        실리콘관삽입술 환자에서 콧속 실리콘관 위치와 배양 결과에 대한 연구

        목적: 실리콘관삽입술 후 비강 내 실리콘관의 위치와 방향 변화와 수술 후 임상양상과의 관계를 알아보고자 하였다. 대상과 방법: 이 연구는 코눈물관폐쇄로 진단받고 실리콘관삽입술을 시행한 52명 70안을 대상으로 시행하였다. 실리콘관 삽입 직후 비강 내 실리콘관의 위치 및 방향과 제거 전 위치 및 방향을 비교하였다. 균 배양은 실리콘관 삽입 전에는 비강 내 면봉 배양검사를 시행하였고 제거 시에는 제거된 실리콘관으로 세균 및 진균 배양을 시행하였다. 결과: 비강 내 실리콘관 길이는 삽입 직후 8.7 ± 3.1 mm에서 제거 시 7.3 ± 3.4 mm로 감소하였으며, 방향은 후방으로 이동하는 양상이었다(p<0.05). 세균 배양률은 실리콘관 삽입 전 94.3%에서 제거 시 75.7%로 변했다. 가장 흔한 그람 양성균은 술 전후 모두 혈청응고효소 음성 포도상구균이 제일 많았고, 그람 음성균은 술 전 클리브시엘라 막대균에서 술 후 스테노트로포모나스 말토필리아로 변했다. 진균 배양률은 7.1%에서 51.4%로 변화하였고, 후방으로 이동하는 양상일 때 진균 배양 양성률이 증가하였다(p<0.05). 실리콘관의 길이 및 방향은 임상증상, 눈꺼풀높이의 변화와 상관관계가 없었다. 결론: 실리콘관삽입술 후 비강 내 실리콘관 길이는 짧아지고, 고정 부위 방향이 후방으로 이동하는 경향을 보였다. 이와 함께 비강내 진균 배양률이 증가하는 양상을 고려하면 비강 내 실리콘관 고정 부위를 아래콧길 전방에 위치시키는 것이 좋을 것으로 생각한다. 추후 눈물배출계의 위치에 따른 진균 배양에 대한 연구가 추가적으로 필요할 것이다. Purpose: To evaluate the relationship between the position and direction of a silicone tube in the nasal cavity after silicone tube intubation and clinical features. Methods: This study included 70 eyes of 52 patients who were diagnosed with nasolacrimal duct obstruction and had silicone intubation surgery. The position and direction of the silicone tube were compared after tube intubation and before extubation. An intranasal swap culture was performed before tube intubation, and the removed silicone tubes were cultured. Results: The length of the silicone tube decreased from 8.7 ± 3.1 mm after intubation to 7.3 ± 3.1 mm before extubation, and the direction of the tube moved backward in the nasal cavity (p < 0.05). The bacterial culture rate changed from 94.3% before intubation to 75.7% before extubation. The most common Gram-positive bacteria (both preoperative and postoperative) was coagulase- negative Staphylococcus aureus. The most common Gram-negative bacteria were converted from Klebsiella aerogenes to Stenotrophomonas maltophilia. The fungal culture rate changed from 7.1% to 51.4%, and the culture rate increased when the silicone tube direction was moved backward (p < 0.05). The length and direction of the silicone tubes were not correlated with clinical symptoms or changes in eyelid height. Conclusions: The length of the silicone tube in the nasal cavity decreased, and the tube tended to move in a posterior direction after silicone tube intubation. Considering the increase in the intra-nasal fungal culture rate, it is better to place the silicone tube in the anterior part of the nasal cavity from the inferior meatus. To clarify further the fungal infection and its clinical findings, a study of cultures with respect to location in the lacrimal system would be required.

        국산 유기가공식품 소비의향 분석

        The purpose of this study is to analyze consumption of homemade organically processed food (HOPF), and to derive directions for consumption promotion of HOPF. A survey was conducted for quantitative analysis regarding consumption. This study used an Ordinal Logistic Regression Model to derive more significant results in analyzing factors of consumption. The findings was that younger consumers with high income are more likely to purchase HOPF. And those consumers with high price and quality contentment are more likely to purchase HOPF. And contentment with certification institutions and improvement of health have a significant positive relationship with consumption. Consumers were found to pay 51 percent more for HOPF than for non-HOPF products. This level show that the current level of price premium for HOPF is 51 percent higher than their desired level. In order to reduce the price premium for HOPF, effective policy programs should be developed. A targeted market strategy to sell HOPF to younger consumers with high income is needed to boost consumption. A strict certification management system should be established to enhance consumer reliability in HOPF.

        광역단지 유기농업 경영체 분석과 발전방향

        The objective of this paper is to analyze management performance of organic farming management organizations in the large-scale environment-friendly agricultural districts, focusing on cases of organic farming organizations in Chungnam Asan city and Gyeongnam Sancheong-gun which are leading regions in Korean organic agriculture. The management performance in Asan Organic Farming Organization showed that ratio of operating gain to revenue in 2008~2009 increased by 1.2% point compared to in 2006~2007. The management performance in Sancheong Organic Farming Organization showed revenue gain by KRW 2 million in 2009 whereas it suffered a loss by KRW 24 million in 2008. The management performances implied that the amount of financial performance was not yet large and would be a symbol for soundly developing organic farming organization. This paper suggested that network system, recycling agriculture, and the enlargement of processing industry would be very useful to maximize synergy effect in organic farming organizations in a large-scale environment-friendly agricultural districts.

        백내장수술 후 시력저하를 유발한 성상유리체증

        Purpose: To report a patient with asteroid hyalosis, which induced reduction of the visual acuity after cataract surgery. Case summary: A 79-year-old female with no ophthalmic history visited our clinic with decreased vision. Her best-corrected visual acuity in both eyes was 20/100 in the right eye and 20/60 in the left eye. She had senile cataract in both eyes, and asteroid hyalosis in the left eye. After cataract surgery, her visual acuity in the left eye decreased to finger count. There were no specific findings that could have caused the visual disturbance, except dense asteroid bodies on fundus examination. After removal of the asteroid bodies by vitrectomy, the best-corrected visual acuity of the left eye recovered to 20/20. Conclusions: In patients with asteroid hyalosis, it is possible that decreased vision may be affected by changes of asteroid body properties in the vitreous gel after cataract surgery. The vision should improve after vitreous surgery. 목적: 백내장수술 후 시력저하를 유발하는 성상유리체증환자 1예를 보고하고자 한다. 증례요약: 특별한 안과적 과거력이 없는 79세 여자 환자가 시력저하를 주소로 내원하였다. 검사상 양안의 최대교정시력은 우안 20/100, 좌안 20/60이었으며, 노년 백내장 및 좌안의 성상유리체증이 관찰되었다. 양안 백내장수술을 시행하였으며, 술 후 좌안의 시력은 안전수지로 저하되었다. 안저검사상 성상유리체증 외에 시력저하를 유발할 만한 특이 소견은 없었다. 이에 대해 좌안 유리체절제술을 시행하였고, 술 후 좌안 교정시력이 20/20으로 호전되었다. 결론: 성상유리체증이 있는 환자에서 백내장수술 시 성상유리체증에 따른 시력저하가 발생할 수 있으며, 유리체절제술로 호전될 수 있음을 염두에 두어야 한다.

      • 유기가공식품의 소비실태분석 연구

        The purpose of this study is to systematically analyze the consumer purchasing behaviors and future consumption towards homemade organically processed food(HOPF), and to derive directions for consumption promotion of HOPF. To this end, a survey was conducted for quantitative analyses regarding consumer purchasing behaviors and future consumption. This study used Ordinal Logistic Regression Model to derive more significant results in analyzing factors of future consumption. The analytical results were used to derive policy directions aimed at boosting the HOPF consumption. The consumer survey was conducted for 503 housewives living in Seoul and Gyeonggi Provinces using online. Among the respondents(400 housewives who have purchased HOPF) those in their 30s amounted to 40(10.5 percent), followed by those in their 40s with 182(45.5 percent), and those in their 50s or above with 178(44.5 percent). From the income perspective, 52.0 percent of the respondents were 3million won to 4million won. Among the respondents(103 housewives who have not purchased HOPF) those in their 30s to 40s amounted to 70.9 percent. From the income perspective, 58.3 percent of the respondents were 3million won to 4million won. The consumer survey conducted regarding certification mark and additives sign has found that the majority of domestic consumers confirm those. In detail 72.8 percent of those responded said that “I confirm certification mark while buying HOPF.” And 67.8 percent of those responded said that “I confirm additives sign while buying HOPF.” In relation to the future consumption toward HOPF, the Ordinal Logistic Regression Model was applied for analysis. The findings was that the younger consumers with high income are more likely to purchase HOPF. And those consumers who recognize the price and quality contentment to be high are more likely to purchase HOPF. And the contentment of certification instrument and improvement of health have a significant plus relationship with future consumption. The consumer survey conducted regarding consumption promotion has found that price reduction, reliability improvement, and store expansion are needed to promote consumption. Consumers who have purchased HOPF were found to pay 51 percent more for it. This level show that the current level of price premium for HOPF is 51 percent higher than their desired level. The consumer survey conducted regarding improvement of health has found that 48.8 percent of those responded said that "HOPF is helpful to family health. In order to reduce price premium for the HOPF effective policy programs should be developed. The target market strategy to sell HOPF to the younger consumers with high income is needed to boost consumption. To expand consumption, it is necessary to strengthen promotional efforts. The strict certification management system should be established to enhance consumer reliability in the food.
