        A Calibration Study of Therapeutic Ultrasound Equipment Output Intensity Accuracy

        Purpose: The principal objective of this study was to evaluate the power output of ultrasound in Korean clinics and compare the value with Korean and global standards. Methods: A total of 69 units were measured for ultrasound power output. The normal range of power output level was ±30% of the output set according to KFDA standards. Device model, manufacturer, ERA, and BNR were obtained via simple questionnaires. A portable ultrasound power meter was used for output measurement. Results: 37 machines, with reported ERA values, were assessed for power output per unit area. Of these machines, 13(37.14%) were considered to be compliant with US FDA standards at 0.5, 1.0, 1.5, 20 W/cm2 and 18 (51.43%) were considered within KFDA standards. The remainder of the machines were outside the standard error and evidenced irregular output levels, even though most of them were the same model. Conclusion: Appropriate ultrasound intensity is incredibly important for safety and effective use. Therefore, the KFDA standards regarding ultrasound may require revision in light of global standards, including BNR and ERA additionally,attention should be paid to regular calibration for safe use in clinical practice.

        누운 자세에서 호흡에 따른 요추분절의 운동학적 분석

        Purpose: The purpose of this study was to describe the influence of respiration on the segmental motion of the lumbar spine in the lying position. Methods: Twelve healthy females without a history of low back pain participated. Lumbosacral lordosis, intervertebral body angles, intervertebral body displacements, and anterior heights of the intervertebral disc of the lumbar spine were measured at inspiration, expiration and forced expiration in the supine and prone positions via fluoroscopy. Results: The results of lumbar kinematic analysis in the supine position according to respiration pattern were as follows. The L4/5 intervertebral body angle was significantly higher at forced expiration than at expiration (p<0.05). The L3/4 anterior height of the intervertebral disc was significantly higher at expiration than at forced inspiration and the L5/S1 anterior height of the intervertebral disc was significantly higher at inspiration than at forced expiration (p<0.05). There were no significant differences in the intervertebral body displacements and lumbosacral lordosis in the supine position (p>0.05). The results of lumbar kinematic analysis in the prone position according to respiration pattern were as follows. The L5/S1 anterior height of the intervertebral disc was significantly higher at inspiration than at forced expiration (p<0.05). However, there was no significant difference in the intervertebral body angle, the intervertebral body displacements, and the lumbosacral lordosis (p>0.05). Conclusion: These findings suggested that respiration can affect the intervertebral body angle and anterior height of the intervertebral disc in some segments. The results from this study serve as a step in the development of guidelines for lumbar kinematic analysis for lumbar breathing training.

        기립경사대 각도 증가에 따른 천골과 둔부 압력 변화

        Purpose: Although use of a tilt table is recommended in clinical practice, there are no published guidelines regarding pressure and inclination for tilt table use. The aim of the current study was to assess the changes of pressure on sacrum and buttock according to different inclination of the tilt table in healthy subjects. Methods: Thirty two healthy subjects participated in this study. Subjects were positioned supine on the tilt table and safety straps were secured across the chest, pelvic, and knee with sufficient tension to prevent the subjects from falling. Pressure and peak pressure of sacrum and buttock were measured using pressure mapping system with the tilt table standing at $0^{\circ}$, $15^{\circ}$, $30^{\circ}$, $45^{\circ}$, $60^{\circ}$, $75^{\circ}$, and $85^{\circ}$ inclination. Results: A significant decrease in the pressure of sacrum and buttock was achieved by increasing tilt table inclination (p<0.05): $0^{\circ}{\sim}15^{\circ}$ (8.16%), $15^{\circ}{\sim}30^{\circ}$ (8.02%), $30^{\circ}{\sim}45^{\circ}$ (11.61%), $45^{\circ}{\sim}60^{\circ}$ (16.18%), $60^{\circ}{\sim}75^{\circ}$ (16%), and $75^{\circ}{\sim}85^{\circ}$ (11.48%). A significant decrease in the peak pressure was achieved by increasing tilt table inclination (p<0.05): $30^{\circ}{\sim}45^{\circ}$ (9.91%), $45^{\circ}{\sim}60^{\circ}$ (19.24%), $60^{\circ}{\sim}75^{\circ}$ (19.93%), and $75^{\circ}{\sim}85^{\circ}$ (11.48%). No significant peak pressure change was observed in $0^{\circ}{\sim}15^{\circ}$, $15^{\circ}{\sim}30^{\circ}$ tilt table inclination (p>0.05). Conclusion: The results of this study showed that the pressure of sacrum and buttock were decreased according to increasing tilt table inclination in healthy subjects. Guidelines are needed in order to optimize patient safety and overall outcome for tilt table standing.

        15분간의 저측굴곡근 정적 스트레칭이 기립 균형에 미치는 단기 효과

        Purpose The purpose of this study was to examine immediate effects of 15 minutes plantarflexor static stretching in quite stance. Methods Twenty-nine subjects were measured static balance during 1 minute in quite stance with eye closed condition before and after 15 minutes plantarflexor static stretching. Static stretching range was limited from 15° to 20° dorsiflexion within comfortable range. Results The result of this study showed that postural sway significantly increased during 1 minute quiet stance after 15 minutes static stretching(p<.05). Before stretching, postural sway significantly decreased during 41-60 seconds compared to 0-20, 21-40 seconds(p<.05), After stretching, postural sway was decreased significantly over time 0-20, 21-40, and 41-60 seconds (p<.05), Conclusion The prolong plantarflexor static stretching may require biomechanical, neurological adaptations prior to walking or sport activities for safety.

        아기띠와 슬링 착용이 체간과 하지의 근활성 및 족저압에 미치는 영향

        Purpose:The Purpose of this study was to compare muscle activation and foot pressure on baby carrier and sling for baby care. Methods:Thirty one women subjects (mean age 23.2 years) participated in four static conditions: unloaded quite standing, carrying an anterior baby carrier, carrying a posterior baby carrier, and sling. The baby carrier and sling were loaded with baby model that filled 7.6kg loads. Surface electromyography was used to measure activity in the internal oblique, T4, L3, L5 paraspinal muscle, vastus medialis, biceps femoris, tibialis anterior, and gastrocnemius for four conditions. And foot pressure was measured by using MatScan system(Tekscan, USA). Results:The activation of Biceps femoris, T4, L3, and L5 paraspinal muscle were significant differences(p<.05), but other muscles were no significant differences in four conditions(p>.05). Right foot contact area and peak pressure of right mid foot area were significant differences in four conditions(p<.05). Conclusion:The results of this study indicate that the use of baby carrier of sling for baby care were influenced postural responses of young women. Further work is recommended to find out the influences of various assistive devices for baby care.

        Nervous system is clinically important, and involved in most disorders directly or indirectly. It could be injury and be a source of symptoms. Injury of central or peripheral nervous system injury may affect that mechanism and interrupt normal function. An understanding of the concepts of axonal transport is important for physical therapist who treat injury of nerves. Three connective tissue layers are the endoneurium, perineurium, epineurium. Each has its own special structural characteristics and functional properties. The blood supply to the nervous system is well equipped in all dynamic and static postures with intrinsic and extrinsic vasculation. After nerve injury, alternations in the ionic compression or pressures within this environment may interfere with blood flow and, consequently conduction and the flow of axoplasm. The cytoskeleton are not static. On the contrary, elements of the cytoskeleton are dynamically regulated and are very likely in continual motion. It permits neural mobility. There are different axonal transport systems within a single axon, of which two main flows have been identified : First, anterograde transport system, Secondly, retrograde transport system. The nervous system adapts lengthening in two basic ways. The one is that the development of tension or increased pressure within the tissues, increased intradural pressure. The other is movements that are gross movement and movement occurring intraneurally between the connective tissues and the neural tissues. In this article, we emphasize the biologic aspects of nervous system that influenced by therapeutic approaches. Although identified scientific information in basic science is utilized at clinic, we would attain the more therapeutic effects and develop the physical therapy science.

        Proprioception can be defined as a specialized variation of touch that encompasses the sensation of joint movement(kinesthesia) and joint position(static joint position sense). Several types of joint and muscular mechanoreceptors provide proprioceptive information for joint stability. Joint mechanoreceptors have been classified into four types based of activation characteristics : Ruffini endings, Pacinian corpuscles, Golgi tendon organlike endings, free nerve endings. The paper review the morphology, distrubution, function of mechanoreceptors in shoulder joint.

        Lumbar Spine Kinematics during Anterior and Posterior Pelvic Tilting in Supine and Prone Positions

        Purpose: The pelvic tilting exercise is a well recognized rehabilitation maneuver. However, little information is available on the changes of lumbar segmental motion during pelvic tilting. This study was conducted to measure the kinematics of the pelvic tilting exercise on the supine and prone positions via fluoroscopy. Methods: A total of 10 female subjects were enrolled. During anterior, neutral, and posterior pelvic tilting, radiographs were taken in each exercise via fluoroscopy (ARCADIS Orbic, Siemens, USA). Images were sent to the picture archiving communication system (PACS), and the digitized images were analyzed using LabVIEW software (National Instruments,USA). Lumbosacral lordosis and the intervertebral body angle, intervertebral disc angle, and intervertebral displacement were analyzed. Results: The results of lumbar kinematic analysis during three tilting postures in the supine and prone positions demonstrated that lumbosacral lordosis and the intervertebral body angle and intervertebral disc angle were significantly higher when the pelvis was tilted anteriorly (p>0.05). However, there was no significant difference between anterior and neutral tilting in the intervertebral disc angle at the L3/4 level in the prone position (p>0.05), and there was no significant difference among tilting positions in intervertebral body displacement in the prone position (p>0.05). Conclusion: This study provides scientific evidence about the pelvic tilting exercise in lumbosacral segmental motion. Depending on the pelvic tilting exercise, kinematic changes were demonstrated in both positions, especially in the supine position. It is suggested that the supine position is effective for mobility, but it should be used carefully for the LBP (Low back pain) patient with hypermobility.
