목적: 본 연구에서 현성 하지 부동으로 발달성 고관절 이형성증(developmental dysplasia of the hip, DDH)을 감별 진단하기 위해 의뢰된 영아들에서 태아기 자세성 변형의 양상을 조사해보고 이러한 태아기 자세성 변형이 DDH와 어떤 연관성이 있는지 알아보고자 하였다. 대상 및 방법: 후향적 연구로, 2013년 3월부터 2015년 3월까지 현성 하지 부동으로 DDH가 의심되어 외래에 내원한 영아 150명의 의무기록과 영상의학적 검사를 조사하였다. 결과: 현성 하지 부동은 좌측 하지가 짧은 경우가(n=86, 57.3%) 많았으며, 62명(41.3%)에서 사두증, 124명(82.7%)에서 체부 만곡이 관찰되었다. 여아의 경우 생식기 방향을 통해 간접적으로 유추하여 평가한 골반 경사(pelvic obliquity)가 62명(63.3%)에서 관찰되었다. 고관절 진찰에서 이상 소견을 보이지 않은 139명에서는 DDH가 발견되지 않았으며, 고관절 진찰에서 이상 소견을 보인 11명 중에 6명에서 DDH로 진단되었다. 결론: 태아기 자세성 변형과의 동반 여부에 상관없이, 고관절 진찰에서 이상 소견이 있는 영아에서만 DDH가 진단되었다. 따라서 일차 DDH 선별 검사 시에 고관절 진찰을 함께 시행함으로써 DDH의 추정 진단에 대해 도움을 줄 수 있음을 염두해 두어야 한다. Purpose: This study aimed to investigate the clinical features of congenital postural deformities and lower extremity asymmetry with respect to the presence of developmental dysplasia of the hip (DDH) in infants with a discrepancy of the limb length referred for suspected DDH. Materials and Methods: We retrospectively reviewed the medical records and radiographs of 150 infants who visited Korea University Anam Hospital Orthopedic Clinic for suspected DDH between March 2013 and March 2015. Results: There were greater numbers of infants with a shorter lower extremity on the left side (n=86, 57.3%) than the right. Plagiocephaly was present in 62 infants and trunk curvature in 124 infants (82.7%). Pelvic tilting—indirectly assessed by a skewed direction of the external genitalia in female infants—was present in 62 infants (63.3%). None of the 139 infants with normal physical examination of the hip were diagnosed with DDH. Of those 11 infants with abnormal findings from the hip physical examination, a total of 6 infants were diagnosed with DDH. Conclusion: Regardless of the associated findings of congenital postural deformation, all infants diagnosed with DDH had abnormal findings from the physical examination of the hip joint. Thus, we conclude that the hip examination is important as the primary clinical screening in aiding the diagnosis of DDH.
과산화수소 농축을 위한 투과증발공정의 정량적 위험성 분석

초고농도 과산화수소 제조를 위한 투과증발 공정의 정량적 위험성 분석을 수행하였다. 잠재적 주요 사고는 분해반응에 따른 폭발 및 화재이며 실험실 규모일 때 사고결과는 카테고리 3에 속하는 것으로 판단된다. 대상공정에서 분해 반응이 일어나는 과정을 사건트리 형태로 모델링하고 사고원인들의 확률함수를 유사사건 발생빈도 자료를 근거로 설정하였다. 구축된 모델을 사용하여 사고율을 계산한 결과, 수용 가능한 위험수준, 즉 사고빈도가 10-4/yr 이하가 되려면 추가 안전장치가 필요한 것으로 파악되었다. 이를 위해 방호계층분석을 적용한 결과, 촉매반응을 막기 위한 본질적 안전설계, 과열을 막기 위한 SIF (safety instrumented function), 그리고 분해반응이 일어나더라도 폭발로 이어지지는 것을 막는 릴리프 시스템이 요구되었다. 제안된 방법은 과산화수소 농축을 포함한 다양한 화학공정의 안전관리시스템 개발에 기여할 수 있을 것으로 기대된다. Quantitative risk analysis has been performed for a pervaporation process for production of high test peroxide. Potential main accidents are explosion and fire caused by a decomposition reaction. As the target process has a laboratory scale, the consequence is considered to belong to Category 3. An event tree has been developed as a model for occurrence of a decomposition reaction in the target process. The probability functions of the accident causes have been established based on the frequency data of similar events. Using the constructed model, the failure rate has been calculated. The result indicates that additional safety devices are required in order to achieve an acceptable risk level, i.e. an accident frequency less than 10-4/yr. Therefore, a layer of protection analysis has been applied. As a result, it is suggested to introduce inherently safer design to avoid catalytic reaction, a safety instrumented function to prevent overheating, and a relief system that prevents explosion even if a decomposition reaction occurs. The proposed method is expected to contribute to developing safety management systems for various chemical processes including concentration of hydrogen peroxide.
양측성 신동맥 협착증에서 발생한 급성 비대상성 심부전과 급성 신손상

Atherosclerotic renal artery stenosis (RAS) may result in hypertension, azotemia, and acute pulmonary edema. We report on a renal angioplasty with stent placement for bilateral RAS in a patient with acute decompensated heart failure and acute kidney injury. A 67-year-old female patient was admitted to our hospital with acute shortness of breath and generalized edema. Echocardiography showed left ventricular wall motion abnormality and the follow up electrocardiography showed T wave inversion in the precordial leads. We performed a coronary angiography to differentiate ischemic heart disease from non-cardiac origin for the cause of the heart failure. The coronary angiography showed no significant luminal narrowing, but bilateral RAS was confirmed on the renal artery angiography, therefore, we performed renal artery revascularization. After the procedure, the pulmonary edema was improved and the serum creatinine was decreased. Two weeks later, an echocardiography showed improvement of the left ventricular systolic function.

AWD system means it delivers drive force to all wheels so that it can help vehicle escape low mu surface or climb hill more conveniently. With the growing concern for vehicle safety issues, application of AWD is not only focusing on vehicle performance but also vehicle safety. To design AWD vehicle control logic, AWD system modeling which has similar simulations results of real AWD vehicle is crucial. This paper describe how to model transfer case which is a mechanical device to transfer some portion of torque of main drive shaft to sub drive shaft. Especially, wet clutch type model which can control the amount of torque distribution by allowing clutch slipping is introduced. To verify the feasibility of transfer case model, Carsim/Simulink based AWD system simulation results of vehicle which is based on RWD will be presented.
