        Purpose: Management of soft-tissue defect after open tibial fractures includes immediate and repeated debridement, skeletal stabilization, and early soft-tissue coverage with muscle flaps. The purpose of this study was to evaluate the outcome of the free rectus abdominis muscle flap (RA flap) for treatment of open fractures of the tibia and to discuss its advantages compared with the latissimus dorsi muscle flap (LD flap) in poly trauma patients. Materials and Methods: We performed a retrospective review of 5 patients who had a severe (Gustilo IIIb or IIIc) open fracture of the tibia treated with RA flap from May 2003 to March 2006. All were men, and the mean age was 46.6 years (range, $28{\sim}68$). Three patients had combined injuries such as pelvic bone fractures, multiple rib fractures with hemothorax, and contralateral tibial fracture. All patients received RA flap within 7 days after trauma except two with established chronic osteomyelitis. Results: All flaps survived, and there was no marginal flap necrosis. During the follow-up period, there was no evidence of persistent or recurrent osteomyelitis. The size of RA flap ranged from $8{\sim}20\;cm$ in length and $6{\sim}10\;cm$ in width. The average time required for RA flap elevation was 32 minutes, which is shorter than LD flap. Flap elevation could be done in supine position which is essential in poly trauma patients. Conclusion: Although a wide variety of options are available, RA flap is regarded as an optimal method for coverage of soft-tissue defect of the open tibial fracture in poly trauma patients. LD flap is reserved for large sized soft-tissue defect which cannot be covered by RA flap.

        봉약침으로 치료한 드꾀르벵 병 1례 보고

        Objective : The aim of this study was to find out the therapeutic effect of the Korean Bee Venom theraphy in the treatment of patient with De Quervain's disease. Methods : As the effect of Bee Venom acupuncture about various articulation diseases observed, we tried to treat a case that was diagnosed De Quervain's disease by using Korean Bee Venom theraphy. Results : After Korean Bee Venom theraphy, 'VNRS' value decreased quite noticeably, and other symptoms have improved as well. Conclusions : Korean BeeVenom theraphy have shown fairly effective to De Quervain's disease. Furthermore, it shows considerable potential in its effectiveness to be utilized other fields of medicine practices.

        백두대간 지리산권역(육십령-천왕봉 구간) 마루금의 식물사회학적 유형분류 및 군집 특성

        본 연구는 국토의 생태축이라고 할 수 있는 백두대간의 지속가능한 보전과 관리방안 수립에 기여하고자 실시되었으며, 식물사회학적 유형분류를 통해 종조성을 파악하고 식생단위별 군집 특성을 파악하였다. 백두대간(육십령-천왕봉 구간) 마루금 주변을 대상으로 2020년 5월부터 9월까지 총 373개소의 산림식생조사를 실시하였다. 식생유형분류 결과, 9개의 식생단위로 구분되었으며 정성적 식생유형분류를 정량적 DCA 분석을 통해 검증하였다. 중요치 분석 결과, 아고산지대에 자생하는 종조성으로 묶인 식생단위 1-5는 구상나무, 가문비나무, 잣나무, 사스래나무 등 아고산식생 위주로 중요치가 높게 나타났으며, 해발고도가 높지 않은 마루금에서는 능선 위주의 지형 특성상 소나무, 졸참나무 등의 중요치가 높게 나타났다. 종다양도 분석 결과, 평균해발고도가 상대적으로 높은 구상나무군락(식생단위 1-5)이 다른 식생단위에 비하여 상대적으로 종다양도가 높게 나타났다. NMS 결과 및 환경인자와의 상관분석 결과, 식생단위 1, 2, 4, 5와 해발고도와의 양의 상관관계를 가지는 것으로 나타났다. 본 연구는 소나무군락, 비목군락의 저해발 지역 식생부터 구상나무군락, 가문비나무군과 같은 고해발 지역 식생까지 모두 포함되었고 습윤한 지역의 계곡부 식생이 포함되지 않은 백두대간의 능선 식생을 특성을 나타내었다. In this study, the forest vegetation structure in the Maruguem (ridge line) area from Yuksipryeong to Cheonwangbong, Baekdudaegan, was analyzed using vegetation classification, importance values, species diversity, and NMS. Data were collected using 373 quadrates in a Braun-Blanquet vegetation survey conducted from May to October 2020. Vegetation was classified into nine vegetation units, which were verified using DCA analysis. Vegetation units 1-5, which were grouped by sub-alpine region, showed high importance values, mainly for sub-alpine vegetation, such as Abies koreana, Picea jezoensis, Pinus koraiensis, and Betula ermanii. In Maruguem, which is not high above sea level, importance values for species such as Pinus densiflora and Quercus serrata were high due to the topographical characteristics of the ridge. The A. koreana community (vegetation unit 1-5), which had a relatively high average elevation, had higher species diversity compared with that of other vegetation units. According to NMS analysis, for abiotic environmental factors, there was a positive correlation between vegetation units 1, 2, 4, and 5 and elevation. Overall, this study describes all low-elevation area vegetation (P. densiflora and Lindera erythrocarpa) to high-elevation area vegetation (A. koreana and P. jezoensis) as well as the characteristics of the Baekdudaegan ridge vegetation that did not include valley vegetation.

        요추관 협착증과 골밀도와의 상관성에 관한 임상례 보고

        Objective : The aim of this study was to evaluate association between lumbar spinal stenosis, one of degenerative diseases, and bone mineral density. Methods : We monitored 9 lumbar spinal stenosis patients in women above 50 years, visiting outpatient clinic of the Jaseng Oriental Medicine Hospital between January 5 2006 and March 31, 2006. They were diagnosed by radiologist ist after taking Magnetic Resonance Imaging(MRI). The cases were Investigated the bone mineral density using Dual Energy X-ray Absorptiomerty(DEXA). and then Picture Archiving Communication System(PACS) were used to assess correlation between lumbar spinal stenosis and bone mineral density. Results : 1. In comparison of the spinal canal area and lumbar spine 2 level bone mineral density, the data showed a significant result 2. The data, between spinal canal area and lumbar spine 1-2 level bone mineral density, indicated a significant result. 3. Also, the result of comparison between spinal canal area and the lowest value of bone mineral density showed significance. Conclusions : It showed that there is a statistically significant correlation between lumbar spinal stenosis and bone mineral density.

        백두대간(깃대배기봉-죽령 구간) 마루금 주변의 산림식생구조

        본 연구는 백두대간 깃대배기봉~죽령 구간의 마루금을 대상으로 식물사회학적 식생유형분류를 실시한 후, 일치법과 CCA 분석을 통해 분류된 식생단위와 입지환경인자와의 상관관계를 파악하고, 종다양도 분석, 군락유사도와 DCA 분석을 통해 식생단위간 유사도와 군락의 통계적 거리를 고찰하며, 주요 목본 수종의 종간상관관계 분석을 통해 산림식생구조를 파악하기 위하여 2018년 4월부터 동년 10월까지 총 298개소의 산림식생조사를 실시하였다. 식생유형분류 결과, 총 13개의 식생단위로 구분되었으며 최상위 수준에서 신갈나무군락군으로 분류되었고, 신갈나무군락군은 산뽕나무군락, 터리풀군락, 쇠물푸레나무군락, 개벚지나무군락, 신갈나무전형군락으로 세분되었고, 산뽕나무군락은 물참대군과 산뽕나무전형군으로 세분되었으며, 터리풀군락은 박새군, 새군, 터리풀전형군으로 세분되었으며, 쇠물푸레나무군락은 소나무군, 일본잎갈나무군, 쇠물푸레나무전형군으로 세분되었다. 물참대군은 다시 가래나무소군, 황벽나무소군, 말발도리소군, 물참대전형군으로 세분되었다. 입지환경인자와 일치법과 CCA를 통한 상관관계 분석 결과, 해발고도에서 터리풀군락(식생단위 6~8)은 1,200 m 이상에서 분포하는 것으로 나타났고, 그 외 식생단위에서는 1,200 m 이하에 분포하는 것으로 나타났으며, CCA 분석 결과에서도 터리풀군락은 해발고도와 양의 상관관계를 가지는 것으로 나타났다. 종다양도 분석 결과, 터리풀군락이 종다양도가 높은 것으로 나타났다. 군락유사도 결과, 쇠물푸레나무군락(식생단위 9~11)의 유사도가 상대적으로 동질하게 나타났으며, 산뽕나무군락(식생단위 1~5)과 새군(식생단위 7)은 상대적으로 이질적인 것으로 나타났다. DCA 분석 결과에서도 산뽕나무군락과 터리풀군락의 통계적 축의 거리가 가장 먼 것으로 나타나 군락유사도 분석 결과와 일치하는 경향을 보였다. 주요 목본 수종의 종간상관관계 분석 결과, 친수성 수종간에는 양의 상관관계가 인정되었고, 참나무류인 신갈나무와 양수인 소나무, 일본잎갈나무 등은 음의 상관관계가 인정되었다. This study was conducted to analyze forest vegetation structure in the Marugeum (Ridge) area of Gitdaebaegibong to Jukryeong, Baekdudaegan. Data were collected in 298 quadrates through a Braun-Blanquet vegetation survey from April, 2018 to October, 2018. Forest vegetation was classified into 13 vegetation units. A Quercus mongolica community was divided into Morus bombycis, Filipendula glaberrima, Fraxinus sieboldiana, Prunus maackii unit and Q. mongolica typical unit. The M. bombycis unit was further classified into a Deutzia glabrata group and M. bombycis typical group. The F. glaberrima unit was subdivided into a Veratrum oxysepalum group, Arundinella hirta group, and F. glaberrima typical group. The F. sieboldiana unit was divided into a Pinus densiflora group, Larix kaempferi group, and F. sieboliana typical group. The relationship between vegetation units and environmental factors was studied through coincidence analysis and CCA. The F. glaberrima unit (VU 6~8) was distributed by elevation above 1,200 m and other vegetation units were distributed below 1,200 m. Results of the CCA analysis showed that the F. glaberrima unit distribution is positively correlated with elevation. As a result of species diversity, the F. glaberrima unit was higher than other vegetation units. A similarity index analysis revealed that the F. sieboldiana unit (VU 9~11) was relatively homogeneous, and the M. bombycis unit (VU 1~5) and A. girta group (VU 7) were relatively heterogeneous. A detrended correspondence analysis determined that the distance between the statistical axes of the M. bombycis and F. glaberrima units was the greatest, which is consistent with the analysis of the similarity index. As a result of interspecific correlation of major woody plants, hydrophilic species were positively correlated, and a negative correlation was found between Q. mongolica and intolerant species such as P. densiflora and L. kaempferi.

      • 전이형 외측 상완 피판술을 이용한 주관절 연부조직 결손의 피복

        Purpose: Soft tissue defect can occur on the posterior aspect of the elbow after trauma or fracture fixation. To cover the defect and maintain elbow functions, various flap surgeries including latissimus dorsi muscle flap, lateral arm flap and radial forearm flap can be performed. We present the clinical results of transposition lateral arm flap for coverage of the elbow defect and discuss the cause of posterior soft tissue necrosis after fracture fixation. Materials and Methods: Two patients who had posterior soft tissue defect of the elbow after open reduction of the fractures around the elbow were treated with transposition lateral arm flap. The mean size of skin defect was 20 $cm^2$. The flap was elevated with posterior radial collateral artery pedicle and transposed to the defect area. Donor defect was covered with split thickness skin graft. The elbow was immobilized for 1 week in extended position and active range of motion was permitted. Results: All two cases of transposition lateral arm flap survived without marginal necrosis. The average range of motion of the elbow was 10~115 degrees. Mayo elbow performance score was 72 and Korean DASH score was 23. Conclusion: When elbow fractures are fixed with three simultaneous plates and screws, skin necrosis can occur on the posterior aspect of the elbow around olecranon area. If the size of skin defect is relatively small, transposition lateral arm flap is very useful option for orthopaedic surgeons without microsurgical technique.

        경기지역에서 대두콩에 대한 피복재배효과

        This study was carried out to investigate the effects of early planting and polyethylene film (P.E) mulch on early maturing soybean of Korean native cultivar Kyeonggi No. 114 in the Kyeonggi area. It was planted four times at the ten days interval from April I to April 30 in 1985 and 1986. The average soil temperature was 1-5$^{\circ}C$ higher under the polyethylene film mulch condition than the conventional cultural practice condition. Soil moisture content also was higher under P.E mulch condition. Days to emergence was shortened 5-16 days and days to flowering was shortened 3-5 days. The fresh pod yield was increased 12% more at the planting date of April 1 and April 10. Income was increased 70, 83, more at the planting date of April 1 and April 10 under the polyethylene mm mulch conditions.

        교통사고로 인한 편타성 손상 환자의 어혈방(瘀血方) 치료 효능에 관한 임상적 연구

        Objectives : The purpose of this study is to investigate the effect of eo-hyeol bang for patients with whiplash injury caused by a traffic accident. Methods : 60 patients were divided into two groups, Group A and Group B, of 30 patients each. Group A was treated with eo-hyeol bang twice per week for four weeks. Group B was treated with general acupuncture treatment twice per week for four weeks. Results : Between the two groups there was no significant difference in the VNRS score and the Neck disability index. Also, the study did not show significant difference between three variations of motion pattern induced pain and four stages based on the trauma duration of Group A. Conclusions : For patients with whiplash injury, eo-hyeol bang has not proven to be more effective than general acupuncture treatment.

      • 경상북도 공,사유림 내 산림습원의 식생구조와 분포

        본 연구는 경상북도 안동시, 영주시, 봉화군, 청송군 네 지역의 공·사유림 내 산림습원의 식생구조와 분포를 파악하기 위하여 보존가치가 있는 산림습원의 발굴 및 보전, 향후 체계적인 관리방안을 수립하는데 기초자료를 제공하기 위함에 있다. 완충구역은 습지의 종류, 지형 등을 고려하여 핵심지역 경계로부터 20~50m로 설정하였다. 판별된 습지는 산림습지조사메뉴얼에 의거하여 식생조사를 위한 방형구 면적 설정을 실시하였다. 출현하는 식생은 기본 현황과 더불어 식물사회학적 조사방법에 의거, 우점도와 군도를 중심으로 산림청의 야장 양식에 따라 기록하였으며 현존식생도 및 투영도와 단면도를 작성하였다. 지역별 산림습원 조사구로는 안동시가 43%로 가장 많았으며 봉화군이 30%, 청송군이 22%로 그 뒤를 따랐고, 군락유형으로는 버드나무 군락이 68.2%로 가장 높은 개소수를 차지하였고 물오리나무 군락이 9%, 개키버들 군락이 9%로 그 뒤를 이었다. 산림습원 생태계를 보호하기 위해 국가 습지보호지역으로 지정, 보호를 위한 감시 및 단속활동, 지속적인 모니터링 및 현명한 관리를 위한 다양한 방안을 모색하였다. To provide the basic information on the excavation, conservation, and systematic management plan for preservation of valuable forested wetlands, a field survey was analyzed at public and private forests in Gyeongsangbuk-do containing Andong-si, Yeongju-si, Bonghwa-gun, Cheongsong-gun, 2013. Considering the type of wetland, topography, etc., the buffer zone was set at 20∼50m from the core area boundary. Distinguished forested wetlands implemented quadrat for vegetation survey on the basis manual of forested wetlands investigation. To understand vegetation structure of wetlands, height, DBH(diameter at breast height), DI(Dominance Index), sociability and constancy were surveyed and the projection diagram and charts were drawn. As for number of Regional Forested wetlands recorded, which average values of 43% was composed of Andong-si while the 30% was composed of Bonghaw-gun. In terms of community type, highest value were manifested in Salix koreensis community at 68.2%, Alnus sibirica community and Salix integra community at 9%. Such several management schemes as the designation for national wetland conservation area, continuous monitoring, cautious surveillance and wise management were suggested to implement successful conservation.
