        Copper T 자궁내장치의 방광 내 전이 후 발생한 방광결석

        An intrauterine device (IUD) is one of the most commonly used contraceptive method in the world. One of the major complications of intrauterine contraception is the perforation through the uterine wall into the pelvic or abdominal cavity. The incidence is 0.9 people per 1000. However, bladder perforation is even more rare. It happens mostly at the time when it was inserted. The symptom varies from no symptom at all to low abdominal pain, massive bleeding, et cetera. We report a case with displaced intrauterine device in bladder, producing the bladder stone which was managed with cystoscopic lithotripsy and intrauterine device removal, followed by a review of the literature.

        복강경 수술을 이용한 대망 낭종의 치료

        Mesenteric cysts are uncommon, and they are histopathologically classified as pseudocyst, mesothelial cyst, lymphangioma and omental cyst. We report a case of a 27-year-old female with a huge cystic mass arising from greater omentum. Patient complained the symptoms of abdominal distention due to massive ascites and intermittent pelvic discomfort. The diagnostic examinations including ultrasonography and computed tomography were performed and identified a huge, intraabdominal cyst. Laparoscopic resection of cyst was done with no clinical and symptomatic evidence of adverse event till 12 months. The diagnostic role of sonography and CT in ascites with unknown etiology is emphasized. Minimal invasive surgery showed comparable result to open exploration.

        Uterus didelphys with unilateral obstructed hemivagina is indeed a very rare congenital anomaly. Uterus didelphys with obstructed hemivagina and ipsilateral renal agenesis usually presents after menarche with progressive abdominal pain during menses secondary to hematocolpos. The method of choice for diagnosis is magnetic resonance imaging (MRI). MRI can do much for the early diagnosis and the prevention against further complications of this condition because it can demonstrate the mullerian duct anomaly complicated with obstructed hemivagina in detail and even ipsilateral renal agenesis. A greater awareness of the syndrome of uterus didelphys, obstructed hemivagina, and ipsilateral renal agenesis should lead to its prompt diagnosis, allowing for early and appropriate surgical intervention as well as decreased long-term morbidity. Early accurate diagnosis after menarche followed by excision and marsupialization of the blind hemivagina offers complete relief of symptoms and preserves reproductive potential. We report a case of uterine didelphys with obstructed hemivagina and ipsilateral renal agensis with a brief review of the literature.

        폐경 여성의 골밀도에서 제지방의 중요성

        연구목적: 본 연구는 폐경 여성에서의 신체 구성 (body composition) 중 체지방, 제지방과 골밀도 (bone mineral density, 이하 BMD)의 상관관계를 알아보았다. 연구재료 및 방법: 본 연구는 가톨릭대학교 의과대학 강남성모병원 산부인과 폐경기 클리닉에서 모집한 지원자 중 건강한 폐경 여성 55명을 대상으로 신장, 체중을 측정하고 체질량 지수를 산출하였다. 골밀도와 체조직 분포는 최근에 개발된 양에너지 방사선 흡수계측기 (Dual energy X-ray absorptiometry, 이하 DXA)를 이용하여 요추와 양측 대퇴골의 골밀도를 측정하고 체지방량과 제지방량을 측정하였다. 결과: 대상 환자의 평균나이는 57.19±6.13세 (범위 50∼77세), 평균 신장은 156±5 cm, 체중은 57.82±5.80 kg, 평균 체질량 지수는 23.73±2.42였다. 폐경 여성에서 남성형 지방량의 비율이 여성형 지방량보다 높게 나타났다. 체지방의 경우 남성형 지방과 총체지방량이 양측 대퇴골의 골밀도와 음의 상관관계를 보였다. 좌, 우측 팔, 다리 몸통 각각과 양측 팔, 다리, 몸통, 여성형 체지방량은 골밀도와 상관관계가 없었다. 제지방량의 경우 좌, 우측 팔 각각과 양측 팔의 경우 골밀도와 상관관계가 없었으나, 좌, 우측 각각의 다리, 몸통, 양측 다리, 몸통의 경우 골밀도와 유의한 상관관계를 보였다. 결론: 신체 지방량 중 남성형 지방량과 총지방량의 비율이 낮을수록 골밀도가 높고, 제지방량중 다리, 몸통의 제지방량이 높을수록 골밀도가 높다는 결론을 얻었다. Objectives: To investigate the importance of lean body mass for bone mineral density (BMD) in postmenopausal women. Methods: The BMD of the lumbar spine and both femurs was measured with dual-energy X-ray absorptiometry (DXA) in 55 healthy postmenopausal women, ages 50∼77 years. Candidates were selected from a group of patients who visited the menopausal clinic at the Catholic University of Korea. Lean body mass and fat mass were assessed in both arms and legs and in the trunk. The BMD of the lumbar spine and both femur necks and trochanters was measured using DXA. Results: The mean patients age was 57.19±6.13 (range 50∼77) years, height was 156±5 cm, weight was 57.82±5.80 kg, and body mass index was 23.73±2.42. In postmenopausal women, the percentage of android fat mass (42.84±0.12%) was higher than that of gynoid fat mass (39.57±4.37%). There was a negative correlation between the BMD of both femurs and both android fat mass and total fat mass. The fat mass in the arms, legs, and trunk and the gynoid fat mass showed no significant correlation with BMD. The lean body mass in the legs and trunk had a significant correlation with the BMD of the lumbar spine and femur, although the lean body mass of the arms did not. Conclusion: These findings suggest that BMD increases as android and total fat mass decrease, and that BMD increases as the lean body mass of the legs and trunk increases.
