Propofol의 청각뇌간반응과 뇌교 청신경핵 부위 절편에서의 시냅스후전류에 대한 효과 사이의 상관성 연구

Background: Although there have been reports showing the changes of the auditory brainstem response (ABR) waves by propofol, no detailed studies have been done at the level of brainstem auditory circuit. So, we studied the effects of propofol on the postsynaptic currents of the medial nucleus of the trapezoid body (MNTB)-lateral superior olive (LSO) synapses by using the whole cell voltage clamp technique and we compared this data with that obtained by the ABR. Methods: 5 rats at postnatal (P) 15 days were used for the study of the ABR. After inducing deep anesthesia using xylazine 6 mg/kg and ketamine 25 mg/kg, the ABRs were recorded before and after intraperitoneal propofol injection (10 mg/kg) and the effects of propofol on the latencies of the I, III, and V waves and the I-III and III-V interwave intervals were evaluated. Rats that were aged under P11 were used in the voltage clamp experiments. After making brainstem slices, the postsynaptic currents (PSCs) elicited by MNTB stimulation were recorded at the LSO, and the changes of the PSCs by the bath application of propofol (100 μM) were monitored. Results: We found small, but statistically significant increases in the latencies of ABR waves III and V and the interwave intervals of I-III and III-V by propofol. However, no significant changes were observed in the glycinergic or glutamatergic PSCs of the MNTB-LSO synpases by the application of propofol (100 μM). Conclusions: Glycinergic or glutamatergic transmission of the MNTB-LSO synapses might not contribute to the propofol-induced changes of the ABR. (Korean J Anesthesiol 2009;56:552~8)

A pulse oximeter is used widely on account of its convenience and very few complications. We experienced a case of accidental burning by a pulse oximeter sensor. An 83-year-old woman underwent total hip replacement surgery. A pulse oximeter (Oxisensor II N-25, Nellcor Puritan Bennett Inc., USA) was placed on the left index finger. The patient complained of pain on her index finger 5 minutes after the operation began. A localized skin burn was found underneath the sensor of the pulse oximeter. The skin burn healed without any sequelae. (Korean J Anesthesiol 2007; 52: 228~30)

Spinal anesthesia in preterm infants offers a safe alternative to general anesthesia, especially if general anesthesia is not preferred because of coexisting diseases, such as bronchopulmonary dysplasia and recurring of apnea. But the single-shot technique of spinal anesthesia has some limitations because the duration of surgical anesthesia is approximately 60 min. Since some procedures may require more time, alternative regional techniques which provide more prolonged surgical anesthesia are needed. We present our experience with a combined spinal caudal anesthesia in a preterm infant. (Korean J Anesthesiol 2006; 51: 764~7)
전신마취하 제왕절개수술 40례에 있어서의 수술중 각성상태와 수술후 기억현상에 관한 연구

Background: Wakefulness during Cesarean Section after anesthetic induction has been one of the well known issues of anethesiologists. In literatures, they said different induction agents might bring different results. The author intended to evaluate intraoperative wakefulness and postoperative recall at Dankook University Hospital. Methods: Forty pregnant women (ASA class Ⅰ or Ⅱ) at term who underwent general anesthesia for cesarean section received thiopental. 4㎎/㎏. A pressure tourniquet inflated to 250 ㎜Hg isolated her one arm from the effects of muscle relaxant so that wakefulness during anesthesia could be assessed by asking the patient to move her hand. To assess wakefulness, the patient was commanded to squeeze the investigator's hand a specified number of times. If the patient promptly squeezed the investigator's hand the appropriate number of times, the command was repeated but a different number of times specified. Only if the patient responded correctly both times, then we noted as a positive response. This test was repeated at 1 munute intervals beginning 1 minute after thiopental injection through the delivery. The tourniquet was deflated after the delivery. The author evaluated the postoperative recall by asking about the verbal commands'. Results: 1) Five of the forty patients responded o the verbal commands. 2) Twenty nine patients made the reaching movements. 3) The patients who had followed commands and the others did not have recall. Conclusions: The author concluded that the conservative general anesthetic method for Cesarean Section should be changed to lower the incidence of the intraoperative wakefulness.

As the advant of capnography, anesthesiologists have been better able to measure and detect acute perioperative variations in end-tidal partial pressure of carbon dioxide(ET PCO_2). The auther observed an acute increase in ET PCO_2 after the release pf an extremity tourniquet during orthopedic surgery. We think a sudden increase in PCO_2 may have detrimental effects in certain patients, such as head-injured patients with an elevated intracranial pressure. If these patients undergo orthopedic and other procedures on the extremities with an arterial tourniquet in place, the increased PCO_2 can elevate intracranial pressure. The purpose of the present study was to investigate the effect of tourniquet release on ET PCO_2 measurements in patients undergoing extremity surgery requiring use of a tourniquet.
