        콩 발아 중 isoflavone malonylglucosides의 함량 변이

        Soybean produces three major types of isoflavones, daidzein, genistein, and glycitein aglycones and their glucosides and malonylglucosides. It has been known that malonylated glucosides are rapidly converted to their corresponding aglycones due to the unstable thermolabile glucoside malonates; therefore, the analytical study of malonylated glucosides has been insufficient. In this study, we analyzed the malonylglucoside content in soybean seeds. Isoflavone analysis of three soybean cultivars revealed that 81.5~90.0% of the total isoflavones were malonylglucosides, whereas aglycones were rarely detected. Moreover, the total isoflavone content increased during a 5-day germination period where growth regulators and coumaric acid treatments tended to yield higher isoflavone content than the normal germination treatment, however the differences were not significant; notably, the isoflavone accumulation trend continued with additional germination days. The content of malonylglucoside was higher than that of other isoflavones, which was 83.7~86.6% of the total isoflavone content in seeds with a 3-day germination period. Furthermore, isoflavones were significantly accumulated in the hypocotyl of seedlings with a 5-day germination period. The content of isoflavone in the hypocotyl of the Pungsannamul-kong was 10,240 ug/g when treated with coumaric acid, which was considerably higher than that of other cultivars and treatments. Additionally, soybean seeds heated at $60^{\circ}C$ for 1 hour produced higher isoflavone content than non-heated soybean seeds. Our results show that it is possible to increase the isoflavone content in soybean seeds through various treatments.

        시멘트 광택 대퇴 금속주(Cemented Polished Femoral Stem)의 5년 이상 추시 연구

        Purpose: Surface finishing of a cemented femoral stem is a subject of controversy even though the contemporary cementing techniques have improved results. Using the Versys Heritage femoral stem, we evaluated the outcome of using a polished surface. Materials and Methods: The subjects of this study were 95 hip arthroplasties in 82 patients and we used a cemented polished femoral stem with the 3rd generation cement technique and all the surgeries were done between October 2000 and August 2003. There were 58 male patients (64 hips) and 24 female patients (31 hips). The mean age at the time of the index arthroplasty was 52.3 years (26~74 years), and the average body mass index was 24.2±2.75 (19.1~29.8). The average follow up period was 80.7 months (64~109 months). All the hips were evaluated clinically by the Harris hip score and the thigh pain, and they were radiologically assessed by the cement grade and the presence of osteolysis around the femoral stem, as well as the presence of stress shielding of the proximal femur. Results: At the final follow up, the Harris hip score for all the patients had improved from preoperative 58.9 (17-83) to post operative 91.7 (72~100). The cement grade was measured using Barrack`s method. Of the 95 hips, 45 (47.3%) cases were grade A, 48 (50.5%) cases were grade B and 2 (2.1%) cases were grade C1 at the final follow up. There was 1 case of definite loosening. Stress shielding was noted in 65 (68.4%) cases of the zero grade and 19 (20.0%) cases of the 1st grade. Conclusion: In this study, the cemented polished femoral stem showed excellent results at the mid term with a minimum follow up of 5 years. But a longer-term follow-up study will be needed for further understanding the implications of cemented polished femoral stem.

        고관절 디지털 방사선을 이용하여 측정한 방사선적 골다공증 측정 방법들의 유용성

        목적: 디지털 방사선 영상을 이용하여 방사선적 골다공증 지표들을 측정하고 방사선적 골다공증 지표들의 유용성을 알아보고자 하였다. 대상 및 방법: 고관절 주위골절로 진단 받았던 65세 이상의 환자 100 명을 골절 군으로 설정하고 65세 미만의 비 골절 50명을 대조 군으로 설정하였다. Singh Index, Canal-to-Calcar Ratio, Cortical Thickness Index (CTI)를 측정하였으며 골밀도 측정을 위해서 이중 에너지 방사선(DEXA) 골밀도 검사를 시행하였다. 결과: CTI 측정 결과들에 대한 관찰자 내 및 관찰자간 Intraclass Correlation Coefficient (ICC) 결과상 골절 군에서는 ICC 값이 모두 0.8 이상으로 대조 군과 비교 시 골절 군에서 모두 높았으며 대조 군에서는 특히 관찰자간 ICC 값이 상대적으로 낮았다. CTI와 DEXA 골밀도 검사결과들간에는 통계적으로 유의한 상관관계가 관찰되었다(r=0.50~0.58, p<0.05). 결론: CTI가 만족스러운 신뢰도를 보이며 DEXA 골밀도 검사와의 연관성을 보여서 특히 노령의 고관절 주위 골절 환자들에서 골다공증을 진단하고 치료하기 위한 노력을 유도하는데 유용하다고 판단된다. Purpose: We evaluated the usefulness of radiographic parameters for osteoporosis by analyzing the results of radiographic parameters determined by digital hip radiographs and bone mineral density T-scores, as assessed by Dual Energy X-ray Absorptiometry (DEXA). Materials and Methods: The authors reviewed 100 subjects in the hip fracture group and 50 in the non-fracture control group. Digital hip radiographs were assessed to determine the values of Singh index, Canal-to-Calcar Ratio, and Cortical Thickness Index (CTI). Bone mineral density was assessed by DEXA. Results: Intraclass Correlation Coefficient (ICC) results of the CTI were above 0.8 in the fracture group. Compared to the control group, the fracture group showed higher ICCs. Interobserver ICCs were especially lower in the control group. There were statistically significant correlations between CTI and DEXA (r=0.50~0.58, p<0.05). In the analysis of ROC curves, a mean threshold for CTI set a value of 0.54 (0.53~0.55), and mean sensitivity and specificity were 75.5% (69~79%) and 67.8% (65~78%), respectively. Conclusion: CTI showed reasonable reliability and correlation with DEXA results. CTI was a useful radiographic parameter to alert the surgeon to recommend referral for osteoporosis evaluation in elderly hip fracture patients.

        류마티스 관절염 환자에서 발생한 급속 파괴성 고관절증 -3예 보고-

        급속 파괴성 고관절증은 골관절염, 대퇴골두 무혈성 괴사, 류마티스 관절염 등에서 드물게 발생하며 원인이 명확하게 알려지지 않은 고관절 질환이다. 급속 파괴성 고관절증은 비교적 고령의 환자에서 임상적으로 극심한 고관절의 동통을 나타내지만 고관절 운동범위는 비교적 보존되어있으며 방사선학적으로 임상 증상이 발현한 후 6~12 개월 이내에 발생하는 급속한 대퇴골두의 파괴, 흡수 및 아탈구, 비구 관절면의 파괴, 50% 이상이나 연간 2 mm 이상의 관절 간격 소실, 미미한 골극 형성 등의 특징적인 소견들을 보인다. 감별 질환들로는 세균성 관절염이나 신경병성 관절염 등이 있다. 류마티스 고관절염은 전체 류마티스 관절염들 중 단지 5-15%에서 발생하며 관절 파괴 진행은 대부분 느리고 점진적이다. 저자들은 급속 파괴성 고관절증과 매우 유사한 소견을 보였던 3 예의 류마티스 고관절염을 경험하고, 이에 본 증례들의 임상 특징들, 병인론 및 치료에 대해서 문헌 고찰과 함께 보고하는 바이다. 저자들은 또한 심한 고관절 통증을 호소하는 류마티스 관절염 환자들에 대해서 반복적인 방사선 촬영이 필요하다고 생각한다. Rapidly destructive coxarthrosis may be caused by osteoarthritis, osteonecrosis of the femoral head and rheumatoid arthritis, but its etiology has not been clarified. Rapidly destructive coxarthrosis generally occurs in old age patients and the patients clinically show severe pain, but a relatively preserved range of motion. Rapidly destructive coxarthrosis is characterized by a rapid destruction, resorption and subluxation of the femoral head, destruction of the articular area in the acetabulum, above 50% or 2 mm/year loss of the joint space and minimal osteophyte formation. The radiologic changes are dramatic and they may mimic neuropathic or septic arthritis. We report here on three cases of rheumatoid arthritis that had acute destruction of the hip joint and rapid resorption of the femoral head, and we also review the relevant medical literature. We recommend taking repetitive radiographs for rheumatoid arthritis patients who suffer with continuing severe hip pain.
