        당뇨병성 신부전증 환자의 선단 궤양에 의한 하지절단(1예 보고)

        Calciphylaxis is a rare disease that appear in patients with secondary hyper-parathyroidism or chronic renal failure or that show defect in calcium phosphate metabolism which is characterized by fibrin deposit or calcification of medial wall of vessels causing gradual ischemic skin necrosis. Calciphylaxis is a disease with poor prognosis as skin necrosis can progress rapidly. If left untreated, calciphylaxis will progress to sepsis with high mortality. The treatment is controversial but kidney transplantation or parathyroidectomy is suggested to recover calcium-phosphate metabolism. The authors have experienced calciphylaxis in a patient with chronic renal failure caused by DM nephropathy with characteristic skin lesion and rapid skin necrosis. We describe this case with documentary reviews.

        족관절 유합술

        Ankle arthrodesis has been used as standard treatment of ankle arthritis unresponsive to conservative treatment. Transfibular approach was used for ankles with severe deformities and minimally invasive arthroscopic ankle arthrodesis was used for patients with mild deformities. Anterior approach may be used when lateral approach couldn't be performed. Tibiotalocalcaneal arthrodesis is standard treatment for coexisting ankle and subtalar arthritis, and modified Blair arthrodesis can be used if the talus body resection is necessary in severe talar necrosis. In serious infection of ankle arthritis, arthrodesis can be performed as staged operation. In cases with low bone density and severe deformities, Charcot arthropathy should be considered.

        Callotasis has been widely used to treat brachymetatarsia. But various complications have been reported. Avascular necrosis of the $4^{th}$ brachymetatarsia treated by callotasis has not been frequently addressed in the literature. We report 1 cases of avascular necrosis of the $4^{th}$ brachymetatarsia treated by callotasis with a review of the literature.

        경골 천정(pilon) 골절의 최신 치료

        Pilon fractures involving distal tibia remain one of the most difficult therapeutic challenges that confront the orthopedic surgeons because of associated soft tissue injury is common. To introduce and describe the diagnosis, current treatment, results and complications of the pilon fractures. In initial assessment, the correct evaluation of the fracture type through radiographic checkup and examination of the soft tissue envelope is needed to decide appropriate treatment planning of pilon fractures. Even though Ruedi and Allgower reported 74% good and excellent results with primary open reduction and internal fixation, recently the second staged treatment of pilon fractures is preferred to orthopedic traumatologist because of the soft tissue problem is common after primary open reduction and internal fixation. The components of the first stage are focused primarily on stabilization of the soft tissue envelope. If fibula is fractured, fibular open reduction and internal fixation is integral part of initial management for reducing the majority of tibial deformities. Ankle-spanning temporary external fixator is used to restore limb alignment and displaced intraarticular fragments through ligamentotaxis and distraction. And the second stage, definitive open reduction and internal fixation of the tibial component, is undertaken when the soft tissue injury has resolved and no infection sign is seen on pin site of external fixator. The goals of definitive internal fixation should include absolute stability and interfragmentary compression of reduced articular segments, stable fixation of the articular segment to the tibial diaphysis, and restoration of coronal, transverse, and sagittal plane alignments. The location, rigidity, and kinds of the implants are based on each individual fractures. The conventional plate fixation has more advantages in anatomical reduction of intraarticular fractures than locking compression plate. But it has more complications as infection, delayed union and nonunion. The locking compression plate fixation provides greater stability and lesser wound problem than conventional implants. But the locking compression plate remains poorly defined for intraarticular fractures of the distal tibia. Active, active assisted, passive range of motion of the ankle is recommended when postoperative rehabilitation is started. Splinting with the foot in neutral is continued until suture is removed at the 2~3 weeks and weight bearing is delayed for approximately 12 weeks. The recognition of the soft tissue injury has evolved as a critical component of the management of pilon fractures. At this point, the second staged treatment of pilon fractures is good treatment option because of it is designed to promote recovery of the soft tissue envelope in first stage operation and get a good result in definitive reduction and stabilization of the articular surface and axial alignment in second stage operation.

        심낭막 절편을 이용한 기관지성형

        For the treatment of bronchial stenosis due to trauma, inflammatory and neoplastic lesion, bronchoplastic procedure in the interest of preservation of lung tissue are relatively new developments in the field of thoracic surgery. We reported on case of bronchoplasty using to pericardial patch for the treatment of bronchial stenosis due to chronic inflammation. The patient was 26 years old female and chief complaint was respiratory difficulty. Bronchogram revealed diffuse stenosis of left main bronchus about 4cm and especially, at just below the carina marked narrowing of lumen and fine serration in the wall. At the time of operation, longitudinal incision was made at left main bronchus about 5cm and reconstructed bronchus using to pericadial patch at membranous compartment of bronchus. The postoperative course was uneventful and post-operative follow up bronchography showed that improvement of bronchoplastic segmented region.

        저전력 네트워크 기반의 확장 용이한 스마트 홈 IoT 시스템

        There are increasing interests on smart home systems. However, most of the existing works focus on the functionality itself. In this paper, we propose an extensible smart home system based on low power networking such as CoAP, 6LoWPAN, and Zigbee. The proposed home IoT system consists of Home APP, Home Server, Home Broker, and Power Devices. Each component of the system is connected by the low-power network technologies aforementioned. As the end device, Power Device senses the current consumption of the attached appliance and controls the power to it. Power Device reports the sensing data to Home Server via Home Broker. The Home Broker enhances the scalability of the system. Home Broker extends the service area and the user's services, and it manages the connection of the underlying devices and processes, and transmits data to Home Server from Power Devices. Through the experimental evaluation, we show that the proposed system achieves the design goals such as extensibility and low power networking.

        고피인용 논문을 활용한 유망기술 발굴

        급변하는 과학기술 환경속에서 효율적인 R&D와 경쟁력 확보를 위해 미래 기술을 예측하고 유망기술을 발굴하고자 하는 활동이 전 세계적으로 활발히 이뤄지고 있다. 이에 본 연구는 고피인용 논문을 분석 방법을 이용해 2008년, 2012년도 각각 과학기술 전분야를 넘나드는 최전선의 연구활동을 확인하고, 2008년과 2012년을 비교해 연구활동의 규모가 급증한 영역을 추출하여 10대 유망기술을 도출하였다. 본 연구결과는 연구개발 및 기업의 전략적, 정책적 의사결정을 지원할 것으로 기대된다. Recently, it becomes essential to forecast the future and identify emerging technologies in order to improve R&D efficiency and gain a competitive advantage under rapidly changing environment of science and technology. Therefore this research aims to identify the future and emerging technologies especially for the industry and applied it to list top ten emerging technologies. In this study, we identify research fronts across all areas of science and technology through verifying and comparing the 2008 and the 2012 surge in research activities. Finally we detect rapidly increasing 10 promising technology areas. This research results are expected to provide valuable information to support stragegic and policy decision making.

        레이저 장비의 전송 경로 자가 교정을 위한 무선 네트워크 시스템

        IIoT stands for Industrial Internet of Things used in manufacturing, healthcare, and transportation in networked smart factories. Recently, IIoT's environment requires an automated control system through intelligent cognition to improve efficiency. In particular, IIoT can be applied to automatic calibration of production equipment for improved management in industrial environments. Such automation systems require a wireless network for transmitting industrial data. Self-calibration systems in laser transmission paths using wireless networks can save resources and improve production quality by real-time monitoring and remote control of laser transmission path. In this paper, we propose a wireless networked system for self-calibration of laser equipment that requires a laser transmission path, and we show the results of the prototype evaluation. The self-calibration system of laser equipment measures the coordinates of the laser points with sensors and sends them to the host using the proposed application protocol. We propose a wireless network service for the wired motor controller to align the laser coordinates. Using this wireless network, the host controls the motor by sending a control command of the motor controller in an HTTP message based on the received coordinate values. Finally, we build a prototype system of the proposed design to verify the detection performance and analyze the network performance.
