        골수강내 교합정을 이용한 장관골 지연 및 불유합의 치료

        Delayed & non-union are common complications of the fracture of long bones. Many kinds of treatment modalities for delayed & non-union have been reported, but rigid fixation and bone graft have been the most common accepted method. The interlocking intramedullary nailing technique has advantages which the rigid internal fixation and bone grafting effect of reaming debris. We treated nine cases of delayed k. non-union of femur and tibia at the Department of Orthopedic Surgery, Seoul Paik Hospital from Jan. 1990 to Octo. 1992 and results were as follows. 1. Nine cases of the delayed & non-union were consisted by 7 cases of femur and 2 cases of tibia. 2. Causes of delayed k non-union were failure of implants in 4 cases, inadequate and inappropriate immobilization in 3 cases, and persistent gap in 2 cases. 3. Union was achieved in 7 cases and union rate is 77.8%

        교합성금속정을 이용한 상완골 간부골절의 치료

        The ideal treatment for humeral shaft fractures has been a topic of discussion for long times. Conventionally conservative methods like hanging arm cast, coaptation splint and functional brace were used. The operative treatment included plate and screws, intramedullary rods were used. The many known advantage of interlocking nails used in femur and tibia fractures were recently applied to humeral shaft fracture. Among them Seidel nail developed by Dr, Seidel in 1989 was inserted by splitting the rotator cuff and distal locking by expanding the distal tip of the nail through a long screw driver. This study is a preliminary report of first l0 cases of humeral shaft fracture which was internally fixed with Seidel nail during the period of September 1992 to September 1993. The findings are as follows. 1. 9 out of 10 cases(90%) achieved union. 2. Average union time was 10 weeks. 3. According to Neer's shoulder functional score, 8 patients(80%) achieved either satisfactory or excellent shoulder function postoperatively. 4. 2 complications occurred. One nonunion and one distal fixation failure. Since fixation of humeral shaft fracture, by Seidel nail can achieve high uruon rate and good shoulder function, it should be considered as one of many methods to treat humeral shaft fracture.

      • 인공 고관절술후 비구컵의 후기 해리와 마모

        고관절 전치환술 후 비감염성 해리는 가장 흔한 실패의 원인으로 보고되고 있으며 특히 비구컵의 해리는 10년 이후 급속히 진행되는 양상을 보여 해결책에 관심이 모아지고 있다. 저자들은 술후 10년 이후에 발생한 비구측의 후기 해리로 재대치술을 시행받은 환자 20명의 임상적, 방사선학적 측정과 현미경적 특징을 살펴봄으로써 후기 해리와 비구컵 마모의 연관성을 알아 보았다. Since the use of the charnley hip prosthesis, aseptic loosening of the component has been the most common causes of failure. Especially, loosening of acetabular cup has been progressed rapidly after 10 years postoperatively. So, the cause and resolution of late loosening of acetabular cup has been the topic. The 20 patients who have admitted for revision at Seoul Paik Hospital from Sept. 1992 to 1993 by late loosening (more than 10 years after postoperatively) of acetabular cup after being performed Charnley Low Friction arthroplasty were studied about the clinical and radiographic analysis, correlation of the real and radiographic measurement of wear and microscopic examination of the acetabular cup obtained by revision. The results were as follows : 1.The average time to revision was 13.2 years and showed unsatisfactory clinical results. 2.Acetabular loosening was found in 14 cases and all of them was involved all three zones and the most common site was zone III. 3.The average wear of polyethylene cup measured from radiographs by charnley and cupic(1973) was 2.60mm the real wear by Wroblewski(1985) was 2.73mm, and the average wear rate was 0.26mm year. 4.There was a significant correlation between radiological and real wear on measurement. 5.Cup wear was checked by SEM and showed abrasion, crack and delamination. 6.Granulation tissue was observed in all cases and polyethylene wear particle was found by microscope. 7.Mechanical and biological that reaction has been contributed to late loosening of acetabular cup, and afterwards invention of instrument to minimize biological reaction will be needed to prolong survivalship.
