목적: 회전근 개 파열의 크기에 따른 극상근, 극하근의 지방 변성의 정도를 분석하였다. 대상 및 방법: 2004년 9월부터 2010년 9월까지 견관절 MRI를 시행한 541예 중 회전근 개 전층 파열이 있는 148예를 대상으로 하였다, 회전근 개의 파열은 MRI 상 시상면에서 전후 직경을 측정하여 크기에 따라 10 mm 미만을 한 개 그룹으로 하고 10 mm 이상 30 mm 미만을 5 mm 간격으로 4개 그룹으로 나누었으며 30 mm 이상을 한 개 그룹으로 하여 1에서 6까지 모두 6개 그룹으로 구분하였고 지방 변성은 Goutallier 분류에 의하여 구분하여 회전근 개 파열 크기와 극상근, 극하근의 지방 변성 정도와 관계를 보았다. 결과: 파열 크기가 증가할수록 극상근 및 극하근의 지방 변성이 증가하는 양상 보였다. 극상근에서는 회전근 개 파열의 크기가 15~19 mm 그룹과 20~24 mm 그룹 사이에서, 그리고 25~29 mm 그룹과 30 mm 이상 그룹 사이에서 타 그룹 간에 비해 지방화 변성이 두드러지게 심해지는 양상을 보였으며 극하근은 회전근 개 파열 크기가 25~29 mm 그룹과 30 mm 이상 그룹 사이에서 타 그룹 간에 비해 지방화 변성이 두드러지게 심해졌다. 10 mm 미만의 파열은 17예가 있었으며 이 중 극하근의 지방화 변성이 8예에서 발견되었다. 결론: 회전근 개 파열에 따른 극상근 및 극하근의 지방화 변성은 특정 파열 크기 이상에서 급속히 심해지는 양상을 보였다. 10 mm 미만의 소파열에서도 극하근의 지방화 변성이 수 예에서 관찰되었는데 이는 극하근이 소파열에서도 해부학적 또는 기능적으로 연관되었음을 시사한다. Purpose: To investigate the relationship between the size of a rotator cuff tear and the grade of fatty degeneration of the supraspinatus and infraspinatus muscles. Materials and Methods: From a database of 541 patients who underwent a shoulder MRI between September 2004 and September 2010, we enrolled 148 patients that had a full-thickness rotator cuff tear. The anteroposterior diameter of the tear was measured in sagittal views on an MRI. The patients were divided into 6 groups reflecting the anteroposterior diameter: group 1 (a width of <10 mm), group 2 (10 mm≤width<15 mm), group 3 (15 mm≤width<20 mm), group 4 (20 mm≤width<25 mm), group 5 (25 mm≤width<30 mm) and group 6 (a width<30 mm). Fatty degeneration was rated according to Goutallier et al. Results: Fatty degeneration in the supraspinatus and infraspinatus muscles increased with the severity of the rotator cuff tear. The supraspinatus fatty degeneration increased prominently as the tear size increased from 15~19 mm to 20~24 mm and from 25~29 mm to over 30 mm. The infraspinatus fatty degeneration increased prominently as the tear size increased from 25~29 mm to over 30 mm. Seventeen patients had tear widths under 10 mm and 8 patients among them had fatty degeneration of the infraspinatus. Conclusion: Fatty degeneration in supraspinatus and infraspinatus muscles increases prominently at specific tear sizes. The presence of tears under 10 mm with infraspinatus fatty degeneration indicates that the infraspinatus tendon may have an anatomical or functional effect even in small tears.

수부에 발생한 석회화 건염은 매우 드문 질환이다. 저자들은 수부의 단무지 외전건의 급성 석회화 건염 2예를 경험하였다. 환자들은 중수지관절 요측에 통증, 부종 및 홍반이 있었다. 저자들은 초음파하에 흡입술 및 스테로이드 국소주사를 시행하였으며 석고 고정술을 시행하였다. 증상이 호전되었으며 좋은 결과를 얻어 저자들은 문헌고찰과 함께 보고하는 바이다. Acute calcific tendinitis in the hand is a very rare disease. We experienced two cases of acute calcific tendinitis of the abductor pollicis brevis in the hand. Both of the patients experienced pain, edema and erythema on radial side of metacarpophalangeal joint of thumb. We successfully treated the condition with aspiration and steroid injection under the ultrasonographic examination and subsequently, applied the casts. An improvement of the symptoms was observed with good results. In the present work, we report these cases with appropriate review of literatures.
