이상운동을 동반하는 파킨슨병은 퇴행성 뇌 질환으로 병에 대한 중증 및 진행 정도를 의사 및 숙련된 전문가의 소견에 의존하여 진단하는 것이 대부분이다. 하지만 보다 효과적인 치료를 위해서는 정확하고 객관적인 진단이 필요한데, 최근에는 웨어러블 장비의 발달과 더불어 병의 증상을 객관적으로 정량화하고자 하는 시도들이 있다. 본 논문에서는 파킨슨병의 대표적 운동장애인 진전(tremor), 운동완서증(bradykinesia), 경직(rigidity)을 기준으로 이상운동증상을 정량화하는 방법들에 대하여 분석하였다.

파킨슨병의 운동적 특징 및 공학적 정량화 방법에 대한 조사

Parkinson’s disease (PD) is a neurodegenerative disease that causes abnormal motor symptoms such as tremor, bradykinesia, stiffness, and postural instability. The number of patients with PD is increasing worldwide. Therefore, it is of great importance to delay the progression of PD through early diagnosis and treatment or to provide appropriate treatment and continuous assistance to PD patients through regular examination to determine changes in their symptoms and the rate of disease progression. In this paper, we investigated motor disease symptoms (tremor, stiffness and bradykinesia) caused by PD and methods that quantified it by engineering. In addition, 67 papers (24 in Korea and 43 abroad) directly related to quantification methods of Parkinsonian motor symptoms were investigated through domestic and international searches of published research papers. As a result of the search, it was confirmed that acceleration / angular velocity (46%) and electromyography (22.7%) were most widely used both in domestic and international research for quantification of movement disorders (tremor, stiffness, bradykinesia) and UPDRS (Unified Parkinson’s Disease Rating Scale) was the most widely used scale (78%, N = 55) to compare and confirm results of such methods. 파킨슨병은 진전, 운동성, 강직 및 자세 불안정과 같은 이상운동증을 유발하는 신경 퇴행성 질환으로 전 세계적으로 환자 수가 증가하고 있다. 병의 초기에 진단 및 치료를 통해 질병의 진행을 지연시키거나 정기적인 검사를 통해 환자에게 적절한 치료를 제공하는 것이 중요하다. 본 연구에서는 파킨슨병으로 인한 이상운동증(떨림, 강직, 운동완서)과 이를 공학적으로 정량화한 방법들을 알아보고자 한다. 발표된연구 논문의 국내 및 국외 조사를 통해 파킨슨병 운동 증상의 정량화 방법과 관련이 있는 67건의 논문(국내 24건, 해외 43건)을 조사하였다. 조사 결과, 국내외 연구에서 가속도/각속도(46%) 및 근전도(22.7%) 를 사용하여 각각의 운동 장애(진동, 경직, 운동완서)를 정량화 한 방법이 가장 많이 사용되었음이 확인되었다. 또한, Unified Parkinson's Disease Rating Scale은 이러한 방법의 결과를 검증하는 데 가장 널리사용되었다(78%, N=55).
