완화의료 전문인들의 일터영성(workplace spirituality)이 말기환자 돌봄수행에 미치는 영향

본 연구의 목적은 일터영성(공동체 의식, 일의 의미)과 돌봄수행(정서적, 사회적, 영적) 그리고 소진 간의 관계를 규명하고, 일터영성이 돌봄수행에 영향을 미치는 과정에서 소진의 조절효과를 파악하는 것이다. 설문조사는 전국 68개 완화의료 전문기관의 의사, 간호사, 사회복지사, 성직자를 대상으로 하였으며, 총 114부를 SPSS Statistics 21로 분석하였다. 분석결과, 첫째, 일터영성의 하위요인 중에서 공동체의식은 심리적 돌봄에 영향을 미치고, 일의의미는 사회적 돌봄과 영적 돌봄에 유의미한 영향을 미치는 것으로 나타났다. 둘째, 소진은 돌봄수행에 직접적인 영향을 미치지는 않지만 소진의 독립적 영향력을 측정한 사후분석에서 사회적 돌봄과 영적 돌봄에 미미한 정도의 영향을 미치는 것으로 밝혀졌다. 셋째, 소진은 일터영성이 돌봄수행에 영향을 미치는 과정에서 조절효과가 없는 것으로 검증되었다. 이러한 연구 결과를 바탕으로 본 연구의 시사점, 제한점 및 추후 연구과제에 대하여 논의하였다. This study aims to examine the direct effects of two workplace spirituality dimensions, community at work and meaning in work, and burnout on three caring dimensions, emotional, social and spiritual care, for terminally ill people. It also seeks to examine the moderating effect of burnout in the relationship between two workplace spirituality dimensions and three caring dimensions. For these purposes, data were collected from 114 interdisciplinary team members, doctors, nurses, social workers and pastors, working in 68 palliative care in nationwide by questionnaires. The study results indicated that community at work has a positive effect on emotional care, and that meaning in work has a positive effect on social and spiritual care. The results also provided that burnout does not impact on three caring dimensions and has no moderating effect in the relationship between two workplace spirituality dimensions and three caring dimensions. In the findings, managers or leaders from palliative care should consider the potentially positive influence of workplace spirituality on caring for terminally ill people and can improve practitioners` performance with any interventions to increase workplace spirituality. The limitations of this study and recommendations for the future research were discussed.

The present study examined the effects of the level(16.1Cal%, 36.6Cal%) and degree of saturation(P/S ratio=1.0 and 0.06) of dietary fat, and age(4 weeks and 26 weeks) on the lipid concentrations of plasma, triglyceride(TG) rich lipoprotein(d<1.006g/ml) and LDL(1.019<d<1.036g/ml) in rats. Rats had a low fat·high carbohydrate diet containing high saturated fatty acids (Y_LS, O_LS)showed higher TG levels both in plasma and TG-rich lipoproteins than those of either groups fed low fat or high fat diet with P/S ratio=1(Y_LS,O_LB or Y_HB,O_HB). LDL TG concentrations showed similar patterns to those of plasma and TG-rich lipoproteins. Plasma total cholesterol concentrations were higher in both groups fed a low fat diet with P/S ratio=0.06(Y_LS,O_LS) and high fat diet with P/S ratio=1(Y_HB,O_HB) than in those groups fed a low fat diet with P/S ratio=1(Y_LB,O_LB). In all experimental groups. LDL cholesterol and LDL protein levels of rats aged 4 weeks were significantly lower than those of 26 week-old rats. These observations suggest that a low fat·high carbohydrate diet induce higher TG levels both in plasma and TG-rich lipoproteins especially when the dietary fat is mainly composed of saturated fatty acids as compared to the diet containining low fat·high carbohydrate(P/S_1) of high fat·low carbohydrate(P/S_1).
