        사단 의무대(대) 보건간호장교의 예하 의무부대 간호업무 지도방문 경험

        Purpose: This study was aimed at preparing basic data needed to improving healthcare services in subordinate medical unit in Republic of Korea Army division. For that purpose, this study investigated nursing officers' vocational guidance experiences for subordinate medical unit. Methods: This study was based on a Colaizzi's phenomenological qualitative study using individual in-depth interviews, and data were collected from 6 army nurses with experiences as nursing officers in army division medical facilities. Results: The 5 categories of 'planning a visit tailored to the subordinate unit', 'starting with visit but ending with censorship', 'facing the indifference and lack of expertise', 'ending the visit with correction', and 'accepting the guidance visit as nurses calling' were obtained. Conclusion: This study analyzed the phenomenon of vocational guidance visit for the healthcare personnel in subordinate medical units without nurses. Therefore, it has value as a scientific basis to develop educational materials for military division nursing officers in the future. Furthermore, the result illuminates the need of estimating military division nursing officers' work on vocational guidance visit.

        국군간호사관생도의 조직몰입, 간호즉응력 및 간호전문직관

        Purpose: The purpose of the study is to investigate the level of institutional commitment, nursing professionalism, and nursing readiness of Korea Armed Forces Nursing Academy (KAFNA) and to analyze correlations among variables. Methods: This study was a cross-sectional descriptive study. Data were collected from 299 cadets from all student years. The self-reported questionnaire is composed of items about general features, institutional commitment, nursing readiness, and nursing professionalism. Results: The mean score of institutional commitment, nursing readiness, and nursing professionalism was 3.93, 3.43, and 3.64 respectively. The level of institutional commitment, nursing readiness, and nursing professionalism was significantly different according to the general features, commonly to ‘student year’. Institutional commitment and nursing readiness were significantly correlated among upper years of cadets but not in lower years. Institutional commitment and nursing professionalism had significant correlation among all student years. Conclusion: Based on the results, it is suggested that KAFNA’s relevant personnel consider the grade year of cadets when developing an education or training program. Furthermore, Educational opportunities must be provided for cadets that as they go up in years they maintain their level of institutional commitment. Lastly, support and educational intervention are essential for senior cadets in order to maintain the high institutional commitment even after they graduate from KAFNA.

        청장년층 뇌졸중 환자의 Prehospital delay와 영향요인

        Purpose : This study examines stroke patients` prehospital delay and its influencing factors to provide basic information for effective management and treatment of young adult and middle-aged stroke patients. Methods : The analyzed medical records were 260 stroke patients who were admitted to a university hospital from January, 2012 to April, 2015. The data included the time range from symptom outbreak to arrival to the hospital, National Institute of Health Stroke Scale (NIHSS) score, and general characteristics. Descriptive statistics, chi-square test, paired t-test, and multinomial logistic regression were conducted. Results : Only 33.1% of the patients arrived at the hospital within 3 hours while 55.8% of them took 6 hours or longer after stroke symptoms` onset. Those who arrived within 3 hours after the symptom onset demonstrated a significant decrease in NIHSS scores when they were discharged, compared to those measured upon arrival, by 1.36 for those in their 40s (p=.010) and 1.04 in their 50s (p=.015), respectively. Sex, age, medical history of atrial fibrillation, and type of stroke were found to influence on prehospital delay of 6 hours or longer. Conclusion : Although prehospital delay affects the prognosis of stroke patients, the number of patients entering a medical facility within 3 hours after onset of symptoms is still low. This study suggests a longitudinal future research to identify influencing factors on prehospital delay by different age groups and to develop education programs for each group.

        Rapid Prototyping Instructional Systems Development에 근거한 국군간호사관학교의 군 간호학 교육체계 개발

        Purpose : The aim of the study is to develop military nursing curriculum of Korea Armed Forces Nursing Academy (KAFNA). Methods: Rapid Prototyping Instructional Systems Development (RP-ISD) was used. The literatures and current military nursing course syllabi were reviewed and 4 experienced instructors at KAFNA were interviewed to verify the current educational goal. To analyze educational needs and relevant contents, 36 newly graduated nursing officers and 40 head nursing officers working in military hospitals as well as the instructors teaching military nursing subjects were interviewed. Data was collected during the interviews and analysed into the categories and themes using content analysis. Results: The goal, objectives, subjects, and components of military nursing curriculum were proposed in the 3 Prototypes. Based on the reviews for usability by the experts who were education specialists, nursing faculty members and experienced nursing officers, prototypes were revised in the final version. The KAFNA military nursing curriculum were finalized with 11 subjects and their detailed components. Conclusion: The study proposed the methodology to develop a nursing curriculum, integrated with skills necessary in military nursing practice.
