본 연구는 병원에 근무하는 간호사의 이직률과 병원의 구조적 특성 간 관련성을 확인하기 위하여 시행하였다. 자료는 한국보건산업진흥원에서 매년 조사하는 병원경영분석자료를 이용하였으며, 2008년에 전국에서 자기기입식으로 입력한 병원은 247개였다. 2008년 우리나라 간호사의 평균 이직률은 32.0%(공공병원 15.5%; 사립병원 35.3%)였으며, 민간병원, 300병상 이하 병원, 비도시 지역병원, 간호사평균급여가 2,000만원 미만인 병원, 100병상 당 간호사 수가 20명 미만 인 병원에서 높았다. 실제 이직률에 영향을 미치는 요인은 경영주체(공공 또는 사립), 병원 규모, 병원 소재지, 간호사의 평균 임금, 100병상당 간호사 수였다. 각 병원의 이직률과 총 병상 수(r=-0.322), 간호사 평균급여(r=-0.186), 100병상 당 간호사 수(r=-0.390)는 상관관계가 있었으나, 이직률에 설명력이 있는 변수는 경영주체의 공공성 여부, 총 병상 수, 100병상 당 간호사 수였다(R2=.257). 결론적으로 병원간호사의 이직률은 민간병원 근무자가, 총 병상수가 적을수록, 100병상 당 간호사 수가 적을수록 높았다. 간호이직 관련요인을 명확히 하기 위하여 개인요인, 사회요인과 병원요인을 동시에 분석하는 추가연구가 필요하다. The aim of this study was to find out the relationship between hospital structural factors and nurses' turnover rate. Nurse turnover rate and the hospital's characteristics was using Statistics for Hospital Management Data of Korean Health Industry Development Institute. The data were reported 247 hospitals in 2008 all over the country. The turnover rate among nurses was 32.0% in Korea as of 2008; those factors actually affecting the rate included whether the hospital was managed publicly or privately, the hospital size, region, the average salary for nurses, and the number of nurses per 100 beds. While each hospital had the turnover rate correlated with the total number of beds (r=-.322), the average salary for nurses(r=-.186) and the number of nurses per 100 beds (r=-.390), variables explaining the turnover rate were whether the hospital was managed publicly or privately, the number of beds, and the number of nurses per 100 beds (R2=.257). In conclusion, nurses working in a privately-managed hospital and hospitals which had a smaller number of beds and which had a smaller number of nurses per 100 beds showed a higher turnover rate. Further study is required to elucidate the factors of the nurses' turnover.
중부 지역 여대생의 커피 음료를 통한 열량 섭취 수준에 따른 영양 섭취 상태와 골밀도 평가

This study was designed to investigate the relationship between the dietary intake according to calorie intake from a coffee containing beverage and the bone health status of 189 female collegians. The study was conducted through questionnaires, anthropometric checkup, 3-days food records and ultrasound measurement of calcaneus bone mineral density. Subjects were divided into three groups: students not drinking coffee(non-coffee group, N=56), students consuming <100 kcal daily from coffee(low-calorie coffee group, N=84), and students consuming ≥100 kcal of their total daily calories from coffee(high-calorie coffee group, N=49). There were no significant differences in weight, height, body mass index, body fat and calcaneus bone mineral density among the three groups. The low-calorie coffee group usually drank black coffee or instant coffee mix, and the high-calorie coffee group habitually drank coffee with milk or sugar syrup. There were no significant differences in the mean daily energy and food intake among the three groups. However, vitamin B2(p<0.05) and calcium (p<0.01) intake in the high-calorie coffee group were higher than in the non-coffee group. Also, mean intake of sugars, fish and shellfishes, milks and beverages in the high-calorie coffee group were also significantly higher than in non coffee group(p<0.05). There was no significant difference in the Korean Dietary Diversity Score(KDDS) among the three groups. The main calcium source was milk in all three groups, and milk intake(total and included with coffee) was highest in the high-calorie coffee group. Although no significant difference was apparent between the high-calorie and non-coffee groups concerning anthropometric factors and calcaneus bone mineral density, consumption of coffee may have influenced food and nutrient intake. The results suggest that consumption of milk-supplemented coffee may be of nutritional benefit.

본 연구는 수술환자의 타지역 의료이용과 관련된 요인을 규명하기 위하여 2004년부터 2006년도의 퇴원손상 환자조사뿐 아니라, 인구센서스 및 보건의료자원 실태조사자료를 이용하여 교차분석, 로지스틱 회귀분석을 실시한 결 과 다음과 같다. 첫째, 수술환자의 의료이용은 수도권 중심으로 집중화 현상이 뚜렷하다. 이를 개선하기 위해서 정부 에서는 의료기관 및 병상수를 공급하는 일 뿐만 아니라 의료의 지역화를 위해 지방에서의 의료의 질적 수준을 올리 는 정책이 필요하다. 둘째, 눈 및 부속기 질환 및 암환자의 타지역 수술률이 높으므로 질병에 따른 의료의 지역화를 위한 특정 질환의 지역화 정책추진이 요구된다. 셋째, 산재환자 수술환자의 타지역 진료율이 시간이 갈수록 증가하므 로 손상환자를 위한 지역화 정책도 필요하다 할 것이다. This study conducted a chi-square test and a logistic regression analysis by not only using a investigation data of discharged patients with damages from 2004 to 2006, but also using a census and a research data on the actual condition from health care system. The result follows; First, the centralization of medical utilization of operation patients is becoming apparent, centrally the capital area. To improve this, a policy that can not only provide medical centers and sickbed, but also improve the quality of local medical treatments for the localization of medical treatments. Second, propelling localization policy of certain diseases for the localization of medical utilization is needed as the rate of non-residential of operation patients that have diseases of the eye and adnexa, or cancers is high. Third, a localization policy for patients with damages is needed as the rate of treatments in other regions of operation patients with industrial accidents is increasing day by day.
