직장 편평세포암은 대장직장암 1,000명당 0.25명에서 1명 정도 보고되는 드문 악성 종양으로 평균 54-60세에 발병하여성에서 다소 흔하게 발생하고 진행된 병기에서 진단된 경우 선암종과 유사한 생존율을 보인다. 대장 직장에 100개 이상의 선종이 있을 때 진단할 수 있는 FAP는 대장선암종의 전구 병변으로서 직장 편평세포암과 동반된 경우가 아직 보고된 바 없다. 저자들은 17세 남성인 FAP 환자가 간 전이를 동반한 직장 편평세포암으로 진단되어 다양한 치료 방법을 통해 적극적으로 치료했으나 진단 13개월 만에 사망한 1예를 경험했기에 문헌고찰과 함께 보고하는 바이다. Squamous cell carcinoma of the rectum is extremely rare, with an incidence between 0.25 and 1 case per 1,000 cases of colorectal carcinoma. In familial adenomatous polyposis (FAP), characterized by the progressive development of hundreds to thousands of adenomatous colonic polyps, unscreened patients and those who are not treated at an early stage of the disease have an extremely high risk of developing colorectal adenocarcinoma. A few reports of squamous cell carcinoma of the rectum have been published but none of the patients had FAP. Here, we report the case of a 17-year-old male with FAP who developed rectal squamous cell carcinoma. (Korean J Med 2015;88:335-339)
