An understanding of the mechanism of healing has increased dramatically during the past decade. The phenomenon of epithelization is important in the normal resurfacing of a donor site or a partial thickness burn wound. This study was conducted to clarify the effect of epidermal growth factor in wound healing of human. Paired donor sites were created in 18 patients who required skin grafting for either burns or reconstructive surgery. One donor site from each patient was treated topically with silver sulfadiazine cream, and the other was treated with silver sulfadiazine cream containing epidermal growth factor(10ug/ml) (Human recombinant epidermal growth factor ; Sigma, St.Louis, Mo). The donor sites were photographed daily, and healing was measured with the use of planimetric anlysis. The donor sites treated with silver sulfadiazine containing epidermal growth factor had an accelerated rate in all 18 patients as compared with that in the paired donor sited treated with silver sulfadiazine alone. Combining data from all patients demonstrated reduced the average time to 100% healing by approximately 2 to 3 days (p<0.01). Light and electron microscopical study of biopsy specimens taken from the centers of donor sites three days after the onset of healing supported these results. In conclusion, epidermal growth factor accelerates the rate of healing of partial thickness skin wound.
협부 화상반흔의 교정에 있어서 Cervicofacial Flap의 이용

The face and its features have been the subject of poet and artistic endeavors throughout the ages. Because a person's face is hghly visible and difficult to camouflage, large burn scar on sheek is obvious to others and may be distressing to the affected individual. Particular skill is required of the surgeon reconstructing large burn scar on cheek, because the cosmetic result will have physical and psychological implocations for patient. Cervicofacial flap has much advantages for reconstruction of large burn scar on cheek as followings; 1. Satisfactory color and controur match. 2. Excellent result in function and appearance. 3. One stage reconstruction as simple procedure. 4. Inset scar lies along nasolabial fold and RSTL. 5. No requirement of delay procedure. 6. Good viability.

Deformities of orbital area, because of its being easily noticeable and difficult to be reconstructed symmetrically, are usually distressing to the patient not only in physiological aspect but also in psychological aspect. And so particular skill and attention are required for surgeons to correct their deformities by more effective reconstructive methods. There are wide varieties of case of orbital deformities, which are congenital anomalies, traumas and tumors. For to achieve the favorable results of orbital deformities, we must know anatomy of orbit, pathophysiology of deformity and characteristic of orbital structure. The author reviewed total 210 patients who were reconstructed at the department of plastic and reconstructive surgery in B.N.U.H. from 1981 to 1986.

Blepharophimosis is a congenital malformation involving the orbital region, usually associated with blepharoptosis and epicanthal folds. It has been known that it is transmitted as an autosomal dominant with essentially 100% penetrance. Although seen in both sexes, it occurs more commonly in the male and is expressed more commonly through the male. The surgical management of this condition is well covered in the current literature. Surgery should be undertaken early enough to prevent deprivation amblyopia, a possible sequele of severe blepharoptosis. The pedigree of our case, comprising four generations, consists of 27 individuals(15males and 12 females). Fifteen of the 27 persons are affected(7 males and 8 females), closely approaching the classical 50% ratio of an autosomal dominant characteristric. Chromosome studies have been negative and any metabolic anomaly has not been discovered.
협부재건에 있어서 Cervicofacial Flap과 Expanded Cervicofacial Flap의 비교관찰

Because of the complexity of check region, large scar on the cheek is obvious to others and may be distressing to the affected individual. Various surgical methods for cheek reconstruction have been used. Among these methods, the ideal surgical technique is local flap using cervicofacial region. We performed conventional cervicofacial in flap 42 patients and expanded cervicofacial flap in 29 patients. Retrospectively comparision between conventional and expanded cervicofacial flap was done. Indication, operative technique, status of scar, color and texture, neck contour, displacement, emotional expression and complication were compared. In conclusion, we can say that if possible, it is better to use the conventional cervicofacial flap for cheek reconstruction because of its superiority of the end result though there is technical difficulty.
삼각피판과 Fukuda씨 연골교정 방법을 이용한 매몰이의 치험예

Cryptotia has been one of the more rare congenital anomalies among Occidentals, but it is not so rare in Orientals. The cause of cryptotia has not been established. In cryptotia, the upper part of the retroauricular sulcus is absent, and the skin of the ear merges directly with that of the temporal region. By applying traction to the buried portion of the helix the contour of the upper portion of the helix and the sulcus becomes visible. When traction is released, the cartilage is once more hidden under the skin, and the classic form of cryptotia is regained. The principle of surgical correction is to add skin to the deficient retroauricular sulcus and cartilage plasty. Many number of methods for cryptotial were introduced. In some methods, however, thexe procedure did not completely correct the anatomic abnormality or anomaly was recurred. The correction of cryptotia was performed on 12 patients from July, 1991 to July, 1993. Our method was as follows : Triangular flap was designed above the superior pole of the auricle with the auricle held in an extended position. Cartilage plasty was carried out by Fukuda's method. After raising the upper portion of the ear by this procedure, the lack of skin is corrected by triangular flap. Patient follow up ranged from 6 months to 13 months with a mean of 10 months. We have treated 12 patients with cryptotial using this method and hav obtained good results in all of them without complication and hereby report the cases with discussion and review of literature.
