Pleomorphic adenoma is the most common benign tumor in the parotid gland and a parotidectomy with preservation of the facial nerve is a widely accepted treatment. We experienced a case of huge pleomorphic adenoma arising in the parotid gland with suspicious skin invasion. This present case was 67-year-old male presenting huge parotid mass (12×7) that has been present for 30 years and this tumor showed rapid-growing nature recently. The overlying skin surface was ulcerated with serous discharge and tightly adherent to the mass. The risk of malignant transformation of a pleomorphic adenoma increases with the duration of disease. The preoperative tentative diagnosis was carcinoma ex pleomorphic adenoma in the parotid gland. The initial pathologic diagnosis of fine needle aspiration biopsy was unsatisfactory smear. After parotidectomy, the histologic examination revealed that the parotid tumor was benign pleomorphic adenoma. We present a case of huge pleomorphic adenoma with skin invasion in the parotid gland with review of literatures.
청신경종양에서 Vascular Endothelial Growth Factor의 발현

Objectives : VEGF(vascular endothelial growth factor) is a potent key mediator of angiogenesis, which is necessary during tumor growth. The goal of this study was to determine expression of VEGF in acoustic neuromas and to examine a correlation with clinical characteristics. Methods : The VEGF expression was evaluated by immunohistochemical staining with monoclonal anti-VEGF antibody in 25 cases of the acoustic neuromas. Results : All tumors showed expression of VEGF in cytoplasm of schwannoma cell. The staining density revealed an insignificant correlation between VEGF expression and tumor size, symptom duration. Conclusions : This study suggests that VEGF in acoustic tumor has a role of both angiogenesis and mitogenesis in the growth of schwannoma.

Objectives This study was performed to assess patients’chief complaint and quality of life after acoustic neuroma surgery and compare the results of quality of life(QOL)between surgery and gamma knife surgery groups. Methods From 2001 to 2005, 35 patients underwent surgery via translabyrinthine approach in Otolaryngology and Head & Neck Surgery department and 37 patients underwent gamma knife surgery in neurosurgery department. An open-ended questionnaire about postoperative main complaint and the Short Form 36(SF-36) QOL instrument were used. Results The response rates about questionnaire in the surgery group and gamma knife surgery group were 71 %( 25 patients) and 54%(20 patients) , respectively. Dizziness was the most disabling symptom in patients after surgery. The most frequent symptom, hearing loss seems to be less important in the patients’perception. In patients who were operated on for acoustic tumor, the QOL showed significantly lower score than normal population. Age had significant influence on the quality of life results while tumor size had no significant effect. No significant difference in quality of life between the surgery and gamma knife surgery group was noted, although the latter showed slightly higher score than the former. Conclusions Because dizziness is the main complaint in patients after surgery via translabyrinthe approach, vestibular rehabilitation exercises are necessary. Patients who are due to surgery or radiosurgery should be informed about the potential risk of deterioration in their quality of life.

Background and Objectives: Sudden sensorineural hearing loss (SSNHL) is commonly defined as a loss of at least 30 dB in three contiguous frequencies occurring within 3 days. Systemic steroid administration has become the most widely accepted treatment option for SSNHL. Since viral infection and vascular compromise are considered specific causes of SSNHL, antiviral agents, anticoagulants, and stellate ganglion block have been used for its treatment, although the evidence of their effectiveness is weak. The present study evaluated the hearing recovery rate in the combination therapy group (systemic steroids, antiviral agent, anticoagulants, and stellate ganglion block) in comparison with patients treated with systemic steroids alone. Subjects and Methods: A total of 85 patients diagnosed with SSNHL were treated with combination therapy (group A, 46 patients) or systemic steroids only (group B, 39 patients). Hearing improvement was defined as a hearing gain of more than slight improvement using Siegel's criteria. All patients were treated with a 10-day course of systemic steroids (10-mg dexamethasone for 5 days, followed by tapering for 5 days). Acyclovir, heparin, and stellate ganglion block were included in the group A treatment regimen. Results: The overall rate of hearing improvement was 60.9% (28/46 patients) in group A, which was significantly higher than that (38.5%, 15/39 patients) in group B. The distribution of prognostic factors was not significantly different between the two groups with the exception of the degree of initial hearing loss, which was more severe in group A. Upon analysis according to prognostic factors, group A showed a better hearing improvement recovery rate than group B in patients with hearing loss >70 dB, age >41 years, dizziness, and early treatment (<1 week). Conclusions: Thus SSNHL patients treated with combination therapy have a higher likelihood of hearing improvement than those treated with systemic steroids alone.
