 거의 일치하였다. 이는 서면의 특성이 도달시간으로 집약되어 나타나는 것을 의미하며, 서면의 특성치로서 유출에 대한 응답함수를 구성할 수 있음을 나타낸다. 특히, 유역내 유출의 평균화를 시도하기 위해서 소류역내 분수계르르 기준으로 다수의 분할 소구역으로 나누어 전체소유역과 분할소구역의 서면특성치를 비교 분석하였다. 응답함수에 의해 유도되는 직접유출수문곡선을 유효강우량의 산정 방법에 따라 그 재현정도가 달라진다. 본 논문에서는 Horton의 침투 eq.을 이용 기본 방정식을 재구성하였고, $\Phi$-index 법과 직접유출수문곡선의 재현성을 비교 검토하였다. The hydrologic response function in a small basin is expressed by the distribution function of slope length. The characteristics of topographical factors is represented to the concentration time, and the instantaneous unit hydrograph is derived as a hydrologic rsponse function by application of probobility density function. The averaging process of runoff characteristics within watershed was analyzed for a few small watershed where was split up the small basin itself. The method of calculation of the effective rainfall should play important roles in the transformation process from hydrologic response function to runoff hydrograph. In this paper, the Horton's infiltration quation is used as a method of calculation of effective rainfall, a new response function of runoff process is derived. The $\Phi$-index method and the infiltration method are tested by comparing the observed and estimated values.

      • 하천계획관리시스템을 이용한 하천정비방법에 관한 연구

        본 연구는 20세기를 기점으로 시작된 치수, 이수 개념의 하천정비기법에서 20세기 이후 경제성장에 따른 생활의 질적 향상과 더불어 추진되어 온 하천의 수질보전 및 하천환경을 중요시하는 환경친화적 하천계획과 효과적인 하천보전 및 관리기법개발에 역점을 두었다. 이를 위해, 하천개수계획, 실시설계 및 관리 등에 필요한 보든 자료를 도면 및 대장 등의 서류로 관리하고 있는 현재의 하천관리방법을 개선하기 위해 공간상 위치를 점유하는 지리 공간요소 (Geographical and spatial element)와 이에 관련된 속성정보(Attribute information)의 통합처리가 가능한 지리정보시스템 (Geographic Information System)을 이용한 하천계획관리시스템을 연구하였다. 구축된 하천관리시스템을 울산광역시 관내하천에 적용하여 자연형 하천정비계획방법과 홍수관리 등 기존의 치.이수대책의 개선방법에 대한 가능성을 검토하였다. 그 결과, 하천관리시스템을 실무에 반영하여 계획과 정보에 대한 관리를 함으로써 기능성과 효율성을 극대화할 수 있을 것으로 판단되었다. This research is focused on the environmental protection plan, efficient conservation of creeks and the development of managerial techniques, which are emphasizing preservation of water quality and creeks' surroundings driven with a improved living standards from rapid economic growth since 20th century. In order to achieve, concerned on what we call "systematic creek management" based on geographic information system, collaborating with geographical and spatial element and its relevant attribute information. Thus, above mentioned methods are to improve the current creek management plan which manages all kinds of necessary information such as improving, enforcing and managing water merely on drawing, paper work file. this study investigated it would be feasible to improve natural friendly creek maintenance plan and flood control which adapted creek management system built to the creeks in ulsan metropolitan. In result of that, it would be very possible to maximize its efficiency by applying creek managing system and practicing on the work as well as controlling information and plans.

        지하주차장 공기환경 개선을 위한 이오나이저의 적용효과 검토

        The main objective of this study was to verify a possibility of indoor air environment enhancement via actual operation of ionizer device. Two pollutants, benzene and toluene, were used as indicators for evaluating the air cleaning performance of ionizers. The test results show that negative air ions may provide the air cleaning effect which can be acquired by supplying 0.058-0.100(㎥/hr), 0.035-0.070(㎥/hr) ventilation. Another indicator of this test result, air cleaning factor(ACF) was also indicated that the impact of negative air ions is consistently maintained during the ionizer operation. In addition, according to experimental results, it was also found that removal efficiency of benzene was 2.1 times higher than that of toluene. From this research and existing literature, one can conclude that additional indoor air quality improvement can be expected by applying ionizers in ventilation system for underground parking facilities.
