        버섯 중독 환자에서의 ^(99m)Tc-galactosyl human serum albumin (GSA) scintigraphy 소견 : 간초음파 소견과의 비교 Comparison with Liver Ultrasonography

        광대버섯과 같은 독버섯 중독시 간세포에 독성을 나타내어 간 기능의 이상을 유발할 수 있다. 저자들은 개나리광대버섯을 섭취한 3명의 환자에서 간 기능을 평가하기 위하여 간세포에 선택적으로 섭취되어 간기능을 평가하는데 사용되어지고 있는 99mTc-galactosyl human serum albumin (Tc-GSA)을 이용한 간 스캔을 시행하였고 간 초음파 검사와 비교하여 보았다. Tc-GSA 185MBq (3mg of GSA)을 정맥주사후 30분동안 동적 영상과 간과 심장 부위의 시간 방사능 곡선을 얻었다. 간기능의 정도를 평가하기 위하여 Tc-GSA의 간섭취 정도와 혈중 정체정도를 시각적인 평가와 간과 심장 부위의 시간 방사능 곡선을 이용한 반정량적인 평가를 시행하여 간섭취지수와 혈중제거지수를 구하였다. 시각적인 평가에서 2명의 환자에서 Tc-GSA의 경미한 정도의 간섭취 감소와 혈중 정체를 보였고 1명에서는 중등도의 간섭취 감소와 혈중 정체를 보였다. 반정량적인 평가에서는 2명의 환자에서 경미한 정도의 간섭취지수의 감소와 혈중제거지수의 증가를 보였고 1명에서 중등도의 간섭취지수의 감소와 혈중제거지수의 증가가 관찰되었다. 간초음파상에서는 1명의 환자는 지방침윤이 관찰되었으며 1명의 환자에서는 미약한 정도의 지방간이 관찰되어 급성 간기능 장애를 적절히 반영하지 못하였다. 본 증례를 통해 저자들은 간초음파 검사가 특이한 소견을 나타내지 못하여 간세포에 영향을 주는 독소나 약물 중독시에 간 영상화에는 Tc-GSA 간스캔이 유용하게 사용될 수 있을 것으로 추측할 수 있었다. ^(99m)Tc-galactosyl human serum albumin (Tc-GSA) is a radiopharmaceutical that binds to asialoglycoprotein receptors, which are specifically present in the hepatocyte membrane. Because these receptors are decreased in hepatic parenchymal damage, the degree of Tc-GSA accumulation in the liver correlates with findings of liver function test. Hepatic images were performed with Tc-GSA in patients with acute hepatic dysfunction by Amantia Subjunquillea poisoning, and compared with these of liver ultrasonography (USG). Tc-GSA (185 MBq, 3 mg of GSA) was injected intravenously, and dynamic images were recorded for 30minutes. Time-activity curves for the heart and liver were generated from regions of interest for the whole liver and precordium. Degree of hepatic uptake and clearance rate of Tc-GSA were generated by visual interpretation and semiquantitative analysis parameters (receptor index :LHL15 and index of blood clearance :HH15). Visual assessment of GSA scintigraphy revealed mildly decreased liver uptake in all of subjects. The mean LHL15 and HH15 were 0.886 and 0.621, graded as mild dysfunction in 2, and mild to moderate dysfunction in 1 subject. In contrast, liver USG showed no remarkable changes of hepatic parenchyme. Tc-GSA scintigraphy was considered as a useful imaging modality in the assessment of the hepatic dysfunction.

      • Anisakids larvae의 형태학적 분류 : 해산어류 및 두족류를 중심으로 On the Marine fishes and Cephalopoda

        In order to study classification of the morphological features and types of anisakids larvae eight species of marine fishes and two cephalopoda purchased in the near southern sea in Korea, were investigated and classified into the morphological characteristics of each type of the larvae, and the following results were obtained on the basis of the methods of Berland(1961), Koyama et al(1969) and Chai et al(1986). 1. Anisakis(typeⅠ) larva: The length and width of nematode are 14.2∼26.4mm and 0.22∼0.51mm respectively. The anterior region has well-developed boring tooth. Tail is 0.09∼0.12mm and the tip of tail has a globular form and well-developed a mucron. 2. Terranova (type A) larva: The length is 24.5∼34.1mm and longest among anisakids larvae. Width is 0.50-0.84mm, and intestinal cecum had reached one-third of anterior larva of ventriculus, and the worm has three lips at anterior end, and had boring tooth on the lips. The tail is 0.08∼0.12mm and has well-developed a mucron. 3. Raphidascaris spp. larva: The nematode was 8.20∼8.42mm in length and 0.21∼ 0.29mm in width. The anterior region has boring tooth. Especially, the genital organ was developed. The tail is 0.11∼0.12mm and has well-developed mucron at the tip of it. 4. Contracaecum(type A) larva: The nematode was 6.8∼9.4mm in length and 0.09∼ 0.15mm in width. The lips were depressed. The Anterior region do not have boring tooth. The tail is 0.08∼0.09mm relatively short and has about 15∼30 micro-spines and mucron at its terminal portion. 5. Contracaecum (Type A') larva: The nematode was 17.5∼24.4mm in length and 0.22∼0.27mm in width. The lips were depressed and the anterior region does not have boring tooth as the type of Contracaecum A. The tail whose length was 0.20∼0.31mm was very long and had about 7∼12 minutes spines at its terminal portion. 6. Contracaecum (Type C) larva: The worm was 13.6∼35.2mm and 0.28∼0.60mm in size, and the lips were depressed and the anterior region does not have boring tooth. The tail did not have mucron, as the type of Contracaecum A, A'. 7. Contracaecum(Type C') larva: The worm was 10.2∼14.8mm and 0.15∼0.22mm in size, and had boring tooth as the type of Contracaecum A, A' and C. The tail was 0.08∼0.09mm and relatively short. In addition, it did not have mucron at its terminal portion. 8. Contracaecum (Type D) larva: The worm was 12.0∼19.8mm and 0.16∼0.25mm in size, and had boring tooth as the type of Contracaecum C'. The tail was 0.11∼0.16 and had small mucron at its terminal portion. 9. Contrecaecum (Type D') larva: The worm was 7.10∼16.5mm and 0.14∼0.25mm in size, and had boring tooth as the type of Contracaecum C' and D. The tail was 0.11∼0.16mm, and covered with sheath, as the type of Contracaecum D but did not have a mucron.

      • 해산어류와 두족류의 Anisakids Iarvae의 감염현황

        In this study, the infection status of the anisakids larvae was investigated. 8 species marine fishes and 2 species cephalopoda purchased from the near Southern sea in Korea, were examined to provide the importance of infection source of human body and the basic data for the prevention. The results obtained from this study were as follows. 1. The infection rate of anisakids larvae in marine fishes was 100% in Trichiurus lepturus, 100% in Scomber japonicus, 75.0% in Trachrus japonicus, 70.0% in Pseudosiciaena manchurica, 69.23% in Astroconger myriaster, 58.33% in Konosirus punctatus, 40.0% in Liparis tanakai, 20.0% in Seriola quinqureradiata, respectively. And, the infection rate of anisakids larvae in cephalopoda was 40.0% in Sepiella maindroni but Octopus rariabilis was not infected at all. 2. The total number of anisakids larvae collected from the parasitic organs was 2,199. The details were as follows: 1,032(46.93%) in the viscera, 868(39.47%) in the omentum. 191(8.69%) in the stomach, 82(3.73%) in the liver and 26(1.10%) in the muscle, respectively.
