Dieulafoy's disease, a vascular anomaly mainly in the upper stomach, is a rare but potentially life-threatening cause of upper gastrointestinal bleeding. Pathogenesis is still controversial, but the most accepted theory is that a persistent caliber vessel in the submucosa is exposed by a small mucosal erosion leading to massive bleeding. The bleeding site is usually within 6 cm of the esophagogastric junction in the cardia or fundus of the stomach. The treatment of choice is therapeutic endoscopy or surgery. The age of patients reported is mainly between 50 and 70 years, and patients of pediatric age are extremely rare. We are reporting a 5-year-old male patient who had Dieulafoy's disease which was diagnosed by emergency upper gastrointestinal endoscopy. Endoscopic finding was a nodular lesion with an adherent clot on the lessor curvature of the stomach 2 cm below the esophagogastric junction. Epinephrine and $Beriplast^{(R)}$ was injected in the lesion. On the second day after endoscopic sclerotherapy, the patient had recurred massive hematemesis and accompanying shock. So we performed gastrotomy and ligation. After the operation, he showed an improved general condition and was discharged at the 12th hospital day.

We have to understand various meanings and functions of tenses to catch the speaker`s significance and convey our thoughts. The objective of this thesis is to discover an effective method for teaching English tenses. As a first step for this research, it was necessary to study theories of tense, developed by grammarians, so that the best criteria for the effective teaching of tenses could be established and, therefore, understood. Next, the reading materials derived from three high school English textbooks were analyzed for having information on the frequency of each tense. As a third step, to measure students` understanding of English tense, a test sheet consisting of 35 questions was made, based on the results of the textbook analysis. Three suggestions can be made on the basis of the survey of students` understanding of tense. First, students need to recognize that time and tense are not always in accordance. Second, when teaching tense, students need to have the context containing the target tense for better understanding. Third, it can be helpful to teach the differences between two confusing tenses, comparing each of them. Fourth, teachers should offer students activities so that they can exercise their knowledge of tense in communication. The teaching methods developed by considering these suggestions follow four steps. It is expected that students can enhance their efficiency of learning while becoming more confident in making themselves understood in English with these four learning steps.
