급성 심정지 환자의 자발순환회복 영향요인: 서울시 119 특별 구급대 데이터

서울시 특별구급대 데이터(2019.07–2020.06)를 활용한 본 연구는 급성심정지 환자의 자발순환회복 여부에 미치는 영향요인을 분석하여, 향후 더욱 체계적인 응급의료 시스템 구축에 기초자료를 제공하고자 실시하였다. 12개월 동안 서울시 특별구급대가 출동한 건수는 총 85,830건 이었고, 그중 심폐정지 세부상황 내역이 존재하는 건수는 총 1,865건 이었다. 최종적으로 597건의 사례를 분석을 위해 활용하였다. 105건은 자발순환회복이 되었으며, 492건은 자발순환회복이 되지 않았다. 따라서 독립변수에 대한 두 군의 분포차이를 알아보기 위해 카이제곱 검정을 실시하였고, 두 군을 결정하는 영향요인을 알아보기 위해 로지스틱 회귀분석을 실시하였다. 자발순환회복여부에 따라 유의한 분포차이를 보인 변수는 성별, 연령, 심정지목격여부, 구급대 제세동 실시여부, AED 응급처치여부 및 실제 활용여부, AED 심전도코드, 기도확보유지기 사용여부, 병원도착까지 걸린시간으로 나타났으며, 이중 연령은 유의한 영향요인으로도 밝혀졌다. 연구결과 특별구급대의 시행은 전반적으로 환자의 자발순환회복률을 높이는 것으로 나타났으며, 이를 통해 환자 생존율을 더욱 높일 필요가 있다. 또한 적극적인 심폐소생술 교육, AED 배치 확대, 특별구급대의 권한 확대 등을 통해 더욱 효율적인 응급의료 시스템의 구축이 필요하다. To provide basic data for the establishment of a systematic emergency medical system by analyzing the factors affecting the recovery of spontaneous circulation in patients with acute cardiac arrest. A total of 85,830 cases were dispatched to the Seoul Special Ambulance during 12 months, of which 1,865 cases had detailed cardiopulmonary arrest details. Finally, the 597 case was used for analysis. A chi-square test was performed to examine the distribution differences for the independent variables, and logistic regression analysis was performed to identify the factors that determine the two groups. Variables that showed significant distribution differences according to spontaneous circulation recovery were gender, age, witnessed cardiac arrest, ambulance defibrillation, AED first aid and actual use, AED electrocardiogram code, use of airway retainer, hospital It showed the time it took to arrive. Age was a significant influencing factor. It has been confirmed that the implementation of the special ambulance service improves the recovery rate of spontaneous circulation in patients overall, and it is necessary to further increase the survival rate of the patient. In addition, it is necessary to establish an efficient emergency medical system through active CPR training, expansion of AED deployment, and expansion of the authority of special ambulance units.
