        디지털 흉부영상에서 주성분분석을 이용한 폐암인식

        흉부의 폐질환으로 폐암발생은 꾸준히 증가하고 있다. 일차적인 폐암진단 방법에는 흉부X선영상이다. 흉부X선영상 이용하여 폐암진단을 하기 위해서는 임상경험이 풍부한 의사가 필요하다. 그러나 풍부한 경험을 가진 의사라도 오진이 발생할 수 있고 이한 폐암의 조기진단과 생존률을 낮게 한다. 본 논문에서는 주성분분석을 이용하여 학습영상의 데이터베이스와 질병이 있는 흉부영상을 진단함으로써 컴퓨터보조진단의 기반을 마련하고자 한다. 이를 의사가 진단하기 전의 예비판독의 단계로 이용한다면 오진으로 인한 환자의 조기 진단률의 감소를 줄일 수가 있다. 실험은 정상흉부X선영상과 악성폐암인 기관지암(Bronchogenic Carcinoma)과 양성종양인 육아종(Granuloma)으로 실험하였다. 영상은 주성분분석 후 정상영상과 질환 영상의 고유영상을 추출하고 상호 비교한 뒤 인식효율을 비교하였다. 결과로는 정상영상과 질환영상간의 인식률은 높았으나 질환간의 인식효율은 정상에 비해 다소 떨어지는 것으로 나타났다. 흉부질환간의 인식효율을 높이기 위해서 관련 알고리즘에 관한 연구가 계속 이어진다면 컴퓨터보조진단에 좋은 연구기반이 되리라 생각한다. Risk of lung cancer among lung-related diseases has gradually increased during last decades. The chest digital radiography is the primary diagnosis method for lung cancer. Diagnosing lung cancer using this method requires doctors of ripe experience. Despite their experience there are often wrong diagnoses, which decrease early diagnosis and survival rates of patients. The aim of this study was intended to establish the base on the Computer Aided Diagnosis (CAD) by analyzing Image Recognition Algorithm using Principle component Analysis (PCA) and diagnosing patient's chest X-ray image. The database obtained through this approach enables a doctor to significantly reduce misdiagnosis during the early diagnosis stage, if he or she utilizes it as the preliminary reading step. Case studies were carried out using normal organ, and organs suffering from bronchogenic carcinoma and granuloma. A normal image and unique disease images were extracted after PCA analysis, and their cross-recognition efficiency were compared each other. The result revealed that the recognition rate was much high between normal and disease images, but relatively low between two disease images. In order to increase the recognition efficiency among chest diseases the related algorithms have to be developed continuously in the future study, and such effort will establish the resolute base for CAD.

        신부전환자의 신장이식 전후 골밀도변화 분석

        골밀도 감소에 의해서 발생되는 골감소증, 골다공증 등은 폐경 후 여성에게 유병률이 높은 질환이며, 골다공증 골절에 따른 사회적 의료비 상승으로 의학적 관심이 급증하고 있다. 신부전환자는 체내에 비타민D 합성 능력이 저하되어 칼슘 흡수가 감소하여 뼈의 섬유화가 발생되고, 골밀도가 감소하는 현상이 나타난다. 따라서 신부전환자는 신장 기능 장애로 그치지 않고, 골다공증과 같은 골밀도 감소에 따른 합병증에 노출되어 있다. 본 연구에서는 치료 중에 있는 신부전환자들의 골밀도 변화를 관찰하였고, 신장이식 전후에서 발견되는 골밀도 변화를 분석하였다. 조사대상은 부산 B종합병원 신장내과 내원환자 중 신부전환자 214명을 신장이식의 유무에 따라 골밀도의 변화를 세대별, 성별로 나누어 분석했다. 분석결과 신장이식 환자군에서는 골밀도가 유지되거나 개선되었지만, 비신장이식 환자군에서는 골밀도가 꾸준히 나빠지는 결과를 보였다. 신부전환자는 신장이식을 받는 것이 최선의 치료책으로 사료되며, 신장이식을 통해서 골밀도의 개선이 가능함을 확인하였다. 이는 추가적으로 예상되는 합병증을 예방하기 위한 자료로도 활용할 수 있다. Disease, such as osteopenia, osteoporosis, etc caused by reduced bone density are common to women after menopause and as the social medical cost increases due to osteoporosis fractures the medical interest in bone density reduction has increased. The bone density reduction is observed even for renal failure patients, due to their decreased ability to synthesize vitamin D which leads to bone fibrosis because of deficiency in calcium absorption. Thus renal failure patients not only suffer from kidney dysfunction, but also are exposed to complications, such as osteoporosis, due to reduced bone density. This research observed the change in bone density of patients receiving renal failure treatment and analyzed the change in bone density before and after kidney transplantations. Subjects were 214 renal failure patients at the department of nephrology Busan B General Hospital. The change in bone density was studied for subjects with and without kidney transplantation according to their age and sex. The research showed improvement or maintenance of bone density for subjects that received kidney transplantation, but showed a tendency of consistent decrease in bone density for subjects without kidney transplantation. Kidney transplantation can be considered as the best cure for renal failure patients, and this researched confirmed that bone density can be improved through kidney transplantation. Thus, this study can also be used as data for preventing complications due to renal failures.

        病院廢水의 特性과 處理에 關한 鼎究

        The hospital wastewaters have to be so disposed as to prevent disease and to protect water resources from hazardous substances; disinfectants, medicines, and chemicals. Polyvinylpyrrolidone-iodine complex (po꺼done-iodine) is widely used in the hospital as one of disinfectants. πùs study was carried out to manifest the effect of disinfectants in growth of activated sludge in treatment of the hospital wastewater by the activated sludge process. The results are as follow. 1. An average water quality of the hospital wastewater showed 7.2 in pH, 3.2 ppm in 00, 293.3 ppm in SS, 96.0 ppm in 800, 151 .1 ppm in COO, 0.4 ppm in povidone-iodine, 0.5 ppm in phenols, 5.4 ppm in surfactants, 1.6 ppm in o-phosphate, 4.6 ppm in NH3 -N, 249 x 104 counts/l00 mQ in coliform group organisms, and 1,369 x 101 counts/mQ in general counts of bacteria. And wastewater amounts discharged per bed was ca1culated 70 Q/d/bed. 2. In batch culture activated sludge process, each of cresol and podone-iodine was not effected in less than 0.1 ppm concentration, but the more concentration, the more inhibit the growth rate of activated sludge. In the mixture of two disinfectants, the growth was more inhibited the effect of single disinfectants. So that this reaction is considered as addition effect of two disinfectants. 3. The removal rates of the disinfectants by continuous cu1ture activated sludge process were 77 .6% in 0.4 ppm povidone-iodine, and in 80D was 85.6%. 4. It is desirable that the hospital wastewater is planed in order to be discharged to two system separately, sewer from life system and wastewater from medica1 system. From those results, it has been conc1uded that the hospital wastewater has to be treated safely by the activated sludge process.

        경남 마산시의 지역별 쾌적도 평가 및 환경쾌적자원 조사분석

        The purpose of this study is to research the environmental amenity factors of a city to devise the strategies to preserve the factors. For this study, the area of Masan city in Korea was chosen, and questionnaires, field studies, and statistical researches were used. Based on the results, the research showed the city surveyed had expanded by 1,069%, and the population of the city had increased by 272% from 1970 to 1995. In the city-life environment, the traffic was not smooth and the streets were not safe. The number of cars increased by 40,797% from 1970 to 1995, but the total length of roads increased only by 333%. The number of traffic accidents increased by 850%. Buildings were not preserved or maintained and did not have any traditional value. Additionally, buildings were not harmonious with environmental surroundings. Land allocation for utilization was not mixed well with tradition. In relation with the surroundings, there were not enough spaces for old people, and parking spaces were limited. In the field of public policy and finance, economic support and fundamental facilities were not enough. The survey of the household environment and the neighborhood environment showed that the room temperature, sunshine, air condition, and space were medial in the household environment. Public social facilities and entertainment culture had the lowest quality in the neighborhood environment. The city-life environment, the neighborhood environment, and the household environment affected the quality of life in order. The above results showed that cities should be planned upon sustainable development with the idea of emphasizing ecological harmony and considering people first, that citizen should be planned upon sustainable life by the harmony between people and the environment. Cities should be places where all people can live together pleasantly and cooperate with each other.

        유비쿼터스 컴퓨팅 환경에서의 DICOM 설계에 대한 연구

        유비쿼터스 컴퓨팅 환경에서의 최근 ICT의 급속한 발전으로 관련 산업은 놀라운 성장을 하고 있다. 따라서 의료 정보산업 관련 디지털 병원 시스템에서도 진료서비스 환경도 여러 형태의 모바일 장비 및 유선의 디바이스를 통한 시간, 장소에 관계없이 차별화된 진료 서비스가 가능하다. 그러므로 정보통신과 접목한 의료 정보 관련 솔루션의 도입은 병원 네트워크의 통합시스템이 주류를 형성하며 시너지 효과를 나타내고 있다. 현재 병원의 PACS 솔루션은 의료영상저장전송시스템으로 많은 병원들이 디지털 환경의 정보화를 위한 시스템으로 채택하고 있으며, 기존의 시스템을 무선통신, 인터넷 등의 영역으로 통합하여 유비쿼터스 컴퓨팅 환경의 개념이 형성되고 있다. 이런 시스템의 통합은 모바일 병원의 빠른 성장을 주도하고 있으며, 각각의 네트워크 및 원격 네트워크에 있는 진료 지원파트 및 임상의들은 획기적인 진료의 업무 프로세스를 요구하고 있다. 따라서 본 논문에서 설계하는 DICOM 엔진은 기존의 PACS DB 서버의 구조를 변경하지 않고 상호운용이 가능하며, 각각의 사용자의 요구에 응답하는 유?무선 통합의 진료지원 시스템이다. In the ubiquitous computing environments, ICT industries of current society are developed in enormous growth. Medicine or patients with mobile devices can access at any time, any place. The medical procedures at the patient bedside are out of the scope of current systems, which means that patient record and image data access during the medical visit or the execution, recording and confirmation of the medicine prescriptions, still do not enjoy computerized support. Today, the exchange of medical images and clinical information is well defined by DICOM and HL7 standards. The DICOM independent terminal equipment image access system was developed in which a DICOM Engine acts as the gateway between a PACS DB and user's terminal. Implementation system is compatible with most currently available Integration system models. This paper presents a software technology where the medical and nursing staff will be equipped with any device connected by wire and wireless to a central server that provides access to the electronic patient records and that will actively inform about tasks pending distribution. The prototype described in this article implements a medical images and structured reports server that makes the search and recovery of data stored in the DICOM standard possible.
