Pediatric laparoscopic appendectomy is controversial particularly in complicated appendicitis. We evaluated the outcomes of laparoscopic appendectomy (LA) and open appendectomy (OA) in simple appendicitis and complicated appendicitis respectively. Since June 2004, initial LA has been our policy in all appendicitis including complicated ones. A total of 160 patients were included in this study, consisting of 80 OA (August 2001 - August 2003) and 80 LA (June 2004 - June 2006). We compared the operating time, the length of hospital stay, the length of antibiotics use, and the postoperative complications between LA and OA. In simple appendicitis (73), there were no differences between LA and OA. However in the 87 patients with complicated appendicitis, the operating time was longer in LA (64.8 min vs. 50.2 min) but the length of hospital stay was shorter in LA than OA (8.5 days vs. 9.6 days). There was one complication in simple appendicitis group and six in complicated appendicitis group (3 cases in LA, 3 cases in OA). There was no difference in the results of LA versus OA in simple appendicitis. Therefore for simple appendicitis, LA is recommended in consideration of the cosmetic effect (fewer scar). In complicated appendicitis, early discharge was an advantage and there were no differences in complications in LA despite a longer operative time. So we conclude LA can be considered as the first choice of treatment for all pediatric appendicitis including complicated appendicitis. To confirm our impressions, more well controlled randomized prospective studied need to be done.

The present study examined the mechanical properties of the friction welding of shaft made of SKH51 and SM45C, of which the diameter is 12mm. Friction welding was done at welding conditions of 2,000rpm, friction pressure of 104MPa, upset pressure of 134MPa, friction time of 0.5sec to 2.5sec by increasing 0.5sec, upset time of 2 seconds. Under these conditions, a tensile test, a bending test, a shear test, a hardness test and a microstructure of weld interface were studied. When the friction time was 1.0 second under the conditions, the maximum tensile strength of the friction weld observed to be 963MPa, which is 89% the tensile strength of SKH51 base metal and 101% of the tensile strength of SM45C base metal. When the friction time was 1.0 seconds under the conditions, the maximum bending strength of the friction weld happened to be 1,647MPa, which is 78% the bending strength of SKH51 base metal(2,113MPa) and 87% of the bending strength of SM45C base metal(1,889MPa). When the friction time was 1.0 seconds under conditions, the maximum shear strength of the friction weld was observed to be 755MPa, which is 92% the shear strength of SKH51 base metal and 122% of the shear strength of SM45C base metal. According to the hardness test, the hardness distribution of the weld interface varied from Hv282 to Hv327. HAZ was formed from the weld interface to 1.2mm of SKH51 and 1.6mm of SM45C. Upon examination it was found that the microstructure became finer along with increase of friction revolution radius.
Protein Kinase C alpha Protects Against Multidrug Resistance in Human Colon Cancer Cells

Multidrug resistance is the phenomenon by which, after exposure to a single chemotherapeutic agent, cancer cells evade the agent’s cytotoxic effects as well as become resistant to several classes of diverse drugs. ATP-binding cassette (ABC) transporters are a family of transporter proteins that contribute to drug resistance via an ATP-dependent drug efflux pump. P-glycoprotein (P-gp) is a prominent ABC superfamily protein encoded by the mdr gene which has the ability to mediate the cellular extrusion of xenobiotics and anticancer drugs from tumor cells. Exclusively expressed P-gp cells from the human colon cancer HCT15/DOX line showed resistance to doxorubicin while parental HCT15 cells treated with doxorubicin displayed typical signs of apoptosis. In order to verify the hypothesis that expression of MDR is controlled in part, by protein kinase C (PKC), expression patterns of different PKC isoforms were examined in both cell lines. Of the PKC isoforms evaluated, the membrane translocation and expression levels of PKC were strikingly increased in HCT15/ DOX cells. PKC reversed doxorubicin-induced apoptosis through the scavenging of ROS as well as inhibition of PARP cleavage. In addition, inhibition of PKC with Go6976, a specific inhibitor of classical PKC, led to reduced MDR expression and increased doxorubicin-induced apoptosis. Knockdown of PKC by siRNA diminished the protective effects of PKC for doxorubicin-induced apoptosis. These results suggested that over-expression and activity of PKC is closely as-sociated with the regulation of the MDR phenotype in human colon cancer HCT15 cells and provided insight into a new strategy for inhibiting doxorubicin resistance in human cancers.
