1985년 11월부터 1974년 12월까지 한일병원 산부인과에서 복식자궁전적출술을 시술 받았던 510예를 임상적 고찰을 하였다. 수술후 감염율의 정의는 산욕열의 정의를 인용하였으며 수술후 첫 24시간을 제외한 감염율 Grade Ⅰ과 수술후 48시간을 제외한 감염율 Grade Ⅱ로 구별 비교하여 다음과 같은 성적을 얻었다. 1) 수술후 평균 입원일수는 9.63일이었다. 2) 적응증은 자궁근종이 127예(24.90%)로 가장 많았다. 3) 수술후 감염율은 GradeⅠ이 33.92%, Grade Ⅱ가 17.45%로 Grade Ⅱ에서 현저한 감소를 볼 수 있었다. 4) 응급수술을 받은 48예에서의 감염율은 Grade Ⅰ이 37.50%, Grade Ⅱ가 27.08%로 양 Grade 공히 높은 비율을 보였다. 5) 유치도뇨관을 2일이상 삽입하였던 69예에서의 감염율도 현저히 높았으며 Grade Ⅰ이 47.83%, Grade Ⅱ가 28.99%이었다. 6) 수혈과 감염율과의 관계를 보면 Grade Ⅰ이 35.07%, Grade Ⅱ가 17.54%로 전반적인 감염율과 거의 같았다. 이상의 성적으로 봐서 앞으로의 수술방법, 수술기재 등의 개선과 아울러 수술후 감염율이 높은 뇨로감염을 중심으로 하는 항생제 투여가 좋을 듯 하며 부인과 영역에서의 감염율은 수술후 첫 48시간을 제외함이 타당하다고 사료된다. A study was carried out to evaluate post operative hospital stay, age distribution, parity, indications and especially post operative morbidity in 510 total abdominal hysterectomies which had been performed from Nov. 1964 to Dec. 1975 at the Dept. of Obstet. and Gynec., Han Il Hospital. Post operative morbidity was evaluated according to the 1) emergency operation, 2) indwelling catheter, 2 days or more, 3) Hb. below 10.0gm% and 4) transfusion. Definition of post operative morbidity was quoted from the definition of puerperal morbidity except for a condition that author arbitarily divided morbidity in to grade 1 (excluding first 34 hours post operatively) and grade Ⅱ (excluding first 48 hours post operatively). Results obtained were as follow; 1) Post operative morbidity was 33.92%(173 cases) in grade Ⅰ and 17.45%(89 cases) in grade Ⅱ. 2) Post operative morbidity in emergency operation was 37.50%(19 cases) in grade Ⅰ and 27.08%(13 cases)in grade Ⅱ. 3) Post operative morbidity with indwelling catheter 2 days or more was 47.83%(33 cases) in grade Ⅰ and 28.99%(20 cases) in grade Ⅱ. 4) Post operative morbidity with Hb. below 10.0 gm% was 30.16% (19 cases) in grade Ⅰ and 22.22%(14 cases) in grade Ⅱ. 5) Post operative morbidity with transfusion was 35.07%(74 cases) in grade Ⅰ and 17.54%(37 cases) in grade Ⅱ. 6) The average post operative hospital stay was 9.63 days. 7) The peak incidence of age distribution was in 40-49 year of age. 8) The peak incidence of parity was 2-5 which occupied 63.14%(322 cases). 9) The indications of hysterectomies were variable, of which myoma uteri was 24.90%(127 cases), D.U.B. was 14.90%(76 cases), pelvic pain and dysmenorrihea was 10.59%(54 cases), ovarian cyst was 9.02%(46 cases) and Ca in situ was 3.73%(19 cases). 10) Of 510 total cases, no death occured.

본증의 빈도는 희유하나 신속한 집단과 치료가 따르지 못하면 예후가 매우 나쁘므로 평소 본증에 대하여 관심을 기울여야 되겠다. 그리고 예방이 중요하므로 분만 3기의 적절한 처치가 요구된다 하겠다. A Case of puerperal inversion of the uterus is presented and the literature is briefly reviewed.
